NY Podiatrist Uses Maggots to Heal Diabetic Ulcers

Dr. Ravi Kamble, a Brooklyn, N.Y.-based podiatrist who specializes in wound care, first got interested in maggots as a child. His father, a general surgeon, would tell him how poor people in India often had wounds infested with the larvae. "He would tell me as an aside that by removing the maggots you were actually doing a disservice to the person because maggots actually helped clean the wound. That fact always stuck in my head," Kamble says.

During medical school, the young Kamble even proposed a research project using maggot therapy -- and "was laughed out of the room" by his professor.Then, late in 2005, Kamble treated Ramon Rivera, a 54-year-old diabetic with a serious foot ulcer -- gangrenous, with a severe infection reaching down to his bone. "I was consulted late in the process, so everyone was already assuming below-the-knee amputation," Kamble says. The proposed cut line was already drawn on Rivera's leg.

About 500 sterile fly larvae were placed directly on the wound and then covered. They were removed a few days later after they'd fed on the dead tissue, then new maggots applied. The therapy worked. "Before treatment, you could see all the way down to the bone. Now he has tissue forming over it," Kamble says. Tissue, according to the textbooks, is not meant to regrow over exposed bone. "It shows you that textbooks are never completely correct," Kamble says.

NY Podiatrist to Be Honorary Reader at Ground Zero

Dr. Arthur Gudeon has been chosen by New York City Mayor Bloomberg's office to be an honorary "Reader" of twelve of the names at the Ground Zero Ceremonies this Tuesday (9-11). Gudeon played a key role coordinating podiatric volunteers during the eight months post 9-11-01 at the St. Paul’s Cathedral, which is adjacent to the former World Trade Center

The names are read alphabetically, and Gudeon’s 12 names are in the "H's" (beginning with James J. Hobin). This commemoration will be televised on Channel 2 (WCBS) from 8:30am 'til 12:30pm, and on Channel 9 (WWOR) from 8:30 'til noon. The actual ceremony begins with a moment of silence at 8:46 am.

PODIATRISTS AND 9/11

Editor’s note: The following note from the editor was published in PM News on 9/12/01 and circulated via the Internet throughout the world.

It is just after 3 AM here in New York City and I simply can't sleep. Looking out the window of my bedroom on the 17th floor, there is an unobstructed view of the Manhattan skyline. I search again for the once familiar sight of the two tall white buildings that marked the lower end of the skyscraper-filled island. All I see are some eerie search lights of the rescue crews.

Like everyone else, I have been in shock all day, particularly since I witnessed in real-time the morbid views of the second plane hitting the World Trade Center and the sequential collapse of both towers.

The gamut of emotions runs from shock, fear, sadness, to anger. Barely one half-century after the Holocaust, despite all our technological advances, man remains uncivilized.

The realization sets in that the terrorists who committed this crime could have and still could set off a nuclear bomb, killing millions. The safety and security we have taken for granted in America is and will never be the same.

My phone has been ringing all day. Family and friends call to determine that everyone is safe. My mom calls to tell me my brother was scheduled to be at the WTC at 9 AM and was at the subway station when the first plane hit. Had the plane hit five minutes later, he would have been on an elevator headed for certain death.

Unfortunately, many thousands were not so lucky, including many employees of Blue Cross /Blue Shield, which only a few years ago moved from midtown to this location. There will many tears shed as the names of those murdered in this horrific disaster become known, many unnecessary funerals to attend. So many innocent lives lost for no reason. So many families destroyed.

Later this morning, my wife and I will donate blood and attempt to do what we can in this time of crisis. We know the entire podiatric community will collectively pray for those affected by this catastrophe and provide whatever support is needed.

WOMEN IN MEDICINE

More Women Choosing Surgical Residencies

As physicians celebrate "Women in Medicine Month" in September, it appears that surgery is beginning to lose its status as one of medicine's male-dominated fields.

Almost 40% of residents entering general surgery residencies in 2007 were women, double the number a decade ago. In the overall surgical work force, the number of women has risen to about 14% in 2005, up from about 9% in 1995, according to the American Medical Association.

Surgery's leaders once feared that more women graduating from medical school would mean fewer surgical residents in the pipeline, as women pursued more lifestyle-friendly medical careers. In fact, the number of residents matching in surgery dipped in the late 1990s and early 2000s, before rebounding in the last few years.

Query: Platelet Rich Plasma Therapy

I have been using platelet rich plasma therapy for plantar fasciitis with good results for the past six months. Orthopedic physicians in my area are also using it for epicondylitis in the elbows. I am interested in trying the therapy for chronic Achilles tendonitis. Is anyone else is using this therapy and has anyone used it at the Achilles tendon area and their results. There is some literature out of Europe stating using it in the Achilles area is accepted and safe but it is limited.

I was also named in a suit in which I was a resident, and was listed in the operative report, even though I was merely observing the surgery. Like Dr. Bernstein, by the time I was served with the summons, I had relocated out-of-state. I contacted the attorney who represented the surgeon who actually performed the surgery and asked him to have me removed from the case.

I suggest you contact the attorney representing your ex-partner and ask him to intervene.
