Examining Patients' Gait and Shoe Wear Often Very Telling: NY Podiatrist
"The feet are the foundation of your body. Ill-fitting shoes, or those that don't provide enough support, affect the entire body, whether through fatigue or by creating an anatomical imbalance," explains podiatrist Miguel Cunha, DPM, owner of Gotham Footcare. One of the first things Dr. Cunha asks his patients to do is to place their shoes on the examination table, because signs of wear are very telling.

"As we all walk, our feet bend inward a little, which means the center of the foot lowers. But if they bend too much, you'll do more damage to the inside of the shoe and the heel," says Cunha. This causes the foot to roll inward, which leads to various problems, such as plantar fasciitis, Achilles tendonitis, pain in the inner knee, hip, and lower back. To find the cause, Cunha often performs gait analyses. Sometimes they are performed in the office, but even a simple video of the patient walking toward and away from the camera can reveal irregularities in the gait or stance. These indicators help determine whether the pain is caused by body mechanics or ill-fitting shoes.

She then completed a three-year residency in foot and ankle surgery at Virginia Tech Carilion School of Medicine in Roanoke, VA. Afterwards, she completed a fellowship in reconstruction, revision, and limb preservation of the foot and ankle at the Virginia Foot and Ankle Fellowship in Richmond, VA.

Medicare Part B Payment Trends for Skin Substitutes Raise Major Concerns About Fraud, Waste, and Abuse
Medicare Part B expenditures for skin substitutes have skyrocketed over the last 2 years, surpassing $10 billion annually by the end of 2024. In this data snapshot, OIG analyzed Part B claims data, Medicare Advantage claims data, and manufacturer-reported sales data for skin substitutes in 2023 and 2024.
Several aspects of Part B spending trends raise serious concerns, including: 1) large increases in the amount of product billed for each enrollee (particularly in home care), 2) a massive gap in spending between Part B and Medicare Advantage, 3) providers’ propensity to shift to more-and-more expensive products, and 4) potential fraudulent schemes uncovered by OIG. The findings in this snapshot illustrate the critical need for payment reforms that address fraud, waste, and abuse in Medicare skin substitute billing. Read the Full Report.

Here we are almost to the middle of September and we have no report from AACPM on total admissions. You would think by now that the class size for each of the schools is solidly established. White coats have been distributed, and ethical administrators have led the classes in the Hippocratic Oath. But no, that is not the case. I contacted some of the schools and asked what the class size for the class entering in 2025 was. A number of schools sent me admissions material, even though the official date for applications is later in the Autumn of 2025 for the class entering in 2026. One school answered the number of students was, “in flux.” Imagine that.
What does that mean? Let’s suppose that you applied to podiatry school to enter now, late summer of 2025, and were rejected. It could be for a low-grade point average or test scores. You could now suddenly receive a “do-over letter” where the school re-considered your application and now after extensive re-evaluation by the dean and treasurer, you can now...

RE: New Chicago-Based Health Care Fraud Section

A recent announcement from the OIG detailed a new Chicago Section of Health Care Fraud, which will work with the local US Attorney General and Health Care Strike force to pursue those who engage in healthcare fraud. This new section will not limit itself to the Chicago metro area, but actually investigate and prosecute those within their jurisdiction who cross state lines. While prosecuting those who endanger the welfare of the Medicare Trust Fund, let's be mindful of the honest providers who may get caught up in these types of investigations. Often, eager prosecutors wish to gain their merit badges by investigating and indicting the "ham sandwich". On the other hand, it is refreshing to note that the OIG finally is responding to investigations of:
New suppliers who rev up just too quickly with a large volume of claims for one specific product. Existing suppliers with acceptable number of claims for a specific product which suddenly revs up their claim numbers.
Suppliers who submit a high number of claims for those products on the government radar (e.g., back and knee braces). Whistleblower complaints coming from employees who suspect their employer is engaged in fraud.
The take home message is to continue to provide DME while improving your documentation supporting medical necessity for any DME you prescribe or dispense.
