AT THE COLLEGES - PART 1
NYCPM Class of 2020 Students Receive White Coats
 
The NYCPM Class of 2020 has been oriented, celebrated, and made to feel at home, but last night, class members received the first real symbol of their entry into medical education: their white coats. At the White Coat Ceremony, held September 6 at the New York Academy of Medicine, Vice President of Academic Affairs and Academic Dean Michael J. Trepal, DPM  made a few opening remarks and introduced Joseph Cataldo (2018), President of the Student Council. Cataldo described to the first-year students the impact of a white coat on a patient. “A patient sees a healer and a helper; a patient sees someone compassionate and understanding; a patient sees someone who can listen.”

Remarks of the keynote speaker, R. Daniel Davis, DPM, President of the American Podiatric Medical Association (APMA), went directly to the crux of the matter: “To the public, the wearer of the white coat is someone who practices medicine.” At the end of Dr. Davis’ remarks, he received a crystal apple with thanks from NYCPM. After the speeches, one by one, the 86 members of the Class of 2020 received and donned their white coats in front of families and friends, and took the Hippocratic Oath.

Western University of Health Sciences College of Podiatric Medicine Assistant Professor Rebecca Moellmer, DPM, is serving as the 2016-17 president of the California Podiatric Medical Association (CPMA).

The Duchess of Cornwall Meets With Podiatrists and Patients
 
The day of meetings followed a busy day yesterday for Camilla (the Duchess of Cornwall) who enjoyed a rare night out with her son Tom Parker Bowles and husband Charles. She started her day at a visit to the Royal London Hospital for Integrated Medicine in central London where she met with podiatrists and patients.

Dressed in a chic blue dress emblazoned with yellow flowers, the Duchess, who is Patron of the College of Podiatry, was treated to a guided tour of the hospital. During her day out, she met with Dr. Tariq Khan, who examined a young patient alongside the royal, as well as showing her the benefits of therapeutic insoles.

FL Podiatrist Apppointed as Health Officer for Charlotte County 
 
The Florida Department of Health has announced Dr. Frank Lepore as the new health officer for the Florida Department of Health in Charlotte County. An accomplished podiatric surgeon, Dr. Lepore joins the department after 14 years at his private practice in Middle Village, New York.

Town Wants to Build Garage on PA Podiatrist's Property
 
A multi-level parking garage with 328 spaces is being proposed at West Liberty and Potomac avenues in Dormont. The garage would displace some local businesses, John Snyder, a podiatrist who owns the property, told the council Tuesday.

MACRA UPDATE
CMS Will Give Providers Flexibility on MACRA Requirements
 
The CMS on Thursday announced it will allow providers to choose the level and pace at which they comply with the new payment reform model aimed at emphasizing quality patient care over volume. Next year, eligible physicians and other clinicians will be given four options to comply with new payment schemes such as the Merit-based Incentive Payment System (MIPS) or an alternative payment model such as accountable care organizations.
 
In the first option offered Thursday, any data reported will allow providers to avoid a negative payment adjustment. The goal is to ease providers into broader participation in the following two years. The second option allows providers to submit data for a reduced number of days. This means their first performance period could begin later than Jan. 1 and that practice could still qualify for a small payment if it submits data on how the practice is using technology and how it's improving. The third option is for practices that are ready to go in 2017. The final option is to participate in an advanced alternative payment model such as a Medicare Shared Savings ACO.

Most physicians have heard of locum tenens and many have worked alongside locums doctors in a hospital or clinic. However, the majority of doctors are unaware of the benefits of locums or even how these temporary assignments really work. 
 
The reasons physicians do locums vary but here are seven primary motives for working locum tenens:
 
3. More time for family — Full-time locums work when they choose to work. If they want to take an extended break for a long family vacation, they can do it. Many doctors say that locums also allows them to be more present at home because they're not worrying about paperwork, staffing decisions, or department meetings.

Query: Physical Therapy Billing
 
We have a physical therapist on staff. What is the proper channel to bill physical therapy services? Do we use the NPI of the actual physical therapist administering the service? Or, do we use the NPI of the DPM? Should we use the physical therapist's NPI as rendering, and DPM as supervising? Does anyone have recent experience with the utilization of actual physical therapy services within their DPM practice?

Response: We have performed PT/OT billing for over 20 years, though not as part of a podiatry practice. The physical therapist should be enrolled under your group and the billing go out with the PT as the rendering provider (with the physical therapist's NPI, of course) and the DPM as the referring provider. Be sure whoever does your billing is aware of the required modifiers, particularly for Medicare. All other aspects pertain, of course - medical record documentation, Rxs, etc.

RE: Individual Focused Meetings With Staff

It is so easy to forget what keeps our office ticking. Being caught up in all the craziness of running a podiatry practice, it is quite easy to forget that our most valuable asset is our staff. Recently, I began to realize how complacent I had become in this arena. So I immediately shifted gears and set up a forty-five minute meeting with each staff member. With pen in hand, I took notes to show that I was not just listening, but that I really heard their thoughts. The idea was to get their assessment and ideas for improvement in all aspects of our practice. The meetings were done outside the office. By opening my mind and ears to them, I learned an amazing amount of useful material to improve the office on all levels, as well as learning a few things about myself.
 
What differentiated these meetings from all the past meetings with staff was that there was focus and a significant amount of time spent with each staff member; questions were being asked, and these meetings were done outside the office in a neutral environment. Their input was sought on specific topics such as patient satisfaction, treatment protocols, and outcomes, reducing overhead, internal external and internal marketing, ways to help their productivity and growth in the office; and very importantly, how I could be a better doctor and boss.
 
The real work begins after the meeting when it is imperative to take action on some of the great ideas brought forth in the areas of the practice that clearly need improvement. Through the process of these focused meetings with our staff, I learned more ways to improve our office on many levels more than any other process in my 26 years of private practice.
