AT THE COLLEGES
NYCPM Class of 2019 Receive White Coats

107 students in the New York College of Podiatric Medicine’s (NYCPM) Class of 2019 recently received their white coats before an audience of family, College faculty, and Trustees in a ceremony at the New York Academy of Medicine. Prakash Krishnan, MD, FACC, Assistant Professor of Medicine and Director of Endovascular Interventions at the Icahn School of Medicine at Mount Sinai and a recognized leader in the field of cardiology, delivered a keynote address in which he explored the history and significance of the short white coat, and gave advice drawn from his own experiences as well. An important note from his experience as a cardiologist is that the podiatrist provides a valuable first step in the diagnosis of vascular disease.

NYCPM Class of 2019

NYCPM’s Class of 2019 is an accomplished and diverse group, according to Dean for Student Services and Enrollment Management, Lisa Lee. 47% female and 53% male, the students come from 22 states and three countries. In addition, students in the class have received over $400,000 in scholarships from the College. The white coats were donned in front of proud parents and the College community, followed by administration of the Hippocratic Oath, led by Michael J. Trepal, DPM, Vice President of Academic Affairs and Dean.

High Heels Put Pressure on Nerves and Cause Pain: DC Podiatrist

QUERIES (NON-CLINICAL)
Query: Replacing Collimator on Excel X-Ray Machine

I have a 26 year old Excel XR machine and need to replace the collimator beam light source. Does anyone have an idea how to do it without having to pay for a service visit? I took the cover off and can’t see a bulb.

QUERIES (OFFICE ISSUES)
Query: Best Place to Find Part-Time Receptionist

I am searching for an office receptionist looking to progress to an office manager role. This can mean a former office manager who is now looking to reduce days/hours and show that he or she is able to help develop a new office. Aside from word-of-mouth, websites such as Indeed, Monster, or Craigslist, and recruiters, where are other good places to look, especially for someone for a part-time role?

RESPONSES/COMMENTS (NEWS STORIES) - PART 1
From: Dennis Shavelson, DPM

Dr. Ulett’s discussion seems to classify thick, discolored, and onycholysed toenails as onychomycosis when, in fact, onychomycosis is but one type of onychodystrophy. Better advice may be that the etiology of onychodystrophy is multifactorial.

Infection of toenails by saprophytic fungi requires an already weak or compromised nail due to precursor repetitive microtrauma (RMT) from shoes, underlying biomechanical pathology, metabolic disease, immunodeficiency, chemotherapy, and dermatological conditions such as psoriasis. Healthy toenails do not serve as acceptable hosts for onychomycosis. Finally, pure fungal toenails are rarely painful. Painful nails are more likely due to bone spurs, ingrown toenails, ballistic trauma, shoe pressure, and RMT due to toenails rubbing against shoes.

The evidence proves that treating cases of onychodystrophy as onychomycosis, in spite of negative cultures and concomitant signs and symptoms, results in low clinical cure rates and recurrences. Our patients may be better served with a multimodal treatment plan.

Dennis Shavelson, DPM, NY, NY

RESPONSES/COMMENTS (NEWS STORIES) - PART 2
From: Robert Kornfeld, DPM

As we can see from this article about out-of-network physician fees, BIG BROTHER is going to step in and CONTROL the amount that these doctors can charge. Does anyone else see the inherent danger here? In a free market, such as the one we supposedly have, consumers get to decide the value of goods and services and then decide for themselves how to spend their money. The only thing that should be "required" is complete disclosure as to whether or not a doc is out-of-network and, if so, what are his/her fees.

As an out-of-network doctor who charges patients at the time of their visit and has NOTHING to do with third-party payments, I am appalled that any insurance company can fund a bill to control what any physician charges. Why are we not controlling the costs of ALL goods and services? How dare any Mercedes Benz dealer charge more for their cars than Hyundai? I believe that ALL cars should cost no more than a Hyundai, regardless of the engineering and cost of production.

I also think it is time we stepped in and put a stop to butchers charging more for filet mignon than for hamburgers. How dare they! It is time to put cap costs on EVERYTHING! As a matter of fact, all homes should cost no more than $100,000, regardless of where they are or the square footage. Enough price gouging. Oh yes, and by the way, execs at all health insurance companies should be paid minimum wage! Who is with me?
