VA Podiatrists Underwriting Orthotics for Local First Responders

Charlottesville police Lt. Tom McKean walked on the same pair of orthotic inserts for almost 20 years. Knowing it could cost as much as $500 to replace them, he kept using them. He knew they’d need to be replaced eventually. Thanks to a new program coordinated by the Charlottesville Police Foundation, McKean recently got a new pair for much less than he would typically pay. Richey & Co. Shoes and 10 participating podiatrists are taking the brunt of the cost, charging police officers, firefighters, and EMS workers only $75 for their orthotics.

Dr. Bryan Snyder, a podiatrist at Albemarle Family Foot and Ankle, said the amount of time police and firefighters spend on their feet participating in strenuous activities generally means more of them need orthotics. Snyder said the inserts can be used to correct current problems and to prevent future ones. Snyder said he and fellow podiatrists in the area were happy to reach out and offer the orthotics at reduced costs. “It’s a way to say thank you,” he said.

LA Podiatrist Leads Sock Collection Drive

At the beginning of 2013, Dr. Erin Mathews, a podiatrist at NOLA Podiatry Group, established a goal of gathering and distributing "Socks for the Sockless." It started out as a small effort among her patients, and it rapidly grew into an overwhelming success, much in part to her hometown community and alma mater A.M. Barbe High School. A total of 6,239 pairs of socks were donated. 2,500 pairs of ankle crew socks were sent overseas as part of care packages to our military. The remaining socks were recently distributed to men, women, and children at the St. Jude Community Health Fair held at the New Orleans Treme Center.

"It is very rewarding to know that such a small inexpensive item can make a real difference in the lives of others," says Dr. Mathews. She did not realize that so many children were without school socks and relates she will focus her future collection efforts on them.

UK Podiatrist to Climb Mount Mt. Kilimanjaro

In just one weeks time, Horsham based podiatrist Kathryn Tyler will swap spending her days examining and treating the feet of patients in her busy Clockhouse podiatry practice, and will instead be using her own feet to climb the highest freestanding mountain in the world, Kilimanjaro.

The trip in total will last 11 days, with 7 of those on the mountain itself; the rest is spent visiting local schools and projects supported by the Village Education Project Kilimanjaro (VEPK), who over the last 20 years have achieved remarkable results in enabling local children to receive a proper education, and also providing vocational training for young adults to help them develop the skills necessary to earn a decent living wage.

Your idea of HBO is an excellent one. A vascular consult is highly recommended, if for nothing else than to have another name in the records, but more importantly to assess the level of potential outcomes. I think this young man/woman has a great chance of recovery. Young arteries and patients are extraordinarily resilient. Very interesting case. Please keep us posted on the outcomes. Serial photos would be amazing!

It may already be too late, but I would try to instigate the use of oral and topical vasodilators. Using HBOT is a great idea, but keep in mind that O2 is a vasoconstrictor. The blood fluid will be richer in oxygen. If you couple the oxygen therapy with a vasodilator, you will see a much quicker and maximized response. I do not like the response quoted from the ortho doctor. I hope his licensing organization is aware of his callous concern for this patient. Topically, I would rub in 1/2 inch of nitroglycerin pasted to the affected foot's arch, tid. Hold for SBP under 100mm. Discuss  with the patient's PMD about using oral medication, like Procardia.
 
My question to the patient's parent, "how did the child tolerate the significant pain that would accompany this travesty?"

What type of procedure was performed? Is it a possible compartment syndrome after the surgery? Sickle cell? Vasculitis? All these need to be worked up.
 
1- Given the proximal aspect of the incision site, refer the patient to vascular to assess the PT artery - it may have been injured in the surgery both at the ankle and its branches in the midfoot.
2- PT nerve block for sympathetic blockade.
3- Nitro paste/patch for vasodilation in the area.
4 - Warm compress behind the knee.
5 - An anecdotal suggestion would be to use Metanx to help NO production.

With all due respect, Dr. Wertheimer, what problems are you talking about that need fixing by podiatric med-mal insurance companies? The only problems I see today are those caused by CPME, AACPM, and APMA. They allowed new schools to open and they knew or should have known that there would not be enough residencies.

Dr. Wertheimer, your voice seems to be more for division of our profession instead of unification. We don't need that.

We ARE the foot and ankle experts. No medical profession comes close to the level of what we learn and can do within the foot and ankle - medically or surgically. Our training is far superior in the management of the lower extremity than that of any other medical professional - again, both medically and surgically. If one chooses not to be a surgical practitioner, there is plenty of pathology and plenty of feet to go around.

Questioning the above is what's wrong with podiatry. There is no question, only a failure to realize who and what we are, and what it is we can do with our training.

I was shocked, dismayed, and offended to open my PM News last night to see the above headline, and pictured below Dr. Emily Splichal. I can't imagine what reason PM News would have for publishing this. I realize you were only quoting another news source, but it was a sensationalist piece, describing her as "a stilleto loving guru", "Dr. Legs", and "midriff baring" in her "glamour shot." It appears to me from the article Dr. Splichal has apparently already been through more than enough without publishing this for all of her colleagues to see. Why the original news source singled her out in the first place, when there are countless women who are victims of domestic violence every day, is beyond me as well. Please show Dr. Splichal, who is a smart, progressive podiatrist, and in my opinion, a new pioneer in our field, some respect, and respect her privacy as well.
