WUCPM Establishes Dr. Thomas Rambacher Endowed Scholarship
Western University of Health Sciences’ College of Podiatric Medicine (CPM) recently received a $120,000 gift to establish the Dr. Thomas Rambacher Reconstructive Surgery Endowed Scholarship. This gift was donated by Thomas Rambacher, DPM. He is hopeful that by opening the scholarship to students from West Virginia, Kentucky, Ohio, and the L.A. basin, it will bring diverse perspectives to CPM.

Dr. Rambacher received his doctorate in podiatric medicine from the Ohio College of Podiatric Medicine. As part of his primary podiatric medicine residency at the Cleveland Clinic Foundation, Dr. Rambacher started working as a resident in Southern California, where he has been practicing ever since. He is board-certified by the American Board of Foot and Ankle Surgery and certified in wound care by the American Professional Wound Care Association.

IL Podiatrist Discusses Tendonitis in Runners
“Running is an amazing form of cardiovascular exercise, but because it’s a high-impact sport that involves repeated trauma to the feet, everyone from long-distance runners to casual joggers is at risk for developing painful and debilitating foot conditions such as tendonitis,” says Bryce A. Paschold, DPM, a board-certified foot and ankle surgeon and a Fellow Member of the American College of Foot and Ankle Surgeons.

Runners can be sidelined with tendonitis if they ignore the warning signs of this overuse condition. There are several forms of tendonitis that affect the Achilles and other areas, and all are treated with rest, icing, strengthening, stretching, and anti-inflammatory medications, and sometimes with orthotics and physical therapy. Because overzealous training is usually the cause, especially among beginners, it’s important to ramp up mileage and speed gradually. A running coach can tailor your training plan to your current fitness level.

UWSMPH Department of Surgery Welcomes WI Podiatrist
The Department of Surgery at the University of Wisconsin School of Medicine and Public Health (UWSMPH) has welcomed Jacob Hagenbucher, DPM, as an Assistant Professor in the Division of Vascular Surgery. As a podiatric surgeon, Dr. Hagenbucher’s responsibilities will include clinical care and teaching in the clinical setting. His inpatient responsibilities will be primarily based at Unity Point-Meriter. Additionally, Dr. Hagenbucher will be responsible for teaching medical students, residents, and fellows within the context of his clinical practice.

8 Tips for Avoiding Employee Handbook Crises
Many medical practices have an employee handbook, or employee policy handout, but fail to update them on a regular basis. There have been a large number of changes to employment law over the past couple of years. Here are some items you should be sure to include:

Does your handbook prevent employees from discussing their pay with other employees?
The NLRB has ruled that companies cannot ban employees from telling other employees how much they are being paid. These rulings are generally aimed at large companies with many employees, but your handbook must be consistent with the current law in your state as well as the latest rulings from the NLRB. The motivation behind this ruling is that when employees have a complaint against management, they need the right to collect data to support their grievance.
As a business owner, you should review the handbook yearly, at a minimum, and consult your company attorney to review any changes to make sure you have adequately protected your company from employee lawsuits. Do not just go on the internet and get a do-it-yourself handbook. Your handbook should be common sense guidance to your employees about your specific business, and these handbooks that are available online are not written to match your individual business.

Chart what your exam finds. Treat what you find. Bill what you treat. Document it all. Play by the real rules, not your wallet. Also, don't cheat yourself or the payor, whomever it might be.

I have two questions:
1) My patient has 2 painful mycotic toenails which I debride and bill Medicare 11720. Because I'm a nice guy (or a fool) I then trim the other 8 non-mycotic or non-dystrophic nails at no charge to the patient or insurance. One of the posts seem to indicate that I specifically need to put in the chart that I am not billing for the trimming for the other nails as charity or whatever. I usually just put down I have trimmed the other nails but I don't make a point of saying I am not charging for them. Do I have to document that I am not charging?
2) My patient has 2 painful mycotic nails which I debride on a regular scheduled basis 11720. Because I have done such a great job of debriding the nails, on the next appointment the nails are still mycotic and have grown back, but are not painful. If I didn't see the patient on a regular basis, the nails would become painful. Do we have to wait for the patient to experience pain before they are able to have the debridement covered? If so, that seems to be against our oath to keep the patient pain-free and unfair to the patient that they must have pain before we can treat them.

I have felt the recent impact of insurances pouncing on DME dispensing. It would be wonderful if doctors didn’t have to pull money out of their pockets for patients in “hopes” the items will be covered. Too often we find out a new requirement buried layers deep in the initiatives or one of the carrier regulations has changed leaving us holding the bag. Then there is the inadvertent “T” not crossed that hits us in the wallet.
I think it would be wonderful if a doctor could submit all paperwork as a prior authorization and a determination be made then and there. However, I have not seen this in reality. With other carriers and Medicare, denials even after prior authorization are all too common. I’ve appealed items to the level of the ALJ, and had actual apologies from the judge after they noted every last requirement had been met from day one.
This is no surprise. The system is designed to discourage physicians from helping their patients with appropriate modalities when it takes monies from the program coffers. The public needs to wake up and realize this is their money that was taken from them, and now controlled by an entity that considers it theirs. When physicians no longer take Medicare, tell patients they no longer dispense DME, and the modalities take weeks to approve, maybe it will make a difference in the voting public.

I remember Al Brown very well-in fact; he was the original podiatrist who encouraged me into our profession. He was a generous man and so friendly. Although I never agreed with his philosophy of treatment as I entered the profession, he was always a friendly warm face to chat with on any given issue. I recall seeing him at a Florida conference about 6 years ago--even at his late age of more than 90 years old, he sat through every lecture of the day. RIP Al.

RE: The Passing of Albert R. Brown, DPM (Steven Ames Brown)
Five years ago, when I was President of the Academy of Minimally Invasive Foot and Ankle Surgery, we were celebrating our 50th anniversary, and we invited Dr. Brown, the only surviving founder to be honored. I expected to see some elderly bent over gentleman with a walker. Boy was I wrong. When he came in, straight up and unassisted, and shook my hand, I almost went to my knees. His forearms would make Popeye jealous, and I thought maybe I should stop lifting weights and using power equipment and start using rasps to build myself up.
He then introduced me to his nurse whom I assumed took care of him at home. She promptly corrected me saying she was his office nurse who picked him up at 7 AM every morning and dropped him off around 6 PM every evening, because beginning in October, all his old Michigan snowbird patients would start calling and he was booked...
