ND Podiatrist Discusses Bunions
 
"If you do not have pain and your bunion doesn't interfere with your lifestyle, medical treatment isn't necessary," says podiatrist Dr. Eric Hart. "However, bunions are permanent. As they get larger, they often become more painful. If you frequently have pain or inflammation, decreased movement in your foot and difficulty finding shoes that fit, make an appointment with a podiatrist (foot and ankle specialist). Your podiatrist can show you simple treatments to help reduce pain and pressure from the bunion, prescribe medications that can help reduce bunion pain and, when needed, perform outpatient surgery to fix the bunion. Not all bunions are treated the same," says Hart.

"Treatment and surgical options depend on how big the bunion is, your general health and if there is also arthritis in the joint. Many people put off evaluation for the problem until there is severe damage to the joint. This can limit treatment options. If in doubt, don't hesitate to schedule an appointment or to discuss your concerns with your primary care provider," says Hart .

MI Podiatrist Joins Lakeland Health
 
Lakeland Health recently added James Maskill, DPM, to its medical staff. He is seeing patients at Community Bone & Joint within the Health Park Medical Suites, St. Joseph. Maskill completed fellowship training with New England Foot and Ankle Specialists in Portland, Maine, where he specialized in complex hindfoot and forefoot reconstruction, total ankle replacements, and foot and ankle trauma.

He earned his podiatric medical degree from the Rosalind Franklin University in Chicago, where he graduated summa cum laude. He completed his foot and ankle residency with Kaiser Permanente in the San Francisco Bay Area. Maskill joins his brother, orthopedic surgeon Michael Maskill, MD, Jeffrey Postma, DO, and Jonathan NyKamp, NP.

The presentations were on ankle osteochondral defects, lateral ankle instability, and Achilles tendon surgery (which he will be also discussing this week at the American Academy of Podiatric Sports Medicine Annual Meeting this week at West Point, NY).

SD Podiatrist Sued for Amputating Toes on Schizophrenic Patient
 
A doctor amputated all the toes on a woman's foot despite warnings from her primary physician that the woman is schizophrenic and her toes were healthy, the patient claims in court. Susan Phillips sued Dr. Jessica Shaw and S.F. Foot Specialist, PLLC dba Sioux Falls Foot Specialists, on Monday in Minnehaha County Court.
  
According to the lawsuit, Shaw wanted to amputate all 10 of Phillips's toes. Shaw is a doctor of podiatry. Phillips's primary physician, Dr. Tara Geis, learned of Shaw's plans on Sept. 8, 2014, according to the complaint. Geis is not a party to the lawsuit. On Sept. 8, according to the complaint, ...

Out-of-Network Billing Fuels Provider-Payer Friction
 
One of the most contentious clashes between providers and payers is over billing for out-of-network medical services. This year, legislatures in at least seven states have considered creating laws to set firm ground rules for how much money physicians and hospitals can charge when they are not included in a payer's provider network. Charges above the in-network rate are called balance billing.
 
The sparks are flying furiously in New Jersey, where Hartford-based Aetna has sued a half dozen out-of-network physicians in recent years, alleging gross over-charging for services. "New Jersey is our primary play," says Elena Butkus, head of state government affairs at Aetna. "They have the highest charges in the nation." From Aetna's perspective, the commercial insurance carrier has an obligation to...

Pro: As we go to new care models, where you might have a health navigator helping with care gaps, maybe that person would want to view [app data] on a regular basis. Reviewing app data might also work in an environment where someone is homebound and a home health nurse stops by, can scroll through the app and say to the physician, "We have some outlier numbers here I think you need to review."   
 
Con: In a primary-care model, where a patient has a direct feed and sends information to the physician for review at a set time, that could again become overly burdensome for the patient. There is a real potential for health apps, but it's a little premature in the current primary-care environment. We still have a lot to learn about how well [apps] can work as trusted sources of data.

I have a patient who has 2 screws in the medial malleolus of the left ankle that were put in several years ago. Now they are protruding and painful. They need to be removed. What ICD-10 would I use?

Response: Series T84 requires a 7th character. The appropriate 7th character is to be added to each code from category T84 options. They are: 
A initial encounter    D subsequent encounter      S sequela 
 
This is not acute, so no "A." This is not complicated, so no "S." That makes "D" just right. 
 
T84.127D (displacement of internal fixation device of bone of left lower leg). Another option may be T84.39- (other mechanical complication of other bone devices, implants and grafts; Inclusion Term: Protrusion of other bone devices, implants and grafts)... 
T84.398D (other mechanical complication of other bone devices, implants and grafts). 
 
Either of these should work, so pick the one that is most relevant to your case.
