Recently, Dr. Jarrett D. Cain, Assistant Professor Department of Orthopaedics, Penn State Hershey Medical Center, Penn State College of Medicine spoke at the 15th Annual European Federation of National Associations of Orthopedics and Traumatology/British Orthopaedic Association Congress in London on diabetic bone healing in the foot and ankle. His topic was "The analysis of osteoblasts cells in the diabetic foot."

Los Gatos foot surgeon Dr. Joan Oloff is bringing comfort to high heel-wearing women. She has designed a high-end shoe line that bears her name and includes 4-inch stilettos, as well as lower heels, wedges, and even flats. "My designs are for women who want pretty shoes that are comfortable," Oloff said. The key to comfort, she added, lies in the design of her shoes' form. "Basically, each shoe company designs their own form," Oloff said. "I've changed the shape of mine by dropping the heel down 3 millimeters. That shifts the weight off the bottom of your foot."

Oloff's forms also contain a hidden "canal" that allows the foot to sink into a soft material. "A shock absorbent material lines the entire insole," she said. Oloff has a Dallas-based sales rep and hopes to bring her footwear to Los Gatos soon. In the meantime, the nearest place to buy her shoes is the Bellagio in Las Vegas. Prices range from $300 to $500. "These shoes are quite expensive to make," Oloff said. "They are all handmade in Italy, using the finest leathers. My insoles cost five times the cost of the typical insole. As I tell people, an orthotic with all the components of my insoles would cost $500 or more, without the shoe.

Cynosure has acquired the assets of Ellman International, Inc. for approximately $13.2 million in cash. In addition, Cynosure assumed certain contractual and current liabilities associated with normal working capital. Based in Hicksville, NY, Ellman develops, manufactures, and markets advanced radiofrequency (RF) technology for precision surgical and aesthetic procedures and offers a line of aesthetic lasers. 
 
Ellman's product line encompasses multiple RF generators and single-use electrodes for aesthetic and multi-specialty surgical indications such as facial plastic and general surgery, gynecology, ear, nose and throat procedures, ophthalmology, oral and maxillofacial surgery, podiatry, and proctology. Ellman's proprietary high frequency, low-temperature RF technology is optimized for achieving surgical precision and controlled hemostasis.

What are the Benefits of a Super Group?
 
The benefits are many. Practices are able to retain their autonomy while achieving efficiencies of scale that help to decrease costs. For example, several of our clients' medical malpractice insurance rates went down because there are more physicians to dilute risk, employee payroll costs and healthcare benefits become cheaper, and practices are able to secure better prices from suppliers.
 
Insurance companies are also looking at new models for doing business. Many would prefer to work with clinically integrated groups, where all of the members are on the same EHR platform and adhering to the same set of standards than they would doing business with IPAs that may not be able to steer their membership toward producing higher-quality outcomes.

I would definitely call the shape of the distal phalanx a distal subungual exostosis. Combined with the incurvated nail and distal soft tissue prominence, I suspect that the osseous prominence is a major portion of the problem. Use a curette under the nail to press on the prominence to check for pain. If it is there, a surgical option to resect the distal portion of the bone with a distal approach and very delicate dissection under the distal nail bed may help.

I respectfully disagree with the no dorsal spur comment. There clearly IS a dorsal upturning of the distal aspect of the distal phalanx likely causing the dystrophic pincer toenail appearance. I qualify my ensuing "treatment" paragraph by reiterating a classic quote from my 3rd year of podiatry school..."if it ain't broke, don't fix it," because you can't take something that isn't painful and make it feel any better!" (Dr. Emanuel Sugarman- NYCPM 1978) - words I have lived by and which have withstood the test of time.
 
That said....In theory, subungual exostectomy provides the vehicle for toenail flattening. From experience, this doesn't always happen, particularly if the dystrophy has been present for a lengthy time period. Oftentimes, the nail matrix's ability to generate normal nail tissue is permanently compromised. Your patient's expectations for eventual normal nail growth will likely not be met. Keep this in mind when making the medical decision whether to attempt to repair this toenail or not. After exostectomy, exhibit patience observing the new nail growth. It is likely that bilateral border partial matricectomy will also be required. That combination creates the best chance for the most normal toenail appearance possible, but again, exercise significant caution when trying to meet an unrealistic cosmetic expectation, ESPECIALLY when asymptomatic.

I'm sure that some of my colleagues will find my suggestion too radical, but since this is a 65 y/o patient who, by definition from her systemic disease, is not extremely active, why not consider 1-5 metatarsal head resections? There will be no need to hope that the retrograde forces will be reduced and no bone on bone healing required. Since PIPJ arthroplasties are already in the mix, removing the metatarsal heads with retrograde K-wire fixation virtually guarantees elimination of all her plantar lesions. She could conceivably use a short leg AirCast boot until the K-wires are removed.

There are two codes for injecting sclerosing alcohol perineurally as treatment for neuromata. CPT 64455 is used when the concentration of alcohol is less than 30%, and CPT 64632 is used when the concentration of alcohol is greater than 30%. Steinberg’s/Dockery’s “mild sclerosing solution” is used multiple times and uses reverse angiogenesis for lesions such as neuromata, soft corns, etc. This is coded CPT 64455.
 
When dealing with intractable, end-stage neurogenic pain and the need for neurolysis, chemical ablation, or permanent destruction of a nerve in lieu of a neurectomy in cases of nerve injury or chronic pain syndromes, toxic levels of alcohol or phenol are injected and properly coded as CPT 64632. In general, no more than one or two treatments are necessary, and this is rarely needed when treating neuromata. The use of CPT 64632 as a code for a 0-29% alcohol injection or when discussing anything but ablation or denervation with patients deserves investigation as insurance fraud.
