NC Podiatrist Follows in Her Father’s Footsteps
For the little girl who engaged in squirt gun battles in the hallway, played in the filing room, and set up pranks with her father on the staff at Triad Foot & Ankle Center, it’s no surprise where Rebecca Sikora, DPM, works today. She is the latest physician to join the practice where her father treated patients for more than 25 years.

“It was always a goal of mine to practice with my dad at Triad Foot & Ankle Center,” said Dr. Sikora. Sadly, he suddenly passed away from esophageal cancer in 2016 before that dream could become a reality. Sikora’s parents had another career path in mind for their child. “My dad wanted me to do something besides podiatry, like radiology or ophthalmology,” Sikora noted. “But as I was researching what field of medicine I wanted to pursue, I realized podiatry was in my heart.”

Court Says Local Health Centers Can Challenge AHCCCS on Reimbursements
A federal appeals court Friday ordered a new hearing for Arizona community health care centers that claim the state’s Medicaid system is wrongly denying reimbursement for chiropractic, dental, optometric, and podiatric care. A three-judge panel of the 9th U.S. Circuit Court of Appeals reversed a lower court decision that dismissed the Arizona Alliance for Community Health Centers’ suit against the Arizona Health Care Cost Containment System (AHCCCS), the state’s Medicaid provider.
In his opinion, Circuit Judge Richard R. Clifton praised the district court judge and the attorneys on both sides “for their skillful handling of the uncommonly complex issues presented by this case.” But he said the decision to dismiss the case was wrong and ordered the case back to the district court for a new hearing. While the Supreme Court “has long held that ‘nothing in the (Medicaid) statute suggests that participating states are required to fund every medical procedure'” included in the mandated categories, CMS has to justify the reason for the decision to exclude coverage. Clifton said it had not.

Response: There is not a specific ICD-10-CM code for a plantar plate tear. With the assumption being that the plantar plate tear is not the result of an acute traumatic incident, the most appropriate ICD-10-CM codes to use would be the following:
M24.374 Pathological dislocation of right foot, not elsewhere classified
M24.375 Pathological dislocation of left foot, not elsewhere classified
Since this is not due to an acute traumatic incident, a 7th character is not needed such as:
A = Initial encounter
D = Subsequent encounter
S = Sequela

RE: The Passing of Sheldon Marne, DPM
It’s the end of an era! Sheldon Marne, DPM of ABC Podiatry in Hendersonville, NC is now resting, in peace, after decades of doing what he loved! On August 27, 2022, Dr. Marne saw his last patient. He was a graduate of the New York College of Podiatric Medicine in 1962. Later, he was director of residency and chief of medical podiatry at Mount Sinai Medical Center in Miami, FL.

He called Hendersonville, NC home for the past 30 years. Let’s help lay to rest our outspoken and opinionated podiatrist who wasn’t afraid to stand up for what he believed in and loved anything to do with podiatry. Thanks for sharing many pieces of your mind with the younger generations and paving the way for others within our field. I am truly blessed to have been a part of your legacy here in North Carolina and podiatry until the end!
The North Carolina Foot and Ankle seminar will be much different this coming year. We will miss the 30 minutes of words of wisdom from the “old codger” in the back row who we often heard snoring over the speakers as he was wiser in years of experience in practice. And when we close our eyes we will see his blue jeans, plaid shirt and suspenders, the uniform of a hard-working, turned mountain man, who awkwardly was admired by many who were able to see past this and were blessed to be able to see what this man had to offer from the inside. Rest in Peace Dr. Marne.

RE: Gene Test for Bunions

I am often asked by patients if they will develop bunions as their parents and grandparents did. There is a genetic test readily available and provided by 23 and Me. I took the test by spitting into a test tube and paid the $99 fee. The test results confirmed my Western European history as well as no susceptibility to the uncommon medical disorders listed on their website.
Incidentally, I was relieved to learn that I do not have the dreaded genetic code for bunions, but sadly the test confirmed my lactose intolerance.

I find it incredibly difficult to believe that Dr. Stewart’s patient or anyone in a sound mental state of mind would pay $1,500-$1,800 for an OTC orthotic. This video has infomercial written all over it!

I received email response to my original article. It pointed out that a number of states have a limited number of podiatric CMEs allowed for licensure through virtual conferencing. For example, the reader noted that Florida recently reduced its allowance for virtual CMEs to eight for podiatrists.
The point is that these regulations fluctuate and are consistently changing. Many states have increased hours of virtual conferencing allowed for DPM continuing medical education credits. When in doubt, check with your state board or state podiatry association to get the current ruling. The CME allowance for podiatric state licensure regulation that was in place yesterday may change quickly and not be in place tomorrow.
