PODIATRISTS IN THE COMMUNITY |
NY Podiatrist Celebrates Labor Day at St. Rocco’s Feast
When the Italian stone mason who settled in Orleans County to work the quarries erected a church, they named it for a saint venerated in their hometowns. The spirit of those whose hands built it, and their proud heritage, is refreshed each year with a re-awakening in a nearby field filled with food, bocce, and music. St. Rocco’s feast day is marked in mid-August, but the festival of his namesake church is always held on the Sunday before Labor Day, drawing in an annual crowd that overwhelms the park.

Dr. Ross Fanara |
Dr. Ross Fanara, a retired podiatrist from Batavia, said he has been celebrating the festival of Rocco, a patron saint for the poor, for 25 years. “The hospitality, the food, the friendships ... it’s wonderful,” said Fanara, whose culinary course started with a bowl of pasta fagioli before moving to a small plate of spaghetti and meatballs.

GA Podiatrist Celebrates 50 years of Star Trek
The first episode of Star Trek was broadcast 50 years ago this week, on Sept. 8, 1966. The TV series was only on the air as long as a Red Shirt’s career in Starfleet, yet it has somehow endured, evolving into six (soon to be seven) TV series and 13 motion pictures. Resistance to its cultural impact has been futile.

Dr. Gary Fields (Photo: Melanie Boyd) |
Dr. Gary Fields, a Carrollton podiatrist, remembers when the show was first broadcast. As a kid growing up in New York City, he had already spent a lot of time watching scary movies about space aliens. When NBC began promoting the show in the fall of 1966, he was looking forward to the first broadcast. From the start, he said, the show was unlike any other science fiction he had seen. “It involved human beings for the most part intermixing and developing relationships,” he said. “Not just with other cultures, but with other beings from other planets, with different thoughts, different ideas, different concepts.”

SUCCESS TIPS FROM THE MASTERS |
Bret Ribotsky: Currently, we evaluate residents and have set benchmarks with the number of cases performed. Is this the best way to evaluate competence?
|
Dr. Sam Mendicino |
Sam Mendicino: For outstanding wound care residencies, residents are forced to get the minimum activity value (MAV) for surgical cases, and great surgical programs are forced to get enough biomechanical patients to work up. I’m not smart enough to treat wounds; I send those patients to others with more experience. I have always favored the dental model (in which dentists can chose a specialization, e.g., orthodontics, periodontics, etc.).
Meet the Masters is broadcast each Tuesday night at 9 PM (EST). This week's guest is to be announced. You can register for future events by clicking here

PRACTICE MANAGEMENT TIP OF THE DAY |
Five Ways to Decrease Patient Wait Times - Part 6
Improving access is a win-win
Wait times are only going to become more of a concern as care demand increases, along with patients being forced to pay out-of-pocket. Patients who must wait months for an appointment will likely take their care needs and their dollars elsewhere. By decreasing wait times and improving physician capacity, practices can gain a competitive advantage in their markets, increase patient satisfaction, and ensure prompt high-quality care.

CODINGLINE CORNER |
RE: Updated Taxonomy Codes for DPMs
An updated set of taxonomy codes has been developed and is set to go into effect January 1, 2017 (for some Medicare contractors, October 1, 2016). It is of utmost importance that you check your taxonomy coding which you submitted to Medicare. The following are accepted codes for podiatrists:
Podiatrist - 213E00000X [definition] Foot & Ankle Surgery - 213ES0103X[definition] Foot Surgery - 213ES0131X [definition] Primary Podiatric Medicine - 213EP1101X[definition] Public Medicine - 213EP0504X [definition] Radiology - 213ER0200X [definition] Sports Medicine - 213ES0000X [definition].
It is especially important to note that for podiatrists, General Practice - 213EG0000X [definition], is being eliminated. Therefore, if you are registered with Medicare under this taxonomy code, you may need to contact your MAC and have the taxonomy code changed. Whether this will be something you can do without an 855 revalidation, I leave it for the enrollment specialists to comment on.

RESPONSES/COMMENTS (-CLINICAL) - PART 1A |
From: Howard Bonenberger, DPM
Consider nerve entrapment at the fibular tunnel in light of the relief you describe. Examine for this and consider referral to a peripheral nerve surgeon in your area. Often these are DPMs who have completed extra training at Johns Hopkins with Lee Dellon, MD. A Google search may help identify someone nearby.
Howard Bonenberger, DPM, Nashua, NH

RESPONSES/COMMENTS (CLINICAL) - PART 1B |
From: Michael M. Rosenblatt, DPM
The doctor who has a patient with "unrelenting metatarsalgia" has relief with a wheal raised just over the posterior tibial nerve. This relief lasts about 5 days.
Whenever I had a patient who experienced unrelenting nerve pain, I recommended looking for a systemic condition that causes the pain. There can be many of them, and usually they require a work-up combined with a referral to a neurologist to help look for that explanation. It is sometimes too easy to get caught up in local podiatry without keeping a necessary wider perspective.
All specialists have a tendency to do this, not just "limited licensed" doctors. Systemic conditions like lupus, MS,...
Editor's note: Dr. Rosenblatt's extended-length letter can be read here.

|
RESPONSES/COMMENTS (CLINICAL) - PART 1C |
From: Matthew Kaiman, DPM
Please refer the patient to a neurologist as the work-up for a radiculopathy appears logical in the absence of any additional information. It seems as if you have established a piece of the algorithm which is beyond the scope of your practice.
Matthew Kaiman, DPM, Englewood, NJ
