ME Podiatrist to Provide Services at Franklin Health Orthopaedics
 
Franklin Memorial Hospital (FMH) and St. Mary’s Center for Orthopaedics have announced that Daniel Buck, DPM, FACFAS, a surgical podiatrist, will be providing podiatry specialty services at Franklin Health Orthopaedics on the FMH campus. Dr. Daniel Buck brings a valuable specialty in podiatry to greater Franklin County, treating patients with a wide range of foot and ankle challenges.

Dr. Buck received his medical degree from the California College of Podiatric Medicine in San Francisco, CA, and completed his residency at the VA Medical Center, also in San Francisco. He is a Fellow of the American College of Foot and Ankle Surgeons, and is board certified in foot and ankle surgery by the American Board of Podiatric Surgery. Along with his medical degree, Dr. Buck has also earned a law degree from the University of Maine School of Law.

NY Podiatrist Debuts Line of Comfortable Fashion Shoes
 
We’ve all heard the expression “No pain, no gain.” It may hold true in some instances, but beautiful shoes do not have to be painful. Podiatrists maintain that your feet should never hurt, yet 53 percent of women experience foot pain, according to the American Podiatric Medical Association. For years, women have been willing to grin and bear it in the name of Christian Louboutin and Jimmy Choo, among others. While many patients longed for lovely shoes, they felt they had to sacrifice today’s popular looks for more matronly comfort. No cage booties or peep-toe pumps for them.

Considering all of this, Dr. Bruce Pinker developed a line of finely crafted, bespoke footwear, called Dr. D-LuCS, which debuts this month. The line of finely crafted footwear – available in pumps, sandals, and boots – provides style coupled with comfort and unique design. In order to achieve the perfect fit, patients begin the process with a personal, computerized gait analysis from which a custom-made orthotic is constructed.

PODIATRISTS AND THE LAW
IA Podiatrist Closes Doors After Losing License
 
A Newton podiatrist had his license suspended amid allegations that he was receiving pain medication from his patients. Dr. Timothy Smith of the Smith Foot Clinic temporarily closed his doors after the Iowa Board of Podiatry suspended his license on Aug. 21. The board issued a report that found Smith was allegedly diverting narcotics from patients by not charging co-pays in exchange for the patient’s pain medication. According to the report, a board investigator interviewed Smith on Aug. 12, when he initially denied any wrongdoing, but as the interview continued, he admitted to the illegal activity. There is a hearing scheduled on the emergency adjudicative order for Sept. 15 in Des Moines.

QUERIES (CLINICAL)
Query: Pain After Foot Surgery Due to Tight Dressing
 
In the last few months, I have seen an increasing number of post-surgical patients who have experienced post-op pain which is not relieved by medication. A majority of these patient have had tight bandage issues. Once the outer elastic outer dressing is removed, the patient gets immediate relief. My dressing consists of 4x4" gauze pads, 4" Webril (cast padding, absorbent), 4" Kerlix roll, and 4" Coban self-adherent wrap. We provide detailed instructions regarding post-op pain expections, and its management, but to no avail. We still have patients having issues. Any suggestions regarding post-op pain management and dressing materials that should be used instead of what is my standard protocol would be helpful.

Query: Billing for Injection of Morton's Neuroma
 
My doctor uses CPT 64632 (destruction by neurolytic agent; plantar common digital nerve) for administering an injection of dehydrated alcohol into a Morton's neuroma. After getting paid barely enough to cover the cost of the ampules, I did some research and found J3490 (unclassified drug) can be used when there is no CPT for the injectable. Has anyone used J3490 and been successful in getting paid? If so, was there any modifier that had to be used?

Response: Check your contractor's LCD, but J3490 (unclassified drug) is a descriptive code here and not payable. I believe you are using the correct code for the injection of dehydrated alcohol. You didn't indicate if you are using the second code for the injection. If you are not using it, please do. There are two separate injection codes for Morton's neuroma: one for injection (CPT 64455) and one for destruction (CPT 64632).

When contemplating surgery on this 65-year old, some peri-operative considerations can be found in this article. Other suggestions found include making sure even transferring the patient from stretcher to OR table is done with extreme care. 
 
The most important pearl: While there are different types of osteogenesis imperfecta, are you really sure you are dong this patient a favor by operating on her severe deformity? Is the painful HAV due to joint pain or shoe issues? Are the IPKs across the board, and is it impossible to maintain comfort with non-surgical care?

I am currently of non-practicing status, but I have experience with this type of patient from my practice. First, and most importantly, get a consultation with the physician who is treating her osseus condition. If she receives medical clearance to your operative plan, K-wires are fine, but you need to avoid a NWB scenario, as these patients must avoid situations that promote disuse atrophy.
 
A Keller with digital arthroplasties and soft tissue releases is reasonable, as osteotomies are very slow to heal and non-union is a potential complication. A post-operative accommodative orthotic is advisable to minimize retrograde sub-met head pressure on the 2nd metatarsal associated with Kellers.

Ask the patient if she gets more pain at night. If so ask her how many/how heavy the blankets are. Most often distal tip hallux pain is caused by heavy/too many blankets or sheets that are tucked under the mattress too tightly. The hallux frequently is the longest digit. One of my patients sleeps with her clean laundry on the bed next to her, tightening the blankets over her feet. The remedy, other than removing the linen, is to put boxes or other items next to the feet under the blankets, or to purchase a blanket support to keep the pressure off of the toes.
