FL Podiatrist Discusses Gout
Gout is a form of arthritis that occurs when there's too much uric acid in the blood. The acid crystallizes and forms deposits in the joints, causing painful inflammation, most commonly in the fingers, wrists, knees, elbows and toes. "The crystals are like having small spikes in the joints," said Dr. Federico Auger, a podiatrist at the Ankle and Foot Center in Carrollwood, FL. He typically sees gout patients during an initial bout, when they don't yet know they have the disease.

The affected joint, often the big toe, suddenly becomes hot to the touch, red, swollen and intensely painful. Why is the big toe affected? Extremities are the coolest parts of the body, and lower temperature is thought to be associated with flare-ups. "An attack can be so painful that they can't even put a sheet on their foot," Auger said. "It can be red all the way to the ankle."

NY Podiatrists Featured in 9/11 Video
Drs. Arthur Gudeon, Wilma Watts, and Kathy Reilly Fallon (Volunteer Foot Doctors at Ground Zero after 9-11) will debut in a film which will be shown at the Irondale Center in Brooklyn, NY from September 6, 2011 until September 11, 2011. Their voices will also be heard in the installation of the "Pieces of Paper" project as well.

MI Podiatrist Accused of Medicare Fraud
Federal prosecutors in Detroit have filed a fresh batch of fraud charges against healthcare providers, including a podiatrist accused of billing Medicare for $700,000 in toenail treatments that weren't provided.
Errol Sherman is a podiatrist in Oak Park. The government said Thursday that he committed fraud for at least four years. Sherman and his attorney declined to comment after they appeared in court.

MGMA, AAFP Urge Changes on Doc Fee Schedule
The CMS' proposed Medicare Part B Physician Fee Schedule for 2012 needs to be revised to relieve information-technology-related administrative burdens on practices and adjust how primary-care services are valued, according to two organizations of healthcare professionals.
In a 17-page letter to CMS Administrator Dr. Donald Berwick, the Medical Group Management Association argues that if, for example, physicians meet electronic health-record system meaningful-use requirements and qualify for EHR subsidies through the CMS' IT incentive programs, they should automatically qualify for bonuses under the Physician Quality Reporting System program and be exempt from penalties related to electronic prescribing.

Query: Hypersensitivity to Narcotics
I have a patient who suffers from both anaphylactic and delayed reactions from just about all narcotics, including: Morphine, Dilaudid, Talwin, Ultram, Methadone, Suboxone, Nucynta, Fentanyl, Codeine, and Nalbuphine. I have her on Butrans now. This worked initially, but after a few days, she is starting to develop a delayed reaction.
I have consulted with pain specialists who really didn't have any recommendations about future pain relief. Does anyone know of a method of patch or other testing to determine what drugs may work without a reaction?

RE: Cycling-specific Orthotics (Jeffrey A Petrinitz, DPM)

I think the cyclist going numb in the toes with prolonged riding would be better off with an orthotic for his BACKSIDE, not his feet. In other words, recommend that he change either the type or pitch of his seat. It is common for bicycle seats to contribute to nerve compression from symptoms in the feet, thighs, or even genitalia. Considering the b/l nature of his symptoms and lack of foot pathology, I would bet on it.

I have found that a thin shell device such as graphite with a low bulk rearfoot post has worked very well. A full-length extension such as EVA with a vinyl cover has worked well and has been very durable in my own shoes. I believe the device spreads the pressure more evenly over the surface of the foot. This, combined with not making the shoe too tight, has eliminated numbness problems for me.

With all due respect, I disagree with one aspect of Dr. Jacobs' chastisement of his colleagues that he claims moan and groan and don't "do something" by serving on various committees.
I do not think that you can blame the membership for not "falling on the sword" for the profession to MAKE UP for the failure of the APMA and the schools. They continue to ask for money, refuse the majorities' will for a degree change, yet want us to join committees that support minutia efforts while ignoring the only remedy that can cure podiatry's perception. Sure, be proud of your DPM for what it is, but please recognize it's not all it should be.
