NE Podiatrist Bikes Across America for Charity
An Omaha state senator recently achieved a “bucket list” milestone, completing a bicycle trip across America, despite breaking his hip midway through the ride. State Sen. Robert Hilkemann pedaled into Washington, DC, the end of the 3,500-mile ride, on Aug. 10, then hoisted his touring bike high above his head at the National Mall. Dr. Hilkemann joined a cross-country bike ride organized by the Fuller Center for Housing, a non-profit that rehabilitates homes for the poor.

The 71-year-old senator, a retired podiatrist, began his ride in 2016 in Seattle and planned to pedal halfway to York, Nebraska. But only miles from the halfway point, he crashed and broke his hip. A couple of screws in his hip later, he recovered and finished the final 1,650 miles this summer in four weeks.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
Improve Your Patient Communications - Part 1
Here are some strategies to help you refocus and enhance your patient interactions:
1. Develop a welcoming ritual - Your patients will draw their first impression of you within the first few seconds of your appointment, so having a practiced ritual is key. That ritual might include announcing your entrance, smiling, and making eye contact. Proceed by addressing your patient by name, introducing yourself and offering a handshake or other appropriate touch. Acknowledge all visitors, sit down if possible and start the conversation with something non-medical to establish rapport.
Once rapport has been established, elicit your patient’s expectations or goals and ensure there is an opportunity for all of his/her concerns to be shared. You can ask questions such as “What were you hoping we’d accomplish today?” and “Is there something else you’d like to talk about?” Once you have the patient’s agenda, summarize it by listing the issues.

Query: Exclusion For Promoting Interoperability (PI) Category
I received information regarding the availability of the MIPS 2019 PI exclusion application. Our practice EHR provided a Webinar for 2019. There was information about claiming exclusions for certain Promoting Interoperability measures. An exclusion measure category points would then be re-allocated to a different category in Promoting Interoperability. For instance, if a provider claims the exclusion for the entire Public Health and Clinical Data Exchange category, the points would be re-allocated to the Provider to Patient Exchange category.
Does anyone have information regarding this? Would it be better to claim an exclusion for the entire Promoting Interoperability category if you know you cannot meet all the measures?
Mary Gail Kwiecinski, DPM, Libertyville, IL
Response: It is your choice. You can claim the exclusion from that one PI objective or from the whole PI category.
As far as the one objective, be sure you meet the requirements of the exception. You mentioned claiming an exclusion for the Public Health and Clinical Data Exchange objective. For that objective, you are allowed to use one registry and claim an exclusion from the rest, but claiming an exclusion from the whole Public Health and Clinical Data Exchange objective is a different story. Be sure you qualify for that if you are considering doing it. I do not think most (or any?) podiatrists qualify for an exclusion from the whole objective.
I think deciding on claiming exclusion from individual PI objective(s) (assuming you qualify) versus the whole category comes down to ease and your scoring. If you do qualify for exclusion from one PI objective and your scoring on the other 4 PI objectives is excellent, maybe you only want to claim the exclusion from the one objective rather than the whole category. On the other hand, if you are a small practice, the exclusion from the whole category is super fast and easy.

On what basis (studies published in peer-reviewed journals) does Dr. Goolsby support his claim that MIS bunion performed with "three small incisions" is associated with a lower infection rate, swelling, and less pain?

From: John Chisholm DPM
Thank you, PM News, for the kind notice of the passing of my friend Skip. You accurately portrayed the accomplishments of a pioneer in podiatric medicine here in San Diego. I practiced with him for 25 years, and the love he had for his patients was exceeded only by their love for him. He worked part time right up to the time of his passing.
Skip was a remarkable man in so many ways. He was a licensed pilot, a professional musician (Outstanding Guitarist at the Monterey Jazz Festival), an avid hunter and fisherman, and spent his summers working as hunting guide in Colorado. He built a home in the mountains of San Diego with his own hands.
Skip always said, "I just want to keep hunting, fishing, and working until I drop dead." He died while fishing on his beloved Lake Jennings. He went out just like he lived - a winner, on his own terms. I will miss my friend and mentor. So will his patients, family, and colleagues here in San Diego.
John Chisholm, DPM, Chula Vista, CA
