ID Podiatrist Pushes for State Medical School
A local doctor and Republican candidate for Legislative District 29’s state senate is making a push for a state medical school based at Idaho State University. Dr. Kert Howard, a Pocatello native and longtime podiatrist, said he has already spoken with ISU and Portneuf Medical Center leaders and other healthcare professionals about the idea, and they are very supportive.
|
|
Dr. Kert Howard |
“I strongly believe Idaho needs an in-state medical school and that school should be part of Idaho State University,” he said, adding that Idaho ranks low among states with educational opportunities for doctors even though the state’s students have high GPAs and do well on the Medical College Admission Test.

Healthcare Spending Growth Expected to Pick Up This Year to 5.6%
The economy's lackluster recovery eroded wages and left millions chronically unemployed. It also offered significant relief from the fiscal distress of U.S. health spending. Now, spending fueled by the improving economy and the healthcare reform law's insurance and Medicaid expansions are likely to turn that around.
Health expenditures grew 3.6% last year, with the delayed and soft recovery a primary cause, according to projections by CMS actuaries published online by the journal Health Affairs. If the projection is confirmed later this year, it would mark the fifth straight year of growth below 4% and the second time in three years that national health spending growth dipped to a historic low.

Query: Osteogenesis Imperfecta Patient
I'm planning on doing surgery on a 65 y/o female patient with osteogenesis Imperfecta w/ fibrous dysplasia. I would like advice from colleagues experienced with these patients. Her chief complaint is B/L foot pain - painful HAV, painful IPK sub-met heads, and painful hammertoes. She has a history of multiple bone fractures in her body, but no prior foot fractures.
|
|
Osteogenesis Imperfecta |
My thoughts are to avoid osteotomy/fusion procedures (i.e., hardware) due to risk of fracture and non-union. Thus, I'm thinking of performing a Keller, PIPJ arthroplasty 2-5, and dorsal MPJ soft tissue release. Hopefully, this will remove retrograde forces and alleviate metatarsalgia/IPK symptoms. Does this approach sound reasonable? Any peri-operative tips or pearls? Is use of bisphosphonates helpful? Is pre-op C-spine x-ray (analogous to RA patients) recommended? Is use of K-wires okay? Is it okay for the patient to be WB with a walker boot post-op or should she be completely NWB, and for how long?

| RESPONSES/COMMENTS (ACADEMIC) | |
From: Thomas Graziano, DPM, MD
As Dr. Secord astutely directs, we should all be aware of the role of gout or more specifically hyperuricemia and its association with the co-morbidities of metabolic syndrome. In my opinion, hyperuricemia is grossly undermanaged by a fair number of primary care physicians. We, as podiatric physicians, are often the first to see the pedal manifestations of hyperuricemia. Furthermore, it is incumbent on us to make sure that our diabetic patients with hyperuricemia are being managed by renal-sparing agents. The literature has clearly provided evidence that hyperuricemia is not without serious consequences. We can and should play an active role in educating our patients in this regard.
