KY Podiatrist Re-appointed to State Board
Gov. Steve Beshear has made the following appointment to Kentucky boards and commissions: Beshear re-appointed Ann K. Farrer, DPM as a member of the State Board of Podiatry, to serve for a term expiring July 25, 2019.

Dr. Farrer, of Lexington, is a podiatrist at First Feet Podiatry. She graduated from the Ohio College of Podiatric Medicine. She is currently Secretary of the Kentucky State Board of Podiatry and is board certified by the American Board of Foot and Ankle Surgery.

QUERIES (MEDICAL-LEGAL) |
Query: Buying a Practice and Transferring Insurance Payments/Contracts
My practice is negotiating to acquire the practice of a retiring DPM. He will continue to work for us part time for the first few months to transition the patients. We are looking for advice on making sure all the payments and insurance transactions come to our main/business office. Is it just a matter of calling the insurance companies and adding his location under our tax ID and his provider number, or is it more involved? What kind of time frame should we expect? Any other pearls from colleagues concerning adding someone's practice is greatly appreciated.

A few reasonable options:
1. Physical therapy referral for TENS treatment
2. Consider nerve release/neurectomy
3. Excise the painful cicatrix and perform TMA for wound closure and better function (it looks like only 2 rays remain)
4. Excise painful cicatrix and perform muscle flap with split thickness skin grafts (STSG)

Maybe it's just me, but I have heard enough about the "doom and gloom" of podiatry, or the medical profession in general. Yes, there are/have been changes. Yes, private practice is challenging. I've been hearing this since I entered OCPM back in the '80s! Blah-blah-blah. There is a book published titled Who Moved My Cheese by Spencer Johnson. It's an easy read that maybe will change some attitudes to being a little less negative.
Ironically, the post just before Dr. Klein's talks about a recent survey showing: despite challenges, physicians remain happy.

I must, very reluctantly, agree with Dr. Klein's prediction. Upon graduation from NYCPM in 1979, I made the fatal mistake of agreeing to see Medicare and Medicaid patients and accepting assignment of benefits from them and every insurance company I could find. Of course, my first mistake was moving to Massachusetts where participation with insurance plans is virtually mandatory, and has been for decades. Because of the rules written here, if a patient sees a doctor who is not in their plan, even before the plague of HMOs, they were and are not eligible for reimbursement of any kind.
It is not realistic to think that patients will pay for health insurance and then additionally pay you...

While I agree with the title "unfortunate foot of the day", I hardly doubt that foot looks the way it does because of the "homeless guy not wearing his diabetic shoes and ending up in the ER." I think the elevated sugars, the PAD, and neuropathy had a little more to do with it than the lack of diabetic shoes. Just a hunch.
