PA Podiatrist Discusses Difference Between Soreness and Injury
 
At one time or another, all runners experience soreness during and after runs. Many of us also unfortunately become injured. What is the difference between the two and should you alter your training regimen? According to podiatrist Brian Crispell, DPM, "Soreness is mild discomfort that lasts 1-3 days. It usually subsides, or lessens after the warm-up period during exercise. Most of the time, soreness is related to a hard running effort or race just prior to onset. It could also be from participating in a new activity, or can be exhibited as chronic joint pain that comes and goes related to training volume. You can usually continue training through soreness by decreasing mileage, pace, and overall impact exercise exposure."

"More intense and concentrated pain in one particular area usually indicates an injury. The pain can last longer than the 1-3 days associated with soreness, and generally worsens over time without treatment. Injury usually requires time off from running and many times medical treatment," says Dr. Crispell.

Jesus Christ understandably means a lot of things to a lot of people. For centuries, mankind has debated everything from His birth to His hair texture. We simply want to know if He could play basketball. Since Jesus is always depicted wearing sandals, we asked a New York City podiatrist.

Dr. John Brummer: So the first thing is: if there is no ankle support, there is a high chance of spraining your ankle. The other thing is that most sandals don’t have a very good arch support so you can strain the plantar fascia, which is quite common. And there’s no cushioning or padding in sandals.

MN Podiatrist Speaks at Kiwanis Meeting
 
Jamestown Kiwanis Club member and Jamestown Regional Medical Center CEO K.C. DeBoer introduced guest speaker Dr. Kyle Abben during the Aug. 24 meeting at the Quality Inn & Suites. Abben is a new foot and ankle surgeon/podiatrist at JRMC. With the addition of Abben, the JRMC now offers many new specialties, including but not limited to total ankle replacement, arthritis of the foot and ankle, arthroscopic ankle surgery, diabetic foot conditions, and Achilles tendon injuries.

Abben is originally from Iowa and went to podiatric medical school at Des Moines University in Iowa. He completed his residency at Regions Hospital in St. Paul, MN, and a fellowship in Atlanta before recently relocating to Jamestown with his family.

Huge Disparity in Leg Amputations in Low-Income and Rural Diabetic Patients: Study
 
People with low incomes or living in rural areas who have type 2 diabetes are significantly more likely to suffer the amputation of a lower extremity, largely due to limited access to preventive healthcare, according to research by the Southern Arizona Limb Salvage Alliance (SALSA) at the University of Arizona Department of Surgery. The study, “A Diabetic Emergency One Million Feet Long: Disparities and Burdens of Illness among Diabetic Foot Ulcer Cases within Emergency Departments in the United States, 2006-2010,” published this month in the journal PLoS One, also found that a system for preventive care for those type 2 diabetes patients could cut national annual healthcare costs by billions of dollars.

“We know now there are significant disparities in diabetes care, particularly within the lower extremities,” said David G. Armstrong, DPM, MD, PhD, UA professor of surgery and director and co-founder of SALSA. “But the great news about these shocking data is that with a little bit of energy, we can affect change and make a difference for these folks no matter where they live or how big their bank account is. This is true across a lot of problems in medicine and surgery. The difference here, however, is the steps are so easy. If we can just get a foot doctor out in those communities, then the risk can go down.”

10th Anniversary Palm Beach County Conference Breaks Attendance Record Again
 
Over 200 physicians attended the 10th Annual Toes in the Sand conference this weekend at the Boca Raton Resort and Club. Hosted by the Palm Beach County Podiatric Medical Association with CME support from Barry University and Platinum sponsorship by Bako, attendees received training on the up and coming ICD-10 transition as well as lectures in Laws & Rules and Medical Errors.

Academic lectures were provided by nationally recognized speakers including Drs. Bradley Bakotic, Lawrence Lavery, Patrick DeHeer, and James Losito. The weekend was capped off by a fantastic intracoastal casino dinner cruise which included salsa dancers, a cigar lounge with hand rolled cigars, and plenty of raffled prizes.

E-HEALTH NEWS
E-Prescribing of Controlled Substances Legal Nationwide
 
On August 28, Vermont became the 50th state to legalize electronic prescribing of controlled substances, making the practice available nationwide.
 
However, while 78 percent of pharmacies can accept e-scripts for controlled substances, only 2 percent of physicians electronically prescribe for these schedule II-V narcotics, according to SureScripts, which operates a nationwide electronic prescribing network.

Despite New Challenges, Physicians Remain Happy: Survey
 
Many physicians would probably tell you overall they are happy, love their job, and if they had the choice, would do it all over again. While that doesn't mean running a medical practice has become easier, it does mean that physicians are finding plenty of avenues for job satisfaction. And there is still room to carve out a practice style or model that works for each individual. Despite recent bellwether changes taking place in government programs like the elimination of the Medicare Sustainable Growth Rate formula and possible softening of requirements for meeting Meaningful Use, the physicians who responded to our survey indicated that "too much third-party influence" is still a major irritant to practicing medicine.
 
Forty percent of respondents claim that is their biggest frustration with being a physician. Contrast that with 13.8 percent (the second highest-scoring answer) who said their declining ability to practice independently caused the most angst. Likely that frustration is aggravated by feeling impotent when it comes to influencing government programs and mandates, like Meaningful Use of an EHR or adopting ICD-10. In fact, many physicians that we spoke to say they have little to no influence over how they will implement government mandates in their practices, nor do they have the time to become involved with local politics or lobbying groups like state medical societies to try to change that. But they are dealing with that frustration in a variety of ways, and by no means are they throwing in the towel.

RE: The Future of Podiatry

The podiatry profession is, I'm afraid, transitioning into the future of large network membership, which will need to collectively bargain for payments for a service. Podiatrists who decide to try solo practice cannot succeed against the strength of large insurance conglomerates that will narrow the network field to control expenses. The days of independent practice are gone. Podiatrists must band together to show strength against these very large conglomerates. 
 
Unfortunately, podiatrists do not perform a unique service. Pedorthists, PAs, NP, PCPs, and orthopedists are all attempting podiatric services, albeit unsuccessfully. If I were a newly graduated podiatrist with a 3-year residency, I'd be looking to join the armed services and establish rank, so that after 30 years, I could retire with a pension. Then I would choose another career field to venture into.

One of my smaller plans was sending me this type of card that I would enter to my charge card machine. They just offered “real” EFT and ERA and upon signing up, I was informed the charge would be 1.9% of the total they would then take out of my bank account for each payment.
 
So…now, not only do we get low payments, but we have to pay for the privilege? Is this legal?
