VA Podiatrist Appointed to Advisory Board on Surgical Assisting
Gov. Ralph Northam has announced the appointment of Srikanth Mahavadi, DPM of Mechanicsville, podiatrist-foot surgeon, New Kent Foot Clinic to the Advisory Board on Surgical Assisting.

Dr. Mahavadi graduated from Des Moines University College of Podiatric Medicine and Surgery in 1996 and completed his residency in New York Governor and Peninsula Hospitals. He is affiliated with the John Randolph Medical Center and the Sentara Northern Virginia Medical Center.

Lions Club Celebrates 99th Birthday of MA Practicing Podiatrist
The Belmont Lions Club helped celebrate long-time member Richard Vallon DPM’s 99th birthday on Aug. 24, with a very special birthday drive-by. “I was thrilled and surprised,” said Vallon. “It was a wonderful sight to see.” The celebration put together by the Lions Club differed from the typical birthday drive-by. Vallon’s friends, family, and neighbors exited their homes and cars donning masks and lining the street to celebrate with him.

Vallon, or “Doc” as he is referred to by Lions Club members, is the oldest member of the Club. He continues to write and deliver the invocation at the start of each of their meetings. Vallon is currently the oldest practicing podiatrist in the state with patients spanning generations. Vallon has had to cut down on his work due to COVID-19, but his patients tell Vallon that they only trust him to touch their feet.

CME Online and MPMA Meeting Successful Despite Internet Outage
CME Online and the Missouri Podiatric Medical Association (MPMA) were scheduled to have day three of the 2020 Summer Podiatry Conference at the time of one of the largest Internet outages ever. The CME Online team recognized the problem and implemented their predefined backup solutions.

4) Make Sure you only Pay for What you Need - While choosing a transcription service plan, you need to thoroughly scrutinize the pricing models. Find out if it's a monthly subscription or a one-time fee. The most important thing to remember, however, is to inquire about the level of support you can bank on, if something were to go wrong.
If you are a small or midsize practice, just about any transcription service will be well-equipped to control the volume of reports you produce on a regular basis. You can also consider investing in a customized transcription services package that will be specifically tailored to suit the needs of your practice.

Query: Unsuccessful Hardware Removal
I attempted to remove a screw under local anesthesia in the office setting that was placed during a bunion correction about 10 yrs ago. I was unsuccessful and the patient will need to undergo the procedure in the operating room. How would I bill this visit? It was a new patient to the office.

Response: Since this was a discontinued procedure based on unforeseen or extenuating circumstances, I would use the -53 modifier on the hardware removal code chosen to reflect the actual hardware removed e.g. CPT code 20680. From CMS (CGS MAC 2012):
“Submit CPT modifier -53 with surgical codes or medical diagnostic codes when the procedure is discontinued because of extenuating circumstances. This modifier is used to report services or procedure when the services or procedure is discontinued after anesthesia is administered to the patient.”

Not having to travel for seminars and not having to rent space for seminar conference space, paints a rosy picture of all of us paying less for our continuing education seminars. Having just attended and lectured at a virtual seminar, I can vouch that as a participant, I was able to hear the speakers better than had I been to an in person seminar. I was also more comfortable sitting in the chair I am now sitting at than some moldy chair in some far away hotel.
There is, however, another side to this issue. We are all linked in our economy, and hotels - even resort hotels - are dependent upon convention business to survive. If post-COVID-19 conventions find that Zoom or "Go To Meetings" seminars make more sense than live seminars, we can expect many resorts to go under in the years to come.

The Podiatry Institute is offering hybrid conferences, virtual conferences, and virtual short courses. The virtual conferences are on Saturday mornings starting at 10am EST so as to not have West Coast colleagues waking up too early. These offer 5 hours of CECH each and are held monthly, with the next being on September 19th (the first occurred in August). On demand courses are being worked on, but have not been fully set up yet.

While I respect Dr. Barney’s response which I agree with, I believe that the original inquiry contained a Q8 modifier as part of the question. That is the reason I included it in my answer. And yes, there are other ways that CPT 11721 can be billed without the Q8 modifier.

I think part of the problem is that the initial nursing facility codes of CPT 99304-99306 are only to be used by the MD who does the admission of the patient into the nursing facility. Podiatrists need to use the subsequent codes of CPT 99307-99310, regardless of whether the patient is new or established. The three MACs I bill nursing facility visits for will not allow/pay for a DPM to bill the initial codes.
Also, Dr. Bass suggested billing the 11721 with a -59 modifier in addition to the Q8 modifier. There is no need for a -59 modifier in this billing scenario, as there was only one procedure performed, the 11721. However, he is absolutely correct that the E/M code needs a -25 modifier.
