Fear of Pain Should Not be a Deterrent to Foot Surgery: AZ Podiatrist
According to Arizona-based foot and ankle surgeon Jason Kayce, DPM, FACFAS, “With the availability of such a large variety of highly effective pain medications, fear of pain should be the last deterrent keeping patients from having foot or ankle surgery. Patients can take comfort in knowing that as surgeons, we have an equally vested interest in keeping them comfortable so their surgical experience is positive and they have a speedy recovery.”

Dr. Kayce explained that patients can receive a local, long-lasting anesthetic immediately following surgery, which significantly decreases pain. Also, in today’s healthcare climate where efforts to reduce prescription drug addiction are at an all-time high, there are stronger anti-inflammatory medications available, which can eliminate the need for pain relievers containing narcotics.

Majority of Running Injuries are Caused by Overuse: NY Podiatrist
Dr. John Connors, who has run almost 20 marathons of his own, addressed a crowd of about 80 cross country and track athletes in the Southern Regional High School auditorium. Connors gave the students an hour-long PowerPoint presentation titled “Running Injuries – Diagnosis, Treatment & Prevention.” At times he slipped into heavy medical nomenclature, talking about things such as patellofermoral pain and femoral neck stress fractures. But much of his talk addressed running injuries in clear and simple language that both the Southern athletes and casual weekend warriors could understand.

There are two basic families of running injuries, said Connors. The first is acute, a.k.a. sudden, which the doctor said accounts for only about 5 percent of his patients. The other is chronic. “The majority of injuries I see are chronic, caused by overuse or stress applied over a period of time,” he explained.

Five Ways to Decrease Patient Wait Times
Practices should consider the following five tips to help improve patient wait times by operating more efficiently and increasing physician capacity:
3. Study provider schedules
Next, analyze provider schedules to determine if they are truly working to full capacity. Does the amount of time providers spend each week on direct patient care match contract requirements? Is too much "overbooking" time blocked? Over time, it's easy for inefficiencies to creep into schedules.
Look for large chunks of time where providers do not see patients — including unused "overbooking" time — to find places where additional appointments slots could be added. If there is more availability, add hours gradually. Make an effort every quarter, for example, to add one hour of direct patient care to the daily schedule. Don't try to zoom to top capacity all at once; rather, chip away at access issues steadily.
Anticipate pushback on scheduling changes, too. For instance, one common complaint is that the EHR creates documentation inefficiencies. If this is an issue, practices should look for technology solutions that can help physicians complete charting faster.

Query: Digital Radiography Penalties?
I am still unclear on how to bill Medicare for x-rays beginning January 1, 2017. If we upgraded to DR (not CR) radiography, how do we show our Medicare contractor that we are not subject to the reduction? Do we need to bill the technical component and professional component separately? Are there any new or different modifiers? What does Medicare require?

Response: Whereas the final rule has the following statement about mammography: "We established a HCPCS modifier for CT services rendered on machines that do not meet an equipment standard," it does not specifically state how they will identify DR, CR, and analog systems for x-rays. It is probable that we will have new modifiers to identify the type of digital imaging system we are using in our office.

I have been reading the discussion on whether the term “overpronation” is better or worse than a term such as “undervaulting.” The reality is that podiatrists need to use different terms when describing the structure and function of the foot and lower extremity when communicating to the lay public, other health professionals, and other podiatrists due to each group’s level of understanding of foot and lower extremity structure and function.
In that context, the term “over-pronation”, though not strictly definable, is certainly an acceptable term when communicating with the lay public since it is commonly used and basically understood by a relatively large portion of the public. The term “undervaulting”, however, is rarely used and not understood by the lay public. A better term for the lay public, though also inexact, may be “lower arch height” rather than “undervaulted”.
However, when communicating with other medical professionals and podiatrists about the biomechanical function of the foot, podiatrists should use terminology that is already widely accepted by the international biomechanics community, rather than what an individual is trying to promote because it is their “pet term.” If podiatry wants to become a more accepted part of the international biomechanics community, then we need to learn and start to use their terminology when at all possible and rid ourselves of terms that are non-definable, ambiguous, and podiatric-centric.
