Changes in Feet May be Indicators of Wider Health Issues: UK Podiatrist
 
They may spend most of the year hidden away under socks, but our feet are in fact a window to what’s really happening in our bodies. “We neglect our feet and put up with signs and symptoms that, in any other part of the body, would have us running to medical professionals,” says podiatrist Jake Heath from One Wellness Clinic in London. “Yet noticeable changes can be indicators of wider health issues.”

Hairless toes could mean vascular disease. Few of us would want hairy feet. But toes totally devoid of hair can be a problem. If the heart is struggling to pump enough blood to your extremities, it will cause problems with your circulation. That will make the skin look shiny and hair will not grow there. “Discolored feet which appear red, white, or purple with thick nails, loss of hair and thin, brittle, or shiny skin, can all be signs of underlying circulatory issues or peripheral vascular disease,” says Jake.

NV Podiatrist Pushes for Creation of Certified Foot Exam Assistants
 
Dr. Larry Rubin, a retired podiatrist, is asking people to bare their "soles:" He and his wife, Arlene, are the founders of the Lower Extremity Amputation Prevention Alliance, which seeks to ensure more diabetics get their feet checked for loss of sensation and poor circulation. To do that, the LEAP Alliance is taking input from health professionals and diabetic patients on how to create awareness of how the simple test can save limbs. It is also proposing that Nevada have healthcare workers become certified foot examination assistants to help physicians perform annual preventative foot exams for diabetic patients.

In the past, LEAP has partnered with various Lions Clubs to do filament screenings at health fairs and other community events. Now, it is looking to expand the effort so that those who work in doctor's offices are trained and certified for more in-depth testing. Rubin said he hopes the effort will eventually trickle out to other states until it covers the entire country. LEAP says foot checks for diabetics should be a part of every doctor visit.

4 Myths about Using Electronic Funds Transfer in Healthcare - Part 5

Myth 4. Not all health plans offer the Healthcare EFT Standard
 
Health plans have been required by law since January 2014 to offer the Healthcare EFT Standard via ACH to any provider who requests it. If a health plan refuses to offer the Healthcare EFT Standard via ACH, providers should speak with the company’s compliance officer and, if necessary, file a HIPAA violation.
 
Additionally, some health plans are offering virtual cards for claims payments instead of EFTs and believe that this complies with the standard. It does not. The healthcare EFT standard is the NACHA CCD+ and must flow through the ACH Network. Providers have the option to say no to virtual cards, and all health plans must offer the healthcare EFT standard via ACH if it is requested.

Query: Removal and Replacement of Implant
 
What would be the best procedure code for the removal of a subtalar implant and re-implantation of new subtalar implant at the same time? The patient has Medicare Noridian.

Response: When removing and reinserting an implant, that is essentially billed as one procedure. There is no additional allowance to remove the original implant. You would only bill to insert the new implant. 
 
That said, many payers do not reimburse for subtalar implant (arthroereisis) procedures - investigational/experimental - so be sure you pre-authorize the procedure (S2117 or 0335T). Since Noridian does not have a mechanism for pre-authorizing the procedure, I strongly recommend that you have the patient sign an ABN so they know they may be held financially responsible.

I have treated many types of scars similar to this one. My main treatment is to perform an adhesiotomy. This is done by making a very small incision proximal to any adhesions and taking a small instrument, like a Freer elevator, and putting it as deep as possible and then sweeping side-to-side, freeing up the deep scarring underneath the entire scar.
 
Try not to allow the instrument to get too shallow. Once this is completed, it is important to make sure that there is not much bleeding, and if there is, apply a small drain for 24 hours. Then, begin application of topical Cica-Care Silastic Gel Sheeting for about one month. This technique is extremely effective in most cases.

Consider a musculo-cutaneous flap (like EDB, if any is left). Look for a primary closure of donor site which is often difficult in post-trauma cases, but at least you can put an STSG in an area of less pressure.

Even though this is a dated article from the Podiatry Institute circa 1993, it fully provides the information on scar revision. Even though I have provided this reference, I would not do any scar revisions on any patient if you have not had formal training to do so, because you could have a worse result than when the patient presented to you.

I recently joined a large multi-specialty hospital group. I too found this to be the LAST stumbling block. Salary, PTO, 403b, health coverage, etc. all seemed to be a snap compared to that negotiation. I finally settled a bit lower than what I asked for, yet compared to what the other codes fetch in terms of time and wRVU, I ultimately agreed.
