MO Podiatrist Discusses Growth Plate Injuries of the Heel
Karl Collins, DPM, FACFAS, a St. Louis-area foot and ankle surgeon, says heel pain occurs frequently in children ages 6 to 14 as their feet grow and the heel bone develops. "As children become more active in sports, they increase their risk for growth-plate injuries and subsequent heel pain," said Collins. This is especially true at back-to-school time when surgeons see an increase in middle and high school athletes experiencing heel pain with football and soccer seasons simultaneously underway.

"New bone forms in an area behind the heel, known as the growth plate, and cartilage is vulnerable to severe inflammation from strain or stress. With repeated stresses and strains from overactivity, the heel becomes very painful," Collins explained. In most cases, mild or moderate heel pain can be treated successfully with shoe inserts to soften the impact.

IMO Reaffirms Podiatrists' Role in Diabetes Management
The Irish Medical Organisation (IMO) has written to the Health Services Executice (HSE) to highlight that GPs who hold a GMS contract are not resourced to undertake chronic disease management in general practice. The move comes after the HSE attempted to introduce changes in diabetes work practices in the North East region in advance of additional resource provisions and a new GMS contract.
A letter from Dr. John McDermott, Chair of the HSE Dublin North East Diabetic Services Implementation Group, detailed the appointment of three full-time and two-part time podiatrists at hospitals. This is part of the National Diabetes Foot Care Programme and includes a national podiatry referral form for GP use. Dr. McDermott wrote that podiatrists would now be accepting referrals for diabetic patients with “high-risk feet.”

Compressing a hematoma is akin to compressing suspected compartment syndrome: don't do it unless a fasciotomy has been performed. Your best option now is to get an MRI or ultrasound (if you have access to reliable ultrasound technicians) in order to ascertain exactly the location and size of any remaining hematoma or fluid. Obviously, if the patient's pain becomes uncontrolled, then don't wait for the imaging. A return to the OR is the most likely outcome, regardless, in order to fully evacuate any remaining coagulated blood or entrapped fluids. I would be sure to identify and cauterize all bleeders and pack the wound open.

There is no better way to manage a post-op hematoma than to drain and irrigate, ASAP. Take action and open it up, particularly if it becomes indurated and hardens, as there is no way to aspirate it once it coagulates. The use of enzymatic treatments to break down the hematoma may seem an answer, but will increase the likelihood of infection and open a Pandora's box. Use a drain to close - the TLS is wonderful, but use whatever you're comfortable with. In essence, an incision and drainage of a post-op hematoma is essential.

About 9 years ago, I made the decision, for many reasons, to go to nursing school with the intention of leaving my private practice of 30 years. The location I was in would have required extensive upgrades, thus making the outright sale of the practice unlikely.
Instead, I merged my practice into a nearby 3-podiatrist practice. I was paid an upfront sum (not huge, but equitable given the economy). I continued to work in that office while attending nursing school at night. I was paid on a percentage of what I brought into the practice. This was great because it gave me an opportunity to introduce my old and valued patients to the other Docs, and it took the rigors of running my own practice off my shoulders while I was in nursing school. When it came time to leave podiatry for nursing, the transition was easy for my patients, the vast majority of whom stayed with the new practice. I'd recommend that the original poster look into some similar arrangement.

I fail to see how "name withheld" is a serial malpracticer when no judgment was made against him. I would have taken issue with the hospital.
When I first started in practice, podiatrists were getting sued, and our premiums were sky high. Why? Insurance companies were willing to settle every lawsuit, whether it had merit or not. Every negligence attorney knew that suing a podiatrist meant a quick settlement with very little work. For the insurance industry, it was cost-effective to settle every case and raise everyone's premiums at the end of the year, and even add on more profits for themselves. They did not care one iota that settling an unwarranted med mal case would be on a doctor's record and interfere with his or her ability to be on staff of certain hospitals. Today, carriers fight back and, unlike the past, they cannot force doctors to settle bogus, nuisance cases.

There is a group you can join called Doctor's Advocate: doctorsadvocate.org. For less than your state and national dues, you can become a member. Their attorneys and staff will handle independently any potential lawsuit, PR issues, threats, slander, etc. Since they are not an insurance company, they can contact your potential plaintiff and their attorney directly to discuss the matter and "educate" them on frivolous lawsuits and potential for counter-suits.
