Over-dependence on Shoes Can Increase Risk of Falls: AZ Podiatrist
Overusing shoes in settings that don’t require them can progressively weaken the feet, according to one functional podiatrist. Emily Splichal, DPM, of Chandler Arizona, explained that there is a common misconception that shoes offer more support to the body.

Dr. Splichal added, “Our toes need to push into the ground to maintain balance, and our foot muscles contract to maintain balance and posture.” Over-dependence on shoes could therefore lead to loss of sensory stimulation, which the body needs to maintain posture and avoid falls.

Podiatrists Discuss Staying Relevant with Increased Competition
Gene Mirkin, DPM and Bret Ribotsky, DPM were guests on The Fit Foot U Podcast with Ben Pearl, DPM. Key for podiatrists is the upcoming 4.5% fee schedule reduction for Medicare. Depending on the percentage of overhead, that potentiality is magnified. If overhead continues to increase, the challenge of the reduction increases. This short video clip highlights some of the ways to stay relevant.

One of the recurring themes is remaining positive and enthusiastic. Patients sense the difference when you are concentrating on the revenue aspect of the interaction versus focusing on the patient. Dr. Ribotsky experienced this as a patient after his boating accident. He added that we have a choice whether to be miserable or happy, which we make every day. Dr. Mirkin, who founded a group of 160 doctors in 90 cities, discussed the leverage of a large supergroup. The group has more negotiating power with insurance companies and creates economies of scale with overhead. Mirkin said that while changes can be administratively slow, revenue streams such as a joint lab can be created within a large group to promote profit.

Telehealth vs. Telewealth: Six Steps to Ensure Your Future
Your success or lack thereof will rest upon patient loyalty. Here are six steps to take:
3. Outservice your competition by finding your glue. Why do your patients stick with you? If you don’t know, ask them. You must nurture loyalty.

Listen with your ears and your eyes. Give the patient in front of you your undivided attention. Make that patient feel special. Start on time, and run on time. If you are running behind – even just a couple of minutes – apologize. And always thank your patient for entrusting their care in you.

Make it personal. Add reminders in your notes – e.g., patient enjoys playing golf at Lakeside, daughter is a freshman at LSU – to affirm your patient is more than just a chart number.

Expect your staff to do the same. They are an extension of you and can make or break you easily. Employees who do not connect with your patients are not the employees you want.

Use online registration and other tools to eliminate the dreaded ‘arrive 30 minutes early for your appointment’.

Avoid banker’s hours. Like it or not, you will need to offer hours more convenient for patients.

Query: Open Versus Closed Fracture
I have a nursing home patient that I have treated for an open wound over the top of her left great toe. I have not seen her in a few months. There is a wound care team at the facility that has been handling the wound. She fell recently and a mobile x-ray was performed. The report states a comminuted fracture of the proximal phalanx. The nursing home sent her back to me for treatment of the fracture. She still has the open wound. So my question… would you code the great toe fracture as open or closed? I normally think of an open fracture as a fracture of the bone with an associated laceration caused at the time of the fracture. In this case, there is a pre-existing wound with a fracture now below it.

Response: Regarding specifically the treatment of the fracture, the classification and use of open and closed refers to the technique the surgeon will use to repair the fracture.
So if you intend to surgically repair the fracture, with reduction and/or fixation, then consider:
CPT 28496 percutaneous fixation of fracture, great toe with manipulation
CPT 28505 open treatment of fracture, with or without fixation
If you intend to treat the fracture without surgery, consider:
CPT 28490 closed treatment of fracture, great toe, without manipulation
CPT 28495 with manipulation

I had a similar issue and while it was not easy, I was able to resolve this on my own. There are no federal or state laws requiring a DEA Number to order or prescribe non-controlled substances. This can easily be resolved by calling the NYS Board of Pharmacy and asking for some information which can be sent to your suppliers. Your NPI and license# should be a suitable substitute for the DEA Number.
As for the insurance companies, offer them a covering physician who would see the patient and Rx a controlled substance if the patient required one. And let them know you are not performing any procedures requiring controlled substances.

I agree wholeheartedly with Dr. Kass that laws should change to allow podiatrists to obtain all their CME virtually. I think it would help if APMA would come up with a statement that supports that stance. We learned during Covid that students at all levels could learn via Zoom; Fortune 500 companies had and still have employees working from home. It is time to have a national standard that eliminates the requirement for in-person learning. Of course, an exception would be for hands-on workshops.
