NYCPM Welcomes Class of 2020
NYCPM’s Class of 2020 has started digging into its studies following a three-day orientation that both made the new students feel comfortable and gave them a thorough and interesting foundation for their careers at NYCPM. Orientation started with a welcome by the school administration, led by President and CEO Louis L. Levine; an introduction of the students; a scavenger hunt; an overview of podiatric medicine by Michael J. Trepal, DPM, Vice President of Academic Affairs and Dean; an overview of student services by Lisa K. Lee, MA, Dean of Student Services and Enrollment Management; iPad mini distribution and training; lunch with Student Ambassadors, and presentations by student government and clubs.

And all of this was topped off with a Welcome Cruise of the Hudson River! The enthusiastic and engaged class is 45% female and 55% male. Its 86 members are an average of 24 years old, and represent 18 states and Canada; 44% are from the tri-state area. Among the students are several second-generation future DPMs.

AZ Podiatrist Delivers Surgery Grand Rounds at Westchester Medical Center
Dr. David G. Armstrong, Professor of Surgery and Director of the University of Arizona’s Southern Arizona Limb Salvage Alliance (SALSA) became the first ever podiatric surgeon to deliver grand rounds as a visiting professor at the institution.

Armstrong discussed his and other teams’ surgical, medical and mechanical efforts to treat patients through into diabetic foot remission. The event was streamed throughout the Westchester Medical System to trainees throughout the region.

Story About CO Podiatrist's Goodbye to Cat Goes Viral
This is Andrew the cat, a treasured member of Dr. Andrew Bernhard’s family for 15½ years. In this photo, he is holding hands with his owners as they drive to the vet to put him down. Last month, Bernhard shared this photo on Reddit with the caption: “He held our hands on his last trip to the vet. Little Andrew was much stronger than his mom and I.” “I was crying a lot, for sure,” Bernhard told BuzzFeed News of realizing that Andrew might know, on some level, that it was time to say goodbye.

Bernhard said that putting Andrew the cat down had become the only “fair” thing to do. “He was by my side for a long time and just had been running out of gas,” he said. “He wasn’t eating or drinking, stopped purring, and just wasn’t himself. It would’ve been selfish to keep him just because I didn’t want to lose him.”

Older Age Associated with DVT in Patients with Isolated Calcaneal Fractures
Recently published results showed patients with isolated calcaneal fractures had an incidence of pre-operative deep vein thrombosis that was almost twice as high as any reported in previously published examinations of lower extremity injuries. Researchers compared 159 patients with isolated calcaneal fractures who had pre-operative deep vein thrombosis (DVT) with a cohort of patients who did not have pre-operative DVT. At least 7 days after injury, all patients had a pre-operative duplex ultrasound performed, according to researchers.
Results showed 19 patients had pre-operative DVT, with most DVTs presenting distally. Using multiple logistic regression with the presence or absence of a DVT as the outcome variable and patient or injury characteristics as the potential predictor variables, researchers found older age was a risk factor for DVT. According to results, the odds of developing a DVT increased by approximately 6% for every one-year increase in age.

ACA UPDATE |
Changes to ACA Exchanges May be Enough to Keep Them Afloat for Now
Proposed changes to help stabilize the Affordable Care Act health insurance exchange markets should be enough to stop the bleeding, but further changes will likely be needed next year. The rule, which was released a couple of months earlier than expected includes changes in 2018 to the ACA risk-adjustment program as well as changes to plan requirements.
The rule changes are a response to tumult in the exchanges as Aetna, Humana, and UnitedHealth Group have all said they will be significantly scaling back their plan offerings in 2017. That and overall low enrollment from consumers has led some to question whether the exchanges can remain viable. There have also been concerns about double-digit premium increases in some states, although the HHS released an analysis stating that coverage would still be affordable for most consumers who receive premium subsidies.

Five Ways to Decrease Patient Wait Times
Practices should consider the following five tips to help improve patient wait times by operating more efficiently and increasing physician capacity:

With a physician champion on board, practices should analyze the average wait time for the third — not first — available appointment. Studying the wait time for the first available appointment is not a reliable metric due to the frequent cancellations and no-shows that occur at practices, which makes the "first available appointment" wait appear artificially brief.

RE: APMA Responds to Proposal to Fix Medicare Appeals Backlog

The APMA is currently working with CMS finding ways to speed up ALJ appeals as they are currently in an 18 to 36 month backlog. I think addressing the issue of why the ALJ is backlogged would be a more practical task. Far too often these days, Medicare is returning denials for the sake of denial. Diabetic patients meeting criteria for diabetic shoes with conditions such as former amputation and current ulceration, along with neuropathy or PAD are being denied. The second level appeal with clinical notes and every single document required by CMS, including the primary care doctor’s notes, are returned denied again as “not medically necessary.” This is not the crooked toe argument being pushed as a deformity requiring specialized shoes.
Both doctors and patients are suffering because of these purposeful denials of stated patient benefit, and the process is being punted to the courts. The unfortunate collateral damage is seen as doctors having to incur the initial cost of purchase, as Medicare will not provide pre-approvals, and the doctor again must incur the cost of processing and returns. The patient is unfortunately not receiving their stated benefit within the healthcare plan they were forced to pay into their entire working lives. I know many of us have seen these conditions lead to amputation. I think the APMA, armed with these specific cases and documentation of support, can make a very clean-cut case for doctors and patients.

RE: OSHA Penalties Increase Significantly

The penalties (sometime referred to as fines) that the Occupational Safety and Health Administration (OSHA) imposes on employers that are found to be in violation of OSHA Standards have recently increased significantly. Prior to the current increase, each serious violation could receive an OSHA penalty of up to $7,000. But now, each serious violation can receive a penalty of up to $12,500. That’s roughly an 80% increase.
Consider the fact that a typical small healthcare employer might be charged with several serious violations during a single OSHA inspection. Four or five violations at 12K each can quickly add up to penalties in the $48 to $60 thousand dollar range. And the penalty for a single violation that OSHA judges to be willful on the part of the employer is now permitted to be as high as $125,000.

I disagree with Dr. Busman’s comment that though "The term 'undervaulted' may be more understandable and useful (than overpronation) when discussing plantar fasciitis" to professionals, it would probably not be very informative to the magazine's target audience”. I offer an article that was published in 2010 in Radius Magazine that is available in doctors’ waiting rooms and online dealing with PF. It offers science, physics, and a logical progression as it develops the causes, diagnosis, and treatment of PF.
We have had almost 40 years to sell overpronation without success to the foot-suffering public; also overpronation (abnormal frontal plane eversion by the calcaneus) is a red herring for what actually occurs in most feet to create pathological tissue stress at the PF insertion into the medial calcaneus.
