PRACTICE MANAGEMENT TIP OF OF THE DAY

4 Myths about Using Electronic Funds Transfer in Healthcare - Part 4

Myth 3 - Healthcare EFTs via ACH are less secure than other methods of payment.

In reality, healthcare EFTs via ACH are among the safest forms of payment available. Funds are transferred directly from bank account to bank account via the ACH Network, which has been in use since the 1970s. Because EFTs are entirely electronic, using them eliminates the risk of a check being lost or stolen.

QUERIES (CLINICAL)
Query: Painful Cicatrix 40 Years Post Partial Amputation

My patient is a 49 year old male, who at 9 years old, sustained a traumatic amputation of his left foot from a riding lawnmower. The amputations were from the left 3rd interspace to the cuboid in guillotine fashion (dorsal to plantar). 40 years ago, the surgeon  placed a graft over the defect. Now this graft is bound down to bone and is immovable; it periodically ulcerates from traction as the graft did not grow with the patient as he matured.

Painful Cicatrix 40 Years Post Partial Amputation

This graft  is about 5 cm in diameter with a healing ulcer noted about 9 o’clock.  My question is: Since this scar is painful due to its inelasticity, does anyone have any advice as to how to proceed, conservative to heroic?

QUERIES (DME)
Query: L3000 WRVU Equivalent

I am currently renegotiating my contract with a large hospital network. I am on a strict WRVU compensation plan. I am not getting any compensation for the custom orthotics that I make and give to my patients because there is no WRVU equivalent for the L3000 code. Is there anyone who is employed by a hospital who is getting compensation for the orthotics they make even though there is technically no WRVU equivalent. If so, can you please explain your compensation model specifically for the orthotics?

QUERIES (NON-CLINICAL)
Query: Brican Patient Education System

I am looking for help from anyone who has a Brican patient education system. This system worked well until my server died and Windows had to be re-installed. In so doing, the Brican system has to be re-registered to work. The company has gone out of business and I have been told that even though I bought the product, there is nothing that they can do except connect me with a technician who can "hack" the system for thousands of dollars.  Any advice?

RESPONSES/COMMENTS (CLINICAL)
From: Stephen Peslar, BSc, DCh

I have been reading the opinions from podiatrists and, to date, no podiatrist has discussed the insurance companies' strategy to maximize profits by reducing payouts for prescribed medical appliances. This is why Dr. Udell is incorrect when he wrote insurance companies, "want to see current, well-written research papers." These companies only want to see profits and they do this by reducing payouts.

I remember 25 years ago, perusing patients' insurance benefits booklets and the benefits covered: prescription orthotics, $400; orthpaedic shoes, $400; braces,...

RE: The Woman Who Had Orgasms in Her Left Foot

Photo: Brand New Images/Getty Images

Mrs. A was a 55-year-old woman living in the Netherlands who began experiencing a strange and embarrassing problem in 2010: Five to six times a day, she felt as if she was having an orgasm ... in her left foot. As in, it felt just the same as an orgasm that happened during sex would, only the sensation began in her foot. In a 2013 report published in The Journal of Sexual Medicine, her medical team discusses her condition, which they say is the first...
