NC Podiatrist Qualifies for Triathlon World Championship
 
Asked what led him to get involved in competing in triathlons, Dr. Matthew Thompson laughed and said, "There's something wrong with me." Based on his triathlon record, there's nothing wrong with him physically, at least. He has competed in a number of races and will be headed to the world championships in Australia next month. Thompson is a versatile individual. He's a podiatrist who practices in Lumberton and Fayetteville. He's also an accomplished painter, with some of his works appearing on the walls at the clinic where he works.

Thompson competes in the 60-65 age group and said a good time for his class is under six hours for the half triathlon and under 14 for the full. He recently posted a personal best, fifth, in a half triathlon in Raleigh - his first time winning a trophy and earning a spot on the podium. He got an even bigger thrill when he learned that he had been qualified to compete in this year's world championships next month at Mooloolaba, Australia.

Dr. Lee Wittenberg and Apache Foot & Ankle Specialists have announced the arrival of Dr. Christina Hulsebos – Foot Doctor in Las Vegas to the team. A newly created web page details her credentials in education and experience. Dr. Hulsebos worked as an associate in a group practice in southern California following her residency and moved to Las Vegas in 2016.

UNLV Community Concert Band to Help Lions Task Force Against Diabetes

Proceeds from ticket sales for a November 9th University of Nevada, Las Vegas (UNLV) Community Concert Band performance in the 1,800-seat Ham Hall will help prevent low income diabetics in Nevada from losing their legs needlessly, according to Dr. Lawrence M. Rubin, the podiatrist director of the non-profit Lower Extremity Amputation Prevention (LEAP) Alliance Task Force Against Diabetes.

“Leg amputations can begin with a minor foot problem such as a thick toenail, a blister, or an inflamed corn,” says Rubin. These common foot ailments can cause pressure points that become open sores, and the sores can become severely infected and lead to eventual amputation. According to the American Podiatric Medical Association, up to 80% of these amputations can be prevented when diabetics are under the care of a podiatrist.

Ali Sadreih: I have tried to place the patient above everything else. I have written a post-op manual that I give to each surgical patient. I direct them to my website which has many videos that explain everything from what to do if your dressing gets wet, to what to expect pre- and post-surgery.  All in all, I have tried to make the experience the best for the patient.

Five Ways to Decrease Patient Wait Times
 
Practices should consider the following five tips to help improve patient wait times by operating more efficiently and increasing physician capacity:
 
1. Find a physician champion
 
The first step is to find a physician leader who wants to drive change and understands how patient access influences care quality and financial performance. Any scheduling or workflow change designed to improve efficiency or capacity is likely to receive a warmer reception from a clinical colleague aligned with the practice manager.

I don't think a fusion is possible at this point. Perform an arthroplasty. Find out the manufacturer of the screw to have appropriate instrumentation for removal after using a reamer.

This case presents several unfortunate red flags to me as you go forward in your treatment choices. The 3 major red flags: 1) the patient is a smoker, 2) you indicate he stated the screw was stripped during surgery; how could he possibly know that? To me, that raises concerns of a litigious patient. 3) The surgery has left a wide gap at the IPJ.
 
If it was a fusion for trauma. There is a fair chance that the IPJ sesamoid ligament was torn from its insertion and is incarcerated in the IPJ. You stated that there is DF & PF with one grade of weakness. What are the goals of the surgery or treatment you want to accomplish?

I realize that I do not have the same billing issues that most private practitioners do; however, I do face the same issues of Charcot joints that chronically break down into ulcers on the bottom of the foot. The big issue here is whether the treating physician can document what would happen if he lets the patient go longer than two weeks.
 
What happens if he lets the patient go 2.5 weeks or 3 weeks between visits? Are there pre-ulcerative changes in the keratoses? I have some Charcot joint patients who, if I don’t see them every 2-3 weeks, will come in with a new ulceration under the foot. This is documented. The fact that a callus can build up in 2 weeks time would necessitate debridement speaks greatly about the ineffectiveness of the shoes/orthotics that the patient is wearing.
 
One of the things that would help document the need for a check-up is a pedobarograph examination of the patient wearing the diabetic shoes/orthotics. Even with the best shoe/orthotic therapy, pathological forces cannot always be adequately alleviated to prevent callus and ulcer formation in the Charcot joint patient.

I was reading a post which highlighted Dr. Andrew Glass' "cutting edge procedure" for bunions. I realize this was a news story and not a post from Dr. Glass, so this comment is in no way directed towards him.
 
I just found the phrase, "cutting edge procedure" somewhat comical. I have been performing this procedure for 30 years with the same excellent results, and so have many other DPMs. But I can tell you, I have patients every week who had consulted other DPMs who wanted to perform a procedure that would keep them off their feet for anywhere from 3-8 weeks. People are looking for better options.  Keep up the good work, Dr. Glass!
