NY Podiatrist Discusses Advantages of MIS Bunion Surgery
Traditional bunion surgery meant breaking a bone in the foot, inserting pins, and staying off your feet for at least six weeks. But podiatrist Dr. Andrew Glass has a different approach. Now, a cutting-edge procedure can have people back on their feet fast.

Glass’ surgical method uses a tiny — rather than inches-long — incision. Shaving rather than breaking the bone means less trauma, and, he says, patients are on their feet the same day. “That makes a big difference for your recovery time, so you can go back into a shoe so much faster because you’re not breaking the bone,” Glass said. He also says there is less pain after the procedure, and patients are back to work in about a week.

Dr. Suniti Patel, from Neville Foot and Ankle Centers, is teaming with Memorial Hermann Hospital to establish a Limb Salvage Team to care for diabetics’ limbs and help prevent amputation.

A limb salvage team involves pairing specialists, such as podiatrists and vascular surgeons together to help reduce the time it could take for diabetes patients to receive proper care. Its ultimate goal is to help prevent and reduce the number of amputations among diabetic patients.

Query: Painful Non-union of Hallux IPJ
A 23 year old smoker who was seen 8 months status post an attempted IPJ fusion states that the screw was stripped during the original surgery. Hallux dorsiflexion and plantar flexion are a low 4/5 strength.

Any tips on revision surgery, especially how to remove the screw while preserving the distal phalanx to attempt revision fusion? I will try a Synthes conical extraction screw. I may have to resort to a conical reamer.

RE: CMS Grace Period for ICD-10 to End
For those of you who never even realized it, the one-year grace period in effect for ICD-10 announced last summer by CMS and AMA will end on October 1.
One of the complaints associated with the "grace period" was that CMS never coordinated a similar grace period from commercial payers. And the majority of those payers did not go along with CMS's "flexibility". Over the past year, I have remarked that essentially CMS's/AMA's announcement of flexibility was little more than a public relations effort for that very reason. If a practice needs to code the highest level of specific for non-Medicare payers, are they really going to change to "grace" mode for Medicare claims? Really?
Regardless, CMS noted that on October 1, 2016, providers will be required to use the "correct degree of specificity" when coding ICD-10 on their claims. That is interpreted as, for example, only using unspecified codes when the situation warranted their use and ensuring that the ICD-10 codes billed and the medical record documentation are supportive of one another.
Interestingly enough, the years of delays leading up to ICD-10 implementation allowed many practices - providers and coders - time to prepare for the transition from ICD-9 to ICD-10. All-in-all, the transition was relatively smooth with minimal glitches, confusion, and loss of productivity reported.

RE: Double Standard for Doctors

Recently, in our office, we were discussing what we found to be interesting situations in our country:
1. Medical professionals are required to be more careful and responsible with patients' charts than members of our State Department are with "top secret" documents!?
2. It is more suspicious for medical professionals to accept a "ball point pen" from a drug rep. to speak with them about their product than for members of the State Department to accept money for someone to speak about their interests?
3. If we would have established a "charitable organization" rather than a medical practice, we could have avoided a great deal of oversight?!

This is an interesting subject and I thank Dr. Levy for publishing on it. We used an abbreviated REALM test on 41 patients in our wound center recently. The test was distributed among patients in percentages equal to their insurance, so any one group was not targeted. The resultant average score in our clinic was ages 5-7 years, basically at a pre-school level. This may be one reason that patients do not follow instructions, even when they say they understand.

In yesterday's PM News, Dr. Shavelson took issue with Dr. Pribut's use of the term "over-pronation" as a possible contributing cause to plantar fasciitis: "Also, many seem to agree that there are words in our biomechanical language that are ambiguous or possibly even unproven by EBM. One of those is “over-pronation” as quoted by Dr. Pribut."
While we all know that there are many contributing factors and that newer research is providing better insight into this common condition, we need to remember the audience that Dr. Pribut was writing for. His comments were extracted from Experience Life Magazine, a layperson's magazine of health and fitness. It may be true that "The term “undervaulted” may be more understandable and useful when discussing plantar fasciitis" to professionals, it would probably not be very informative to the magazine's target audience. I applaud Experience Life for quoting a podiatrist, not just the handiest orthopod.
