PA Podiatrist Discusses Aging Feet

Just like the rest of our bodies, feet change as we age. Lynn Donnelly of Media, PA spent most of her working years on her feet as a nurse. Podiatrist Dr. John Pron of Riddle Hospital Main Line Health says you don't need a job like that for time to take a toll on your feet. For people who are too sedentary, the 33 joints and nearly 100 tendons and ligaments in the foot can dry out and become tight. "Bones can shift, joints can move. And now we're putting pressure on things we shouldn't put pressure on," said Dr. Pron.

Extra weight also adds pressure to feet and knees - 3 pounds for each one over your target weight. But Dr. Pron says staying active can head off many problems! NJ Podiatrist Discusses Bunion Prevention 
 
If you have bunions, taking care of them now can help you avoid more serious treatment later, an expert says. Left untreated, bunions can cause debilitating pain and may require surgery to correct, said Dr. Brent Rosenthal, a podiatrist and podiatric surgeon at CentraState Medical Center in Freehold, N.J. To prevent bunions, avoid wearing shoes with pointed triangular tips and don't wear high heels for long periods of time each day, Rosenthal said.

You should know your correct shoe size, which can increase with age, weight gain and pregnancy, he added. If you do develop a bunion, start wearing shoes with a wide and deep toe box. Shoe inserts may reduce symptoms and prevent the bunion from getting worse, according to Rosenthal.

CMS Finalizes EHR Meaningful-Use Rule, Adds Some Flexibility
 
The CMS, late Friday, finalized a rule allowing hospitals and eligible professionals more flexibility in how they meet meaningful-use requirements for the electronic health-record incentive program. The agency had first proposed the idea in a May draft rule. Friday's final rule left the May proposal unchanged.
 
The rule would grant providers a longer timeline and more flexibility in meeting the incentive goals laid out by the stimulus program first created in the 2009 HITECH Act. The incentive program provides doctors and hospitals stimulus funding to implement electronic health records. The rule pushes back the beginning of the third stage of meaningful use for the first cohort of adopters until Jan. 1, 2017, as opposed to the old standard of Jan. 1, 2016.

SUNSHINE ACT UPDATE
Open Payments Site Scheduled for More Downtime; Complaints Continue
 
The troubled Open Payments Act website is encountering a bit more turbulence. The CMS announced Thursday that it plans upcoming outages for the site and that downtime would be used for “scheduled maintenance upgrades.” As a result, the review-and-dispute process for doctor data displayed on the site will be extended until Sept. 10, the agency said. That delay will push the corrections period to Sept. 25. The agency still says that the data will be released to the public Sept. 30. The outages are scheduled for Aug. 30 between 1 and 11 a.m., and Sept. 5 for an as-yet-undisclosed duration.
 
The latest delay comes after trade group Pharmaceutical Research and Manufacturers of America questioned the agency's transparency in rejecting data submissions from manufacturers for the site. The CMS, concerned with the possibility of intermingling, that is, matching payments data with the wrong doctor, has reportedly sent back about one-third of records for manufacturer correction.

QUERIES CLINICAL
Query: Post-op Hematoma
 
I removed a bone fragment from the first intermetatarsal space at the base of the proximal phalange. Initially, all went well, but after sutures were removed, the patient developed a painful distension in the area. I have compressed it with an Unna boot, with no improvement. I think there is probably a hematoma. Does anyone have any hints for resolving this other than time?

My associate gave a woman an injection and she developed a cellulitis/allergic reaction. She was treated successfully, but still has slight edema. She asked to be financially compensated for what she claims was two weeks of work lost. It now turns out her health insurance was inactive at the time of her visit. How would you handle this?

Query: E/M Bullet Points for Exam
 
When counting the bullet points for an exam to determine what office visit level to code, does each limb count as a bullet point? I'm being told by my billing office that the only time that bilateral counts is for the musculoskeletal. So, a vascular palpation of DP/PT pulses either on one or both feet counts as only one bullet. Is this correct? Can someone lead me to the place to read the exact definition?

RE: Using Cell Phones to Supply X-Ray Copies

I have recently discovered a novel approach for patients to get a copy of their x-rays. They simply take a picture with their cell phone while the x-rays are being viewed on a plain old-fashioned view box. They come out great! They are also able to enlarge the view like any other photo. I don't know if they print well.

RE: Podiatric Medicine and Colorectal Cancer (Leonard A. Levy, DPM, MPH)

In May 2005, there was a paper published in the New England Journal of Medicine correlating lower rates of colorectal cancer and the use of statins to treat hyperlipidemia. The findings were challenged by three physicians at the Brigham Hospital who asserted that the cohort of patients studied was unusual in that these people were taking statins. This meant that they were under greater medical care, and hence their physicians would have screened them more carefully for blood in their stools or ordered colonoscopies where appropriate. Thus, the lower rate of colorectal cancer was not a benefit of statins, but a result of the greater medical attention these patients were receiving.
 
The findings in the paper cited by Dr. Levy might be attributed to the same thing. People with peripheral vascular disease who are well treated may indeed be in a population that is under greater medical scrutiny, and hence are screened much earlier for any signs of colorectal cancer.
 
We podiatrists, because of the age of the patients we tend to see, are in a greater position to ask if a person above the age of 50 has been screened for colon cancer, and if need be, we can make appropriate referrals.
