TPMF Mission Treats 600 in Mexico
In 2 days of clinic, 600 patients were treated, and 48 selected for surgeries. Over 400 orthotics were provided courtesy of JM Orthotics- Forward Motion Team during clinic. Also, 65 new pairs of shoes and socks were provided to two orphanages, along with foot screenings and meds. Ten Texas podiatric residents were sponsored to attend the mission this year. Many people were provided treatment and medicines that they could not get otherwise.

This is the 18th year of the Texas Podiatric Medical Foundation (TPMF) supporting medical missions to Mexico. The mission was originally started by Joseph Caporusso, DPM in Reynosa, Mexico, and then was moved to San Miguel in 2010 under the leadership of William Van Pelt, DPM and Donald Falknor, DPM.

IL Podiatrist Describes How Leg and Foot Muscles Interact
The feet control where we walk or run, but they can also have an effect on our knees, hips, and lower back. In the same vein, parts of our legs actually help control muscles in our feet. According to Lisa M. Schoene, DPM, ATC, a triple board certified sports medicine podiatrist and athletic trainer practicing at Gurnee Podiatry & Sports Medicine Associates, there are 19 internal muscles and 13 external muscles that move the foot and toes. “The internal muscles move the toes and help to keep all the toes flat while walking and balancing,” she notes.

“The external muscles originate below the knee and enter the foot to move the foot up-and-down, in and out, and [help] to lift the heel. In the leg, other than the gastrocnemius and soleus muscles, there are five ‘secondary’ heel lifters. These are very important for running gait and are the posterior tibialis muscle, flexor digitorum longus, flexor hallucis longus, peroneus brevis and peroneus longus. These muscles help propel the foot forward and stabilize side to side as well,” says Dr. Schoene.

The paper received a manuscript award at the 2019 ACFAS ASC and will be presented at the upcoming 2020 AAOS Annual Meeting. This publication is the latest installment of the Institute’s quality assurance and research program, which aims to combine cutting edge surgical techniques with patient-centered outcomes to produce consistent and superior operative results.

Six technologies have profound impacts on patient engagement and practice workflow, according to a new report. The report from Medical Group Management Association and Cedar found these technologies have helped improve the patient-physician relationship, mitigate rising costs, and address the challenges of patient consumerism. Other added benefits include improved connectivity, streamlined accessibility, elimination of paper documents and financial gains from a simplified billing process.
3. Check-in Technologies - More practices allow patients to check in for appointments using online, kiosks, and tablets. One practice found that 75% of patients pre-registered for their appointments online within six months. Patients were more complete and accurate than when filled out on pen and paper.

Query: Billing for Profore Dressing
The wrapping portion of the dressing, A6441, A6443, A6449, and A6454, are listed as per yard. Profore is 3.5 meters for each of these items. Generally, the entire wrap is used on each leg. Should we be billing this as three units? A previous post stated that Profore is billed as a single unit.

Response: The application of a Profore dressing is billed using CPT code 29581: Application of multi-layer compression system; leg (below knee), including ankle and foot.
What about the supplies, or the “wrapping portion of the dressing?” The HCPCS Level II codes that are generally used are the following:
A6207: Contact layer, sterile, more than 16 sq. in., but less than or equal to 48 sq. in., each dressing
A6441: Padding bandage, non-elastic, non-woven/non-knitted, width greater than or equal to three inches and less than five inches, per yard
A6443: Conforming bandage, non-elastic, knitted/woven, non-sterile, width greater than or equal to three inches and less than five inches, per yard
A6449: Light compression bandage, elastic, knitted/woven, width greater than or equal to three inches and less than five inches, per yard
A6454: Self-adherent bandage, elastic, non-knitted/non-woven, width greater than or equal to three inches and less than five inches, per yard
I believe that each of the respective layers should be billed as one unit only. The supplies are not billed to the Durable Medical Equipment Regional Carrier. They are billed to the respective Medical Administrative Carrier (i.e. MAC).

You’re wasting time and money and resources. It is extremely rare to have a B/L tarsal tunnel compression or syndrome ("TTS"). When I suggested this as a differential in my earlier post in response to your post, I should have emphasized that it is my impression that your patient is 99.9% more likely to have the proximal problem; that is, the nerve root (back, vertebrae) in this B/L condition. I have seen enough of these and all the variations to think I am right. The nerve conduction velocity exam ("NCV") is a waste, even if it were accurate; it may not be any more accurate, as your clinical evaluation is just as effective in your diagnosis of TTS. Send the patient to a neurosurgeon. Or, make a friend, and send the patient to an orthopedist.

Yesterday, I ordered a bottle of 100 celecoxib, 200mg from Henry Schein, Inc. for $8.49 plus shipping. That's 88.5 cents apiece when a patient of mine last week was going to be charged $7.29 apiece for the same pills off of a prescription at a chain pharmacy. I understand and accept the fact that the real citizens of this country are the corporations. But they have to be controlled, like we, the original citizens have been controlled. Hopefully, the judgment against Johnson & Johnson this week in Oklahoma will be the start of a turn-around.
