Spanish Podologists Gain Critical Surgical Skills at NYCPM
 
Anatomical dissection and surgical skills were among the subjects that thirteen podologists from Spain studied during a week in August at NYCPM’s Bako Anatomy Lab. Michael J. Trepal, DPM, Vice President for Academic Affairs and Dean, directs the semi-annual program for NYCPM. Professors Ricardo Becerro de Bengoa Vallejo, DPM, PhD, a member of NYCPM’s Class of 1995, and Marta Los Iglesias, PhD, work with Dr. Trepal to direct the program.

New Mexico Albuquerque VA Hires New Podiatric Section Chief
 
The Raymond G. Murphy VA Medical Center in Albuquerque has announced the hiring of S. Jeffrey Siegel, DPM. Dr. Siegel graduated from Samuel Merritt University/California College of Podiatric Medicine in 1990 with top honors, and completed an adult and pediatric reconstructive surgical residency at Kaiser Permanente Medical Center in Santa Clara, CA.

Over the years, Siegel has held many academic positions; his most recent posts were at TUSPM as an Adjunct Professor and at Cooper University Hospital and Medical School, a Level 1 trauma center, as an Attending Physician. Siegel replaces Dr. Mark Margiotta who left the VA after 24 years of federal service for private practice in Albuquerque, and will continue to be an active member of the teaching faculty in the VA’s prestigious residency program.

KS Podiatrist Delivers Keynote Address at Women of Influence Reception
 
Topeka native Dr. Priti Lakhani knows from personal experience that circumstantial constraints can influence an individual’s capabilities. Lakhani, who works for Cerner as an adviser to hospital corporations, shared personal stories Wednesday in a keynote address at the Women of Influence reception at the Capitol Plaza Hotel to illustrate that catalyst. She challenged attendees to ask the event’s honorees what their constraints were and how they defied them.

When Lakhani was 6 years old, she arrived home after school to find her mother lying on the floor. Lakhani said she didn’t know it then, but her mother had contracted typhoid fever, which she said still carries a 25 percent mortality rate in India. “My only thought — my singular, recursive rumination — was simply this: ‘She needs a doctor. She needs a doctor. She needs a doctor,’” she said. “So I became a doctor.” Lakhani, a surgical podiatrist and social entrepreneur focused on global health, identifies it as “a seminal moment” in her life. “I watched those that were really dear to me suffer, and that became the fire that fueled me. That constraint became my capability,” she said.

Thermogram Mat is Early Detector of DFUs: AZ Podiatrist 
 
One good thing about foot ulcers is their early warning system. When a foot ulcer is developing, it may not be obvious even to a doctor, but it will likely be a warmer spot on your foot. To take advantage of this warning, a new medical system uses temperature data to predict that an ulcer will develop more than a month before it surfaces. With this system, doctors would have more time to treat it. Dr. Robert Frykberg was the principal investigator of a prospective, multicenter cohort study that followed 129 high-risk people for 34 weeks. Those at a highest risk of developing a foot ulcer are people who already had one, he says. The study included only those people with diabetes who had a healed ulcer or an amputation of the foot.

During the study, 37 of the subjects developed a total of 53 ulcers. The study showed that using the mat could predict the development of 97 percent of the ulcers about five weeks ahead of time. “People who use it have nothing to do except keep it turned on and step on it each morning,” Dr. Frykberg says. This takes a thermogram of their feet and sends off the data to a central server. A difference of four or more degrees Fahrenheit on two successive days will register an alert, which is then transmitted to the user and to his or her doctor for necessary treatment.

The technique I like the most is to be sure to drive the surgical needle from the nail bed out to nail, not from the nail plate to nail bed. This helps avoid the nail debris, which the body reacts to as though it is a foreign body, potentially creating more harm than good. There are many techniques outlined in the first few chapters of McGlamry's textbook, Comprehensive Textbook of Foot and Ankle Surgery. Inform the patient that the toenail will not grow back normal, and is better off removed.

I would like to thank Dr. Levy for sharing the importance of the work published in the July 2016 edition of Diabetes Care. The paper, "Cognitive Dysfunction: Part and Parcel of the Diabetic Foot", which Dr. Levy cites, found there was decreased cognitive status in patients with diabetic foot ulcerations when compared to diabetic patients without foot ulcerations. Considering the high correlation between diabetic foot ulcerations and polyneuropathy, this work suggests a possible relationship between peripheral and central nervous system degeneration in diabetics.
 
Although diabetes is a well known cause of both peripheral and central neurodegeneration, this study raises the possibility of a more direct correlation between the two. Are patients with diabetic polyneuropathy more prone to decreased cognitive status than diabetics who do not suffer from polyneuropathy?

I appreciate Dr. Siegel's point of view on borrowing money, and leverage toward investments. Earlier in my life, this is how the math worked out for me also. I also thought that if we borrowed money at a lower interest rate than what we expected to earn on our investments, we must come out ahead. I do not consider myself naive about investments, as I have invested in mutual funds and the market since I was in my first year in college. Over the years though, I did find myself naive about the math.  
 
Risk versus reward plus psychology plays a significant role in how we manage our money. I am not a gambling man, but I would wager the banks who loan us this money are quite aware of investing in mutual funds and leverage as a tactic to earn higher rates of return. I do not consider myself smarter at the math than the bankers at these large institutions, yet most of us around the country think we are outsmarting the banks at their own game. Cash is king.

In think-tanking with colleagues about biomechanics, many of us are now doing something to raise the navicular and drop the 1st metatarsal when casting and prescribing custom foot orthotics beyond “STJ Neutral casting positioning”. Also, many seem to agree that there are words in our biomechanical language that are ambiguous or possibly even unproven by EBM. One of those is “over-pronation” as quoted by Dr. Pribut.
 
If we are becoming more concerned with sagittal plane repair and elevating the “arch” of collapsed feet, and if we are moving more towards 3-D scanning, x-raying, and modelling, perhaps the term “undervaulted” may be more understandable and useful when discussing plantar fasciitis. This is a more accurate term because it better describes the 3-D nature of the problem as well as the fact that we are dealing with a 3-D foot, and not simply the inward and outward rolling of the rearfoot in closed chain.

Dr. Rogers deserves to be furious over Mylan Pharmaceutical's price gouging. Any human being of sound mind, hearing this on the news must be upset to hear about a drug company increasing the cost of its potentially life-saving drug from 57 dollars to over 600 dollars. We all know, however, that this is not the first time that drug companies have jacked up their prices overnight. Look at econazole nitrate which most of us prescribed to treat dermatophytosis. It went from a buck fifty for an 80 gram tube to over two hundred and fifty dollars for the same size tube. Percentage wise, that is far more of an increase than what Mylan is getting away with. What about what we pay for a small vial of dexamethazone? In 2016, as compared to what it cost us two years ago, the  increase in price is also more than 400%.
 
I hope that enough people stay upset so that this price gouging does not get swept under the carpet. Congress must either reign in the makers of both brand and generic medications or allow the public to buy medications directly from Canada where drug prices are controlled by the government.
