IL Podiatrist Performs Unique Procedure to Save Dancer's Career
 
Fourteen-year-old Jacalyn Tatro’s worst fears happened last year in front of an audience of judges evaluating her dance technique at a national dance competition in New York City.  The young Naperville dancer landed awkwardly on one ankle, rupturing her anterior talofibular ligament--the main ligament of the ankle. Flash forward to a year later and Tatro made a remarkable comeback, winning the same competition she injured her ankle at.

Fortunately, her mom knew a gymnast who had been successfully treated for a lower limb injury by Naperville board certified podiatric physician Lawrence Kosova, DPM. “I repaired her ankle by not locking it down or tightening her ankle as I normally would,” he explained. “I fixed her loose, since I had to keep her injured ankle as  loose as her hypermobile other ankle. I also used her ligament as a graft to support this fix, since it was just falling off,” Kosova said. He had researched similar injuries and possible surgical approaches in the medical literature but found nothing similar.

Children's Foot Problems Should be Treated Early: Canadian Podiatrist
 
Correcting foot problems in children is relatively easy because children’s bones and joints are flexible. But left untreated, these problems can cause pain and severe complications once the child is an adult. Dr. Marilyne Langlois is the owner of Clinique Podiatrique Kirkland. A certified podiatrist, Langlois completed a four-year program to become a doctor of podiatric medicine from the only podiatry school in Quebec, Université du Québec à Trois-Rivières. She completed an internship in the United States before returning home to Montreal.

Every child should see a podiatrist for an evaluation around the age of 2 or 3. Common foot and posture problems begin when a child starts to walk. Signs of a problem are flat feet, feet that point inward or outward, excessive falling when the child walks or runs, and—rarely, since it’s less common at a young age—complaints of foot or back pain.

GA Podiatrist Discusses Overcoming Practice Management Challenges   
 
Dr. David Helfman, CEO of Village Podiatry Centers, recently appeared on Top Docs Radio with host Charles Hall. The segment touched on the challenges faced when starting the solo practice in 1992 and discusses the unique approach Dr. Helfman took in order to create a thriving business. Village Podiatry Centers has grown substantially over the past couple of decades and is now recognized as the largest podiatric practice in the Southeast. With over 30 offices in Metro Atlanta and Middle Georgia, the practice continues to evolve despite the rapid changes within the healthcare market.

FDA Okays Submicron Diclofenac (Zorvolex) for Osteoarthritis
 
The U.S. Food and Drug Administration (FDA) has approved diclofenac capsules (Zorvolex, Iroko Pharmaceuticals) for the management of osteoarthritis (OA) pain, the company announced today. OA pain is the second indication for Zorvolex, first approved by the FDA in October 2013 for mild-to-moderate acute pain in adults, as reported by Medscape Medical News.
 
The recommended dosages are 18 or 35 mg 3 times per day for acute pain and 35 mg 3 times daily for OA pain. Zorvolex is the first FDA-approved low-dose nonsteroidal anti-inflammatory drug (NSAID) developed by using proprietary SoluMatrix Fine Particle Technology, the company notes in a news release. The drug contains diclofenac as submicron particles that are approximately 20 times smaller than their original size. The reduction in particle size provides an increased surface area, leading to faster dissolution, the company says.

I had a very similar presentation in an older patient. There were positive wound cultures with Pseudomonal colonization. The condition was non-responsive to antimicrobial therapy systemically, and topically with vigilant local care. Biopsy eventually confirmed an invasive squamous cell carcinoma. If this condition fails to respond as one would expect on a timely basis, consider a biopsy.

RE: Podiatric Medicine and Colorectal Cancer

The broad spectrum of podiatric medicine and its implications to other fields of medicine and surgery are too often marginalized by the profession. The current issue of Diabetes Care (August 2014; 37:2296-2301)) published by the American Diabetes Association includes an article by Margolis, Hoffman, and Weibe entitled, "Lower-Extremity Amputation Risk Is Associated With Variation in Behavioral Risk Factor Surveillance System Reponses."  Among the dramatic conclusions of this study is that regional variation (i.e. geographic) in colorectal cancer screening was associated with regional variation in lower extremity amputation (LEA). Regions that had the higher rates of LEA had lower rates of colon cancer screening.

In addition to all of the pitfalls so correctly and astutely mentioned by Dr. Jacobs, sale of prescription medications by a physician in the physician's office may be a violation of the pharmacy laws in some states. In Virginia, for instance, it is a felony for any physician (MD, DPM, or DO) to prepare and sell prescription medications unless the physician is also licensed as a pharmacist.

Dr. Jacobs is correct when he states, "In-office compounding has been around a long time." My dad is a retired pharmacist. When I was little kid in the 1950s, I remember visiting his drug store. In those days, pharmacists were not highly educated professionals. Their roles were often reduced to taking pills out of a stock bottle, counting them out into a small plastic bottle, and slapping  a label on it. In those days, all physicians  wrote for compounds, and my father showed me how he would hand-make ointments, cough syrups, pills, and suppositories based on physicians' prescriptions (which were always written in Latin). With compounding pharmacies, the profession has come full circle and pharmacists are once again doing the work that measures up to their years of intense training.
