Bahamas Podiatrist Discusses Children's Feet
According to podiatrist Dr. Monique Mitchell, "Nearly all children appear to have flat feet when they first start walking. This is partly due to posture and is associated with fatty deposits in the foot. When babies walk, they have to balance a relatively large head and torso, so they walk with the knees bent, legs wide apart, and the feet turned outwards."

"Many of the problems found in children's feet are associated with growth, overuse, weight gain, and postural changes. Genetics also play a crucial role in the development of your child's feet. It is estimated that 10 to 20 percent of children have flat feet. Most children grow out of in-toeing, out-toeing, and toe-walking by 24 months of age," says Mitchell.

Forensic podiatry took the podium for several presentations at the 99th Educational Conference of the International Association for Identification in Minneapolis August 10-15, 2014. At the IAI conference, members of the American Society of Forensic Podiatry (ASFP) gave sessions to educate attendees about forensic podiatry and how it can assist in a criminal investigation. The ASFP, established in 2003, promotes a research-based podiatric approach in forensic cases and also promotes forensic educational opportunities.

Dia-Foot Acquires DavMar Comfort Shoes
Dia-Foot has announced the acquisition of DavMar Comfort Shoes. Dia-Foot, a Florida based company also specializes in shoes for diabetics. The transition took effect on August 28th. Dia-Foot’s President and CEO, Robert M. Gaynor, DPM, C.Ped, is a podiatrist who is participating in the crusade against the damage that diabetes can cause. By making available specially manufactured shoes and inserts, he is hoping to reduce the number of severe complications caused by the disease. Since forming Dia-Foot 10 years ago, the privately-held company now has over 25 employees.

This blending of DavMar and Dia-Foot will provide more comprehensive attention to the fast-growing population of diabetics. As in the past, the company will continue to provide footwear to podiatrists, orthotists, prosthetists, pedorthists, and pharmacies that are treating those who have special needs.

CA Medical Board to Post Doctor Discipline Online Indefinitely
Gov. Jerry Brown has signed legislation that requires the Medical Board of California to post public information on serious disciplinary action against doctors online indefinitely. Assembly Bill 1886 by Democratic Assemblywoman Susan Talamantes Eggman from Stockton keeps information posted that’s currently taken down after 10 years. The information includes revocation, suspension, probation or surrender of a license, or equivalent action by the state medical board or a board in another state.
AB 1886 was sponsored by the board, which suggested last year that the 10-year limit be removed to make physician records more transparent. Since the website states it posts all publicly available information on a physician or surgeon’s license, there was concern that consumers would be misled into believing — incorrectly — that some doctors had no record of discipline.

Center for Medicare Advocacy Sues to Fix Broken Medicare Appeals System
The Center for Medicare Advocacy filed a nationwide class action lawsuit in United States District Court in Connecticut today (Lessler, et al. v. Burwell, 3:14-CV-1230, D. Conn.). The five named plaintiffs from Connecticut, New York, and Ohio have all waited longer than the statutory 90-day limit for a decision on their Medicare appeals. The current average wait time is over five times the congressionally mandated time limit.
The Medicare Act requires Administrative Law Judges (ALJs) to issue decisions within 90 days after a request for a hearing. Yet, as of July 2014, the current wait time for a decision averaged 489 days. Due to an unjust and time-consuming Medicare appeals process that essentially “rubber stamps” denials at the first two levels of appeal, the ALJ hearing is the third level of appeal, and represents the first chance for a meaningful review of a beneficiary’s appeal – which can include the chance to provide oral and witness testimony.

Start stat vinegar H20 soaks B.I.D. and 2% aqueous gentian violet with some paper tissue inter-digitally. You and your patient will be amazed how fast this condition clears up.
In addition to the oral antibiotics, which were a match for the microorganisms you isolated and cultured, you might wish to have the patient apply topical povidine iodine solution daily to all interspaces. In addition to this, have the patient apply sterile gauze to each toe interspace to prevent friction and to absorb any fluids.

This looks like a classic case of severe bacterial intertrigo. Treatment is predicated upon drying of the maceration with Epsom foot soaks. Soaking in tepid water mixed with antibacterial soap and then drying thoroughly is another alternative. Wearing better ventilated or open footgear as well as regular dusting with medicated talcum powders would help prevent recurrence. Nonetheless, the patient should apply a topical antibiotic daily; perhaps generics would be cheaper given his insurance issues, however, I recommend mupirocin ointment due to the presence of Pseudomonas. Have the patient alternate this with topical gentian violet, again to help dry out the area.
Keep in mind that If the patient has even mild PAD, oral antibiotics may not be reaching this area of the foot in great enough concentration to rid him of the infection. Meanwhile, you may want to keep a close eye on his bilateral Achilles tendons if you plan on prolonged Cipro therapy, especially if he is one of those 40 year old weekend warriors.
My advice is to get an infectious disease consultation ASAP. You've tried conservative measures. You haven't given any medical history on the patient (e.g. diabetic? immunocompromised?), but he needs intravenous therapy.

I agree with the comments about the C-arm. I purchased a flouroscan when I opened my practice in 1994 (used) and am still using the same unit today! I use it about three times per week for surgeries, joint injections, closed manipulations of fractures and subluxated cuboids, etc. It is one of the best purchases I have made!
