GA Podiatrist Discusses How to Choose a Good Children's Shoe
"Just because you spend a lot of money on a shoe doesn't make it a good shoe," says Aaron Solomon, DPM of Village Podiatry. "To find a good shoe there is a simple test. You take the shoe out of the box and see if it bends in the middle. If it does, it's not a good shoe. The foot does not bend in the middle, it bends in the toes," says Dr. Solomon.

"Make sure that there is some extra room at the front of the shoe for two reasons," says Dr. Solomon." Number one: when we stand, the arch is going to flatten and expand a little, and number two: children's feet are actively growing a little; however, you don't want to buy a shoe too big as that creates other problems."

CT Podiatrist Volunteers at Diabetic Patient Screening
The Comprehensive Wound Care Center at Griffin Hospital recently gave free diabetic foot screenings to 15 individuals from the community. Podiatrist Luke Jeffries, DPM, of Yale Podiatry Group volunteered to conduct the screenings along with members of the Comprehensive Wound Care Center. Three individuals were referred to podiatrists for follow-up tests.

Diabetic foot screening can help determine risk factors early and reduce amputations and ulcerations. Studies have shown that early detection and intervention may prevent up to 85% of amputations in diabetic patients.

WI Podiatrist Opens Office in Escanaba
Conway McLean, DPM cut the ribbon at the grand opening of the Superior Foot and Ankle Center Wednesday evening in Escanaba. McLean was joined by his wife Jo and staff - Abby Letts, Garrett Griffin, Patty Ferraro, and Devin Rettke. McLean realized there was a need for a podiatrist in Escanaba.

The Superior Foot and Ankle Center has another office in Marquette. Dr. McLean has practiced since 1991 and has practiced five years in the Upper Peninsula after practicing in Chicago. Members of the Ambassadors of Delta County and Delta County Chamber of Commerce also attended the opening.

Query: Denials for Office Visits and Same Day Procedures
Has anyone had problems billing an office visit and a procedure on the same day with private insurance companies lately? We are not getting paid for both here in Texas and having to do the procedures on a separate day in order to get reimbursed. Does anyone know if our national lobby or APMA are addressing this issue? This is not just podiatry, it’s every specialty practice.

Response: This is becoming an FAQ for anyone dealing with coding and billing these days. Insurers have become much more strict, for lack of another word, about procedures and E&M submissions on the same day. The significant uptick in the use of -25 for separately identifiable E&Ms is a big sign of this.
In every procedure, there is inherently built in a small amount of payment for an E&M to be included with the procedure. What this means is that they expect your exam and management is already paid for when doing a procedure as part of the procedural payment. The -25 modifier is designed to show the payer that the provider is doing a “separately identifiable (E&M) service” at the same time as a procedure for a DIFFERENT problem.
I do not always agree with their policies as they often simply deny the E&M without allowing the doctor to provide documentation to prove it is separately identifiable. Of course, many providers automatically affix a -25 and an E&M code every time they follow up a similar or same condition/problem. So, yes this is being seen across all platforms of medicine. It is paramount to make sure if you are indeed doing a separately identifiable service, you append the -25 to your E&M and document clearly and completely why it is different from the problem you are providing a procedure for.

If a flat-heel sandal (i.e. “zero-drop” shoe) is improving this patient’s heel pain, then it may be worth having the patient purchase a pair of Altra running shoes in order to see how his heel pain responds. Altra shoes are “zero-drop” shoes, meaning that the heel and forefoot sole thicknesses are the same.
Alternatively, the Hoka Bondi shoe, with only a 4 mm heel-drop, but also a 33 mm thickness of soft foam under the heel may also help resolve the patient’s pain. This is certainly a relatively inexpensive and practical method for your patient to...

It has been obvious to me that large pharmaceutical companies are state sanctioned monopolies. I come to this conclusion after realizing what the FDA regulations and our patent laws do to in effect to protect their profits. I think they should be treated as utilities are treated. I know that the practice of medicine and law can also be considered state sanctioned monopolies.
I believe the government has more control over them. I understand when some people try to point out the difference between true capitalism and what we have now. We should start by declaring that lobbying our politicians is obviously a crime.
