PRACTICE MANAGEMENT TIP OF OF THE DAY

10 Strategies for Protecting Patient Data - Part 10

10.  Instill a Culture of Security

Every employee is a guardian of the patient’s data. Although employee negligence and lost/stolen devices continue to be primary causes of data breaches, as Rick Kam, president and co-founder of security firm ID Experts, points out, one of the major findings of a recent Ponemon Institute report is that criminal attacks are now the leading cause of breaches in healthcare. While criminal attacks are often referred to as cyber-attacks, they can also include malicious insider threats, according to Kam.

QUERIES (CLINICAL)
Query: Discolored Toes

Discolored toes

A 19 year old female patient presented with very mild contractures of the toes without any hyperkeratotic skin of the pigmented areas. X-rays have not been taken yet to look at any underlying deformity. I really don't think that any hammertoe procedure will remove the skin other than removal of the lesions. I have never run across this discoloration before. Any other thoughts?

RESPONSES/COMMENTS (CLINICAL) - PART 1
From: Dock Dockery, DPM

In reference to the article on alcohol injections into the rat sciatic nerve, I have a few observations:
1. This study doesn't explain why literally hundreds of thousands of patients have had permanent relief from nerve symptoms that were injected with a 4% ethanol solution.
2. This study explained that the sciatic nerves were "exposed" prior to injections and were not injected into an intact animal.
3. This study only used one injection of each trial agent: 0.5 ml of 0.5% Marcaine alone, left sciatic nerve in 11 rats and 0.5 ml of 4% ethanol with 0.5% Marcaine, right sciatic nerve
in 11 rats, and also 0.5 ml of 20% ethanol with 0.5% Marcaine,...

Editor's note: Dr. Dockery's extended-length letter can be read here.

RESPONSES/COMMENTS (CLINICAL) - PART 2
RE: Are We Still "Kings" of Orthotics? (Doug Richie, DPM)
From: Elliot Udell, DPM

True, the paper by Wrobel, et al., did not show that custom orthotics were the be all and end all in the management of all symptoms caused by plantar fasciitis. Yes, it showed a 125% improvement in spontaneous activity over three months as compared to pre-fabricated orthoses. It did not show improvement in pain. This is why all of us use injections, physical medicine, NSAIDs, etc., in addition to custom orthotics in the management of plantar fasciitis.

What is most significant with Wrobel's paper, however, is that it is a good attempt at bringing evidence-based medicine to one aspect of biomechanics and foot orthoses. Up until now, education in biomechanics has been, by and large, in the domain of self-declared experts. They occupy bully pulpits at conventions where they espouse their own theories of how an orthotic should be made and why it works. The obvious problem is that if you attend five lectures by five different individuals on this topic, you will hear five different sets of conflicting theories and recommendations. This is because the information given by these speakers has not stood up to the scrutiny of solid university-based research. We need more papers like that of Wrobel, et al. Then biomechanics and custom orthoses will graduate from pet theories into actual science.

YOU CAN'T MAKE THESE THINGS UP

RE: Outrageous Shoes of the Day

Perfect for Chinese New Year's Day?

These shoes are inspired by Shisa, a traditional Ryukyuan decoration, often in pairs, resembling a cross between a lion and a dog, from Okinawan mythology. People place pairs of shisa on their rooftops or flanking the gates to their houses. Shisa are wards, believed to protect people from some evils. When in pairs, the left shisa traditionally has a closed mouth, the right one an open mouth. The open mouth wards off evil spirits, and the closed mouth keeps good spirits in.
