Gov Appoints 2 to State Board of Podiatric Medical Examiners
 
Tennessee Gov. Bill Haslam today announced the appointments of 118 Tennesseans to 52 boards and commissions. "I am honored to make these appointments, and I appreciate these men and women who are so willing to serve in this capacity," Haslam said. "Tennessee will be well-represented on these boards and commissions."

Dr. Sarah E. Edgar of Greenwich, a member of the American Podiatric Medical Association and the American College of Foot and Ankle Surgeons, was recently appointed to the Norwalk Hospital medical staff.

Dr. Edgar completed her three-year podiatric medicine and surgery residency at Yale New Haven Hospital where she was named Chief Resident in 2013-2014 and was also recognized as Outstanding Resident for two years. She was awarded a Doctor of Podiatric Medicine from Scholl College of Podiatric Medicine at Rosalind Franklin University of Medicine and Science in Chicago.

Aussie Makes Career Change to Podiatry
 
For 32-year-old Yvette Cunningham the future is in feet. Aware that the writing was on the wall for her science communications job in the rural NSW town of Narrabri, she set about planning and saving for a career overhaul. "Our research facility ran out of funding and I decided that I wanted to have a job that had security and flexibility and allowed me to live in the country, so I did my research and I decided on podiatry," she explains. Initially, she completed some subjects for her three-year degree in podiatry at Newcastle University. Then when the research facility closed, she moved to Sydney, with $15,000 in savings.

As the study ramps up in her final year, she will probably give up her part-time job and move in with her mother before heading back to regional NSW for her placement. It's all worth it, though. "I can't think of anything worse than always having to look for work or not being happy in a job I'm doing for 30 years."

Billing Medicare for Poor Care Not a False Claim, Court Rules
 
A recent appellate court ruling in favor of an Illinois nursing home could shield other healthcare providers from False Claims Act allegations that involve poor quality of care. Last week, the 7th U.S. Circuit Court of Appeals vacated a judgment against Momence (IL) Meadows Nursing Center and in favor of two whistle-blowers, both nurses who used to work at the facility. A District Court ordered Momence to pay more than $9 million under the False Claims Act. The appeals court, however, concluded that the plaintiffs' argument rested on an incorrect application of the “worthless services” theory.
 
Experts say the ruling reaffirms that the False Claims Act is rarely an appropriate tool for policing medical negligence. “It will serve as a standard by which other circuit courts will evaluate similar cases,” said Gary Eiland, a partner in the Houston office of law firm King & Spalding who works on healthcare false claims cases.

Query: Pseudomonas and Group B Strep
 
This 40 year old African American male presented with erythema and macerated web spaces. He was initially treated by a nurse practitioner and prescribed terbinafine. There is serous fluid and malodor to the area. Initially, I wanted to start him on erythromycin gel, but he had to go to oral due to his insurance. I then prescribed ciclopirox gel. 
 
Pseudomonas and Group B Strep Infection
 
A culture taken last week showed  heavy Pseudomonas and group B Strep. I started him on oral Cipro and doxycycline. I have seen him for 3 weeks and nothing seems to be working. Any thoughts?

Query; Help Coding Neuroma Injections 
 
I would appreciate help with how to get paid for injections of more than one neuroma administered on each foot. Currently, most insurance plans are only allowing for payment for one of the two injections that I give. For example, if I place cortisone injections in the left foot 2nd and 3rd intermetatarsal spaces, they are only paying for one injection (CPT 64455), saying that the other injection is included with that payment. This is also happening with sclerosing injections (CPT 64632).

Response: The answer to this question is in the definition of the steroid neuroma injection code(s). It states injection(s) meaning one or more in the same foot. The terminology was a concession that was made when the neuroma injection codes were developed at CPT. Injection of the second foot would be allowed, but will be subject to the multiple procedure discount.

They get you to write a prescription and charge the patient and/or their insurance company several hundred to over a thousand dollars for something that costs pennies. Secondly, what if the insurance company doesn't pay, which more and more are refusing to do.Your patient is responsible for the cost. Lastly, you get paid a few pennies for this company to make thousands. Skip it. If your patient needs one of these medications, find a local compounding pharmacy.
