CA Podiatrist Performs Surgery on Woman's Marathon Record Holder

Marathon world record holder Paula Radcliffe announced on Twitter that she had surgery on her left foot earlier today. "Recovering well from successful surgery in Palo Alto, huge thanks to Dr. Saxena and Palo Alto Med Clinic," Radcliffe wrote. Amol Saxena is a leading sports podiatrist, based in Palo Alto, California.

Radcliffe reported that she had a bone graft done on an 18-year-old fracture and is on crutches. She also posted a photo of the foot, showing a metal frame that, she wrote, keeps the joint apart to hasten recovery.

TX Podiatrist Joins Hill Country Memorial Hospital

Hill Country Memorial Hospital (HCM) in Fredericksburg is welcoming podiatry and wound care specialist Angela L. Schladoer, DPM, CWS, to its medical staff. Dr. Schladoer purchased the podiatry office at 307 S. Washington Street in Fredericksburg, and offers a full range of foot and ankle treatments and sees patients of all ages. In addition to her podiatry practice, she has joined Marshall W. Cunningham, MD in providing wound healing at Hill Country Memorial’s Wound-Healing Clinic.

Dr. Angela Schladoer

“My family and I are excited to back in the Hill Country, and I’m glad to have an opportunity to join the Hill Country Memorial family,” Dr. Schladoer said. “This is a great place to live and practice medicine, and I’m honored to be a member of the hospital medical staff.” Dr. Schladoer has five years of experience as a podiatric and wound specialist. She is coming here from Eagle Pass, TX, where she practiced at United Medical Centers and Hyperbaric Medicine and Wound Care Center/Fort Duncan Regional Medical Center, where she was a co-medical director.

NY Podiatrist Worried That Re-zoning Could Impact Local Businesses

Four different public hearings were rolled into one, concerning amendments to the village code and zoning. Chief among them was the creation of heretofore-prohibited “entrepreneur lofts” above Broadway businesses – one-bedroom apartments with up to two occupants. Monticello podiatrist Dr. Marc Hudes worried that the village isn’t prepared for such a change, especially if apartment residents – rather than customers – park in front of the stores.

Dr. Marc Hudes

“I think that it’s going to be very difficult to police the rules and regulations you’re going to set up,” he cautioned.

HEALTHCARE NEWS

U.S. Physicians Suffer More Burnout Than Other Workers

Physicians in the United States suffer from more burnout than other workers in the United States, new research shows. A national survey of more than 7,000 U.S. physicians reveals that close to one half report having at least one symptom of burnout. "The fact that almost 1 in 2 U.S. physicians has symptoms of burnout implies that the origins of this problem are rooted in the environment and care delivery system rather than in the personal characteristics of a few susceptible individuals.

"Policy makers and health care organizations must address the problem of physician burnout for the sake of physicians and their patients," the authors, led by Tait D. Shanafelt, MD, Mayo Clinic, Rochester, Minnesota, write. The survey findings were published online August 20 in the Archives of Internal Medicine.

PRACTICE MANAGEMENT TIP OF THE DAY

Calm Complainers

Calm complainers by first agreeing with something they say. Example: “You’re right; your wait was too long.” Empathize with their position, apologize and then move forward to resolve their problem.

QUERIES (NON-CLINICAL)

Query: Compensation in a Wound Care Center

I would appreciate comments on compensation structures of DPMs working in wound centers (with or without hyperbarics.), i.e. bill for what you see or ask for an hourly salary (what would be a fair rate? Are there some national guidelines to determine this?). Any idea what a facility gets for a technical component for your services? If there is hyperbarics, is there a portion of that reimbursement that you could get if it is used during your session?

RESPONSES/COMMENTS (CLINICAL)

RE: Recurring Forefoot Pain in a Cyclist (Mark Aldrich, DPM)
From: Robert M. Conenello, DPM

Many times, simply adjusting the cleats to a position proximal to the metatarsal heads can help. You may also suggest that the patient wear a thinner sock as well as evaluating the fit of the bike shoe. Many cycling shoes are very restrictive in the forefoot. The Bont shoe has a wider toe box and may also be an option.

RE: Recurring Forefoot Pain in a Cyclist (Mark Aldrich, DPM)
From: Ira Meyers, DPM

In response to the question about recurring forefoot pain in a cyclist, there are several possibilities. The easiest and often most effective treatment is adjustment of the seat--either forward or back, and/or up and down.

RE: Scope of Practice (Robert Bijack, DPM)
From: Paul Kesselman, DPM

Is Dr. Bijack serious about his opinion on podiatrists not discussing BMI with their patients? Is it his interpretation that podiatrists can't discuss how systemic body functions and illnesses affect the foot and ankle?

Why does he limit his opinion to obesity and not also include smoking, diabetes, hypertension, high cholesterol, and other systemic maladies which certainly DO affect the foot. I dare say the average podiatrist sees the ravages of all of the above on a daily basis.

Most public health policies have the exact opposite opinion from Dr. Bijack. Why else would...

Editor's note: Dr. Kesselman's extended-length letter can be read here.

RE: Microphone for Dragon Med 11 (David Kahan, DPM)
From: Gary A, Lieber, DPM

I have been using Dragon for 6-7 years now using off-the-shelf Bluetooth headsets. I am currently achieving 95% accuracy at a normal rate of speech. The model I am using now is the Blue Parrot Expressway which is available on e-Bay for about $90.

If you use a wired headset, you can get very excellent dictation for even less. Please remember if you use a Bluetooth headset, you must set up a separate profile for it and make sure you configure it as a Bluetooth mic in the setup.

RESPONSES/COMMENTS (NEWS STORIES)

RE: UK Podiatrist Tells Kate Middleton to Fix Her Feet (Sam Bell, DPM)
From: Barry Francis

Dr. Bell raises some most interesting points when commenting on the newspaper article written by a British podiatrist. I have spoken to a number of of my podiatry colleagues about the ethics and advisability of not just the comments made but whether the article should have appeared at all under the signature of a podiatrist. While I acknowledge the professional abilities of the practitioner concerned, I and others deplore not only the format of the 'letter' but the univited comments on health-related issues of a public figure in a particularly detailed manner, not all, by the way, that I would agree with.

A previous correspondent has applauded this action largely on the grounds that it will advance the cause of the profession. On the contrary, I believe that this will be regarded as a cheap publicity stunt by many in the UK, including members of other medical disciplines.
