PODIATRISTS IN THE NEWS - PART 1
Barefoot Walking at Home Increases Intrinsic Muscular Strength: NY Podiatrist

“Barefoot is skin to ground,” explained licensed clinical podiatrist Robert Conenello, DPM. “I’m a big advocate for going barefoot at home,” Dr. Conenello said. “[The practice] increases intrinsic muscular strength within the feet. Anything else is considered to be shod, as even socks alter the mechanics of movement.”

Dr. Robert Conenello

He explained that the primary benefit of walking barefoot is the reinforcement of the muscles in the feet, which tend to weaken “as we age and wear shoes.” These muscles are closely linked to our overall mobility, so their deterioration can contribute to reduced movement as we get older. “Many of the pathologies that I see in my practice are due to the inability to engage these muscles for normal movements and metabolic efficiency,” Dr. Conenello added.

PODIATRISTS IN THE NEWS - PART 2
NY Podiatrist Discusses Compression Socks

Compression socks offer specific amounts of elasticity to provide a supportive feel to the feet, ankles, and legs, explains Bruce Pinker, DPM, a foot and ankle surgeon with Progressive Foot Care in New York. “There are different amounts of compression depending upon the patient's needs,” he says. Levels of compression are measured in millimeters of mercury (mmHg). They start around the range of eight to 15 mmHg, which Dr. Pinker says works for most people. Standard medical-grade compression socks are 20 to 30 mmHg, but Pinker points out that they can go up to about 50 mmHg (which is a lot of compression).

Dr. Bruce Pinker

While the benefits outweigh the cons, Dr. Pinker says you shouldn’t wear them while sleeping, as they can be uncomfortable and even restrict blood flow in bed. Most people can benefit from wearing compression socks, but there are a few who should steer clear. Avoid them if you have an open wound. Pinker adds it’s generally not recommended since it can irritate your raw skin.

PODIATRISTS AND FOOTWEAR
Hands-Free Shoes Ideal for Those with Mobility Issues: TX Podiatrist

When it comes to the best walking shoes and sneakers, there’s nothing that can slow you down more than having to tie—and re-tie—pesky laces. That’s why we love a trusty pair of slip-ons. The best hands-free shoes are specially designed to take on and off with ease—which in addition to helping you save time, also provide a secure fit.

Dr. Anne Sharkey

This makes hands-free, slip-on shoes especially beneficial for seniors and those with mobility issues because of their accessibility features, says Anne Sharkey, DPM, a board-certified podiatrist at North Austin Foot & Ankle Institute. “They’re great for anyone with difficulty with mobility in bending down to tie shoes including those with injuries, back pain, arthritis, pregnancy and more,” she notes. “They are also fantastic for individuals who suffer from hand dexterity challenges as well as arthritis which can limit ability to tie laces.”

PRACTICE MANAGEMENT TIP OF THE DAY
Preventing a Malpractice Claim Before the Patient Arrives

By the time a patient sets foot in your waiting room, many malpractice risks have already been set in motion or avoided. From your online presence to your appointment scheduling process, preventive steps taken before the first visit can help minimize legal exposure and protect your reputation. Here are nine ways to get ahead of potential trouble.

9.  Train front-desk staff on red flags and tone-setting

The first human interaction — whether by phone or email — sets the tone for the patient relationship. Front-desk staff should be trained to identify red flags, such as aggressive behavior or unrealistic demands, and to handle them professionally. Strong front-desk protocols can help you avoid onboarding patients who could pose legal risks.

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Podiatric Groundhog Day
From: Rod Tomczak, DPM, MD, EdD

I came across the book The Evolution of a Profession: The First 75 Years of the American College of Foot and Ankle Surgeons, written by historian Kenneth Durr, PhD with Jerome S. Noll, DPM, EdD. It can be downloaded here as a .PDF.

I suggest that everyone concerned about the growth pains podiatry is experiencing read the salient parts of the 180 page book (first 90 pages) to come to the conclusion that as we grow, we keep getting in our own way, stumbling over our own feet, and continue to be jealous of those that have a little bit more. Except for our growth in podiatric knowledge and scientific/procedural advancements, we are right where we were in the 1950s and '60s. They argued about certification,...

RESPONSES/COMMENTS (PODIATRIC MEDICAL EDUCATION) - PART 1A
from: Douglas Richie, DPM

I fully agree with the observations made by Dr. Kirby regarding the demise of biomechanics in the podiatric profession. The push by leaders in our profession to have podiatric education mirror allopathic medical schools has eliminated the unique advantage that podiatrists previously had in evaluating and treating all foot and ankle pathologies. The APMA Vision 21st Century influenced all of the podiatric medical schools to substantially reduce all formal courses in biomechanics which were replaced by general medicine courses.

In 2022, the Council on Podiatric Medical Education passed CPME 320 which reduced the number of biomechanics cases in podiatric residency by thirty percent. Without adequate biomechanics training, podiatric physicians can only wish to be as good, but not better than our orthopedic colleagues.

RESPONSES/COMMENTS (PODIATRIC MEDICAL EDUCATION) - PART 1B
From: Howard Dananberg, DPM

I’ve been following the thread on how podiatry is losing its expertise in biomechanics. I understand the allure of surgery, but with the pathetic insurance reimbursements, stress, and periodic complications that a surgical practice creates, it’s surprising that designing and dispensing custom foot orthotics doesn’t have greater acceptance. It can be financially rewarding, and patients love them and the practitioners who provide them.

When combined with lower extremity manipulation techniques, many long-term issues can spontaneously resolve as well. And, thinking through biomechanical issues can be a very satisfying academic endeavor and rewarding to both patient and doctor. Think about attending the Richard O. Schuster seminar on October 24 and 26 at NYCPM in New York.  Well worth it.
