PODIATRISTS IN THE NEWS
OH Podiatrist Discusses Plantar Fasciitis

According to Nicole Nicolosi, DPM, "podiatry is a gateway to other health concerns, being proper alignment from the ground up helps out your back, can affect different conditions. If you can't be active, then you can't exercise. You can't maintain your cardiovascular health. So having good foot health, in general, is important for your overall well-being."

Dr. Nicole Nicolosi

"One condition in particular is plantar fasciitis, and this is a condition which can result from overuse, from a tight ligament on the bottom of your foot, which supports your arch. And when your foot arch is not adequately supported and your arch collapses, this ligament is tight, it causes tearing or ripping of this ligament. And this results in inflammation and the pain that results can be debilitating. So it's very important, in general, to wear good arch supports, good foot supports," says Dr. Nicolosi.

PRACTICE MANAGEMENT TIP OF THE DAY
5 Best Practices for Cleaning a Medical Office

The CDC and OSHA have set industry standards for medical office cleaning. These research-based procedures ensure staff and patient safety, lower risks of health care associated infections and reduces cross-contamination. Here are the key areas for medical office staff to focus on when cleaning the clinic.

4. Keep the bathrooms clean

It may seem like common sense to keep the bathrooms clean, but these areas often go unnoticed when the office becomes busy. The state of the bathroom reflects your clinic’s attitude towards cleanliness. Patients who use your bathroom and see that there are paper towels lying around, soap stuck to sinks and other dirty areas may not want to be treated in that office.

Restroom checks need to be done regularly at all medical clinics. Set a schedule for cleaning countertops, disinfecting toilets, sinks, and other fixtures. Also always keep the bathroom stocked with soap, paper towels, and toilet paper.

RESPONSES/COMMENTS (DME)
RE: Another OIG Investigation Worthy of Looking at
From: Paul Kesselman, DPM)

Another OIG investigation is worthy of looking at as to how this may affect your practice. Yesterday, the OIG announced the findings of an investigation against an ophthalmic lens manufacturer that seems eerily familiar to a similar issue podiatrists also face when accepting rebates or gifts from manufacturers of cellular tissue products or other products billed to third-party payers.

There are "safe harbors" to the Anti-Kickback and Stark Violations regulations. When offered any rebate or financial incentive from a manufacturer, the wisest choice is to contact a healthcare attorney and provide them with the language of the incentives/discounts being offered to you. If the offer you receive is not within one of the safe harbors, it is wise to decline the offer. For more on this story, click here.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
RE: DPM Degree Recognized Worldwide as a Needed Participant For Wound Healing Centers
From: Steven Kravitz, DPM

Several years ago, just prior to the Covid outbreak, I was approached by European wound healing specialists to assist them in advancing DPM licensure in Third World countries. The objective was have the DPM trained podiatrist become a stable practice in wound healing centers in these countries. The lack of podiatric services impacted negatively on the quality of care. Since the COVID-19 pandemic started, international travel was curtailed, and the program has been on hold.

The International Compression Club (ICC) annual meeting will be held May 5, 2023 in Milan Italy in conjunction with the European Wound Management Association. Depending on how international travel develops, there may be opportunity for face-to-face meeting with international colleagues. I plan to initiate discussions again with my international colleagues in early September.

I think it is worth mentioning that this initiative clearly demonstrates the degree to which the DPM degree is recognized worldwide as a standard in providing lower extremity care. The ICC are not looking for an MD foot and ankle specialist. They're looking for a DPM degree trained specialist. During my career, the DPM degree has allowed me to stand out from the MD crowd and be more easily recognized to increase referral to my practice as well as other professional opportunities. It has served me well.

Steven Kravitz, DPM, Winston-Salem, NC

RESPONSES/COMMENTS (NON-CLINICAL) - PART 3
From: Larry Lloyd, DPM

If you are a private practice podiatrist who relies on the bulk of your take home pay from fee-for-service Medicare or commercial insurance, your viable days of sustained private practice have just shortened. Start writing your obituary now. According to Becker's ASC Review, CMS seeks a 4.42% physician pay cut in 2023. In my opinion, for decades, and with malice aforethought, CMS has planned the destruction of private practice medicine. This has been to consolidate private practice medicine into larger entities such as hospital employed models, or direct employees of large health insurance entities, such as Optum. Most recently, we see Wall Street private equity groups taking a piece of the action with the likely intent to sell to a larger managing entity, such as Optum.

In turn, Optum will contract directly with CMS for nationwide health delivery. Why? Vertical integration of medical care and economy of...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 4
From: Steven D Epstein, DPM

Name Withheld's comment about obtaining used open vials from the OR while the nurses look the other way is a direct violation of safe injection practices. This CDC document explains that multi-use vials should be dedicated to a single patient, whenever possible.

In many states, such as Pennsylvania, where I practice, all it takes is a single anonymous complaint from a patient, employee, or other person to the department of health that you are doing something wrong and that will bring down a surprise inspection of your office, in which everything in your office is fair game for them to look at. This then can result in immediate license suspension. This is done by the health department's administrative power without due process, and without any specific regulations within the podiatry practice act having been violated. Then you have to appeal to get your license back. Many docs would be surprised at what a bunch of nurses and epidemiologists might consider substandard, such as your flooring material and the kind of products you use for office cleaning. All of these things are covered in a host of CDC and other relevant agency documents.

It would be very easy for Name Withheld to get into trouble. An employee of the hospital who disagrees with another employee's practices could simply lodge that complaint.

YOU CAN'T MAKE THESE THINGS UP

RE: Outrageous Brain Teaser of the Day

How smart is your right foot? This is hysterical. You have to try this. It is absolutely true. I guess there are some things that the brain cannot handle.

1. While sitting in the chair, lift your right foot of the floor and make clockwise circles.

How smart is your right foot?

2. Now, while doing this, draw the number "6" in the air with your right hand. Your foot will change direction.

I told you so! And there is absolutely nothing you can do about it! You and I both know how stupid it is, but before the day is done you are going to try it again, if you have not already done so. - Myrna Bianco, a faithfull Our Town reader.
