Laura J. Pickard, DPM has been named Alumnus of the Year by the Scholl College of Podiatric Medicine. Dr. Pickard is a 1990 graduate of the Dr. William Scholl College of Podiatric Medicine. She completed her podiatric residency program at Cook County Hospital in Chicago, IL. She is Past President of the Illinois Podiatric Medical Association and currently serves on the Board of Trustees of the American Podiatric Medicial Association.

IN Podiatrists Describes Difference Between Tendonitis and Tendinosis
 
Q: What is the difference between tendonitis and tendinosis, and what causes it? Answer: (by Dr. Mark Jones, a podiatrist with the Methodist Physician Group who has offices in Highland and Merrillville): "A tendon is a flexible band of tissue that connects muscle to the bones in your joints. Tendonitis is inflammation of the tendon, while tendinosis is the thickening of the tendon or small tears in the tendon. If it doesn’t heal properly, it becomes thick and irregular and runs the risk of rupturing.

"It can be caused by a variety of factors, including running or other sports, climbing stairs, or walking. It can occur through repetitive movements including typing, gripping a tool, or even cleaning," says Jones.

IFAF Welcomes 3 New Residents to The Swedish Medical Center Residency Program
 
 
The Board of Directors of the International Foot & Ankle Foundation (IFAF) recently welcomed three new 2015 residents to the Swedish Podiatric Surgical Residency 3-year podiatric residency program (PM&S-36) at Swedish Medical Center, Seattle, Washington. This year, there are two new first-year residents: Midori "Mimi" Higashi, DPM, from Western University of Health Sciences, College of Podiatric Medicine, and Michael Gerrity, DPM, from Temple University School of Podiatric Medicine.

Query: Post-Operative Non-Compliance Issues
 
My patient was non-compliant post-operatively and experienced a lot of pain related to edema and tight dressings. She presented to the emergency department instead of calling me, got the usual work-up for deep vein thrombosis, etc. The ED physician placed her on 23 hour observation, and then called me to see her. I saw her in her room in the observation area, explained (for the umpteenth time) to the patient and her daughter about the need for elevation and very limited walking post-operatively. Is there a CPT code/modifier that we can use with the edema or pain diagnosis so that we can be reimbursed for the hour I spent dealing with this situation? The patient has Medicare.

Response: The only available modifier for an E/M service (which is what you performed in the observation area) is the "-24" modifier which is for an E/M service that is performed during a post-op global which is for a reason unrelated to the procedure for which the patient is in the global. 
 
Your documentation would have to support that the E/M was unrelated to the surgery, and it doesn’t sound like that was the case here.

The poster does not say if the lesion is on the foot or leg. The lesion is referred to as a “nodule”, but no clinical exam findings are stated. The lesion may be nodular, but it doesn’t look like it. The fact that it exists in more than one location is a factor, but on sun-damaged skin, this could be a carcinoma. The dark area in the center is difficult to diagnose from a photo if (the) pigment or eschar (is) due to dry seroma/blood/crust. All signs point to doing a biopsy.

RE: Sclerosing Alcohol Injections

I wanted to share this article from Foot and Ankle International for all of you still using low % ethanol injections for treatment of Morton’s neuroma in hopes that you will consider some other treatment or at least just try Marcaine alone. The authors of this study found no difference at all when injecting rat nerves with plain Marcaine or with 4% ethanol solution. Patients do not like having a series of these injections. 
 
This is clear evidence that using anything lower than 30% solution is essentially doing the same as injecting plain Marcaine, but can be more uncomfortable for the patient. If anything, add a little Decadron. I would like to hear from those of you who use this treatment on a regular basis who can refute clear evidence-based medicine.

Dr. Udell appropriately applauds the recently published study published by Wrobel, et al., supported by a grant from the APMA. However, the "positive difference" which Dr. Udell cites which were provided by custom foot orthoses, compared to pre-fabricated and sham orthoses, must be put into proper context.  When examining the results of this study:
 
There was no significant benefit of any type of orthosis, including sham devices over traditional stretching and icing in terms of reducing morning pain. The custom and pre-fabricated orthoses did reduce evening pain compared to the sham device, but there was no superiority of custom devices. There was no benefit of custom or pre-fab foot orthoses compared to sham devices in terms of overall foot function. The custom foot orthoses did produce a significant improvement in spontaneous physical activity, duration of standing, and duration of walking compared to the other devices.
 
The message here is: If improving activity is your primary goal when treating plantar heel pain, then custom devices appear to be superior to pre-fab based upon this study. If relief of pain is your primary treatment goal, custom devices continue to show no advantage over pre-fab devices.

What you are noticing may not be endemic to your own practice. I went for my annual physical last week and mine was the only car in the parking lot, which seemed very unusual for this busy practice. In the same week, I went to my allergist and he was willing to see me, which is unusual since PAs and nurse practitioners run most of that practice.
 
Part of this may be due to hot summer weather. People are lethargic or may be on vacation. The other part may be insurance-related due to very high co-payments and annual deductibles, and people are just not interested in going to doctors unless they have an emergency. I hope it's the former and not the latter reason .
