NJ Podiatrist Completes Lake Placid Ironman
 
Moorestown resident Ana Cafengiu, DPM participated in Ironman Lake Placid, NY on July 20, crossing the finish line in 14 hours and 31 minutes. “I was just glad that I completed it, but when I heard the time, I was doubly proud,” Dr. Cafengiu said. Held in the village of Lake Placid, New York, the triathlon consists of a 2.4-mile swim in Mirror Lake, a 112-mile bike course through the Adirondack Mountains, and a full 26.2-mile marathon past scenic routes.

Cafengiu, a wife and a mother of two, operates her podiatry and sports medicine practice – Cafengiu Podiatry & Sports Medicine – in Moorestown. In 1997 she received her Doctorate of Podiatric Medicine from Temple University School of Podiatric Medicine in Philadelphia. After that she went on to complete a three-year surgical residency at St. Michael’s Medical Center in Newark, New Jersey. Cafengiu was selected for a sports medicine fellowship at Virginia Mason Sports Medicine Center in Seattle, Washington, which she completed in 2001.

VA Podiatrists Highlight Podiatry Military History
 
Patrick Agnew, DPM and Ben Pearl, DPM reviewed the legacy of podiatry in the military. Dr. Agnew was a naval officer and served during desert storm. His roles included Captain, Medical Service Corps, Sea Bee Combat Warfare Officer, and the United States Navy retired reserve. "I would have accepted a military scholarship, but none were available when I entered podiatric medical school. I spoke at length with my classmate Ray Fritz, DPM and accepted a commission in the United States Navy Reserve after graduation. He and I served in the same unit while we were residents. We both went on to careers lasting 20 years.” Here’s a clip from the podcast.

They also discussed footwear in the military. Agnew related that footwear technology lagged by at least one war in terms of new innovation. This is why many squads still wear older combat boots which leave the soldiers more susceptible to injury. Finally, they addressed the various roles podiatry has played in different levels of medical care in the military from stateside hospitals to battlefield medical stations.

Many years ago, when I first came under the tutelage of Dr. Charlie Turchin, one of the first things he taught was to remove all the springs from clippers and nippers. His reasoning was that you get a better precision and feel for the tissue you are cutting through without having to first overcome the resistance of the spring. 
 
I admit that at first it seemed awkward and difficult and required a different grip on the instrument. My 4th finger became the spring. Once mastered, it did give me a better feel for the tissue. I never used a spring on an instrument for the remainder of my 32 years of practice. Try it!

Dr. Ribotsky and the others who have responded to his post have noted a decrease in knowledge and technical skills in foot and lower extremity biomechanics within the podiatry profession. After 40 years of private practice and having trained numerous surgical residents in biomechanics and foot orthosis therapy over a 25-year period from the Kaiser Sacramento residency program, I have also noted that there has been a gradual lessening of biomechanical skills and knowledge in our more recently trained podiatrists. In fact, I have commented on this same fact multiple times here on PM News over the past decade or more.
 
What I find interesting is that during my podiatry school years from 1979 to 1983 at the California College of Podiatric Medicine, many of our professors commonly made comments tending to emphasize the belief that podiatric surgeons were better than orthopedic surgeons simply because of their biomechanical knowledge. In fact, it seemed to be commonly believed among the podiatry profession during the 1980s and 1990s that the one thing that set podiatry apart from our orthopedic surgery colleagues was that podiatrists were more educated on foot and lower extremity biomechanics and, as such, could make better surgical...

Having written on this subject many times here, it seems that despite some efforts by the schools industry and others, not much has changed. The question is not so much who is at fault, but who is responsible for taking charge of this situation and coming up with a remedy. Is it industry, the schools, CPME, residency directors, attendings hiring new practitioners out of residency, or APMA, ACFAS, ABPM, etc.? Or is it a combination of all of the above?
 
To repeat the same old story and a reflection of the issue(s):
 
Karen Langone, Jeffrey Ross, I, and others less than a decade ago presented a three-hour symposium on "Advanced Biomechanics for the 21st Century" at a past APMA meeting. Fewer than 50 attendees out of the thousand at the APMA meeting attended this...

I have two daughters who are physician extenders, but I can’t simply define what a physician extender really is. Ludwig Wittgenstein, an analytical philosopher, said in 1953, “The meaning of a word is its use in the language.” That certainly clears it up.
 
It seems an extender is now able to perform a task or interpret some evidence and act on it in a realm that previously was limited to a physician. A CRNA provides anesthesia for our patients. They act independently except during induction and emergence when the anesthesiologist is required to be present in the operating room according to most state laws. You and I know that is not always the case. State laws limit the number of ...

I am saddened to hear of the death of Dr. Shaw. Like many in our profession, the suddenness of the tragic circumstances of his death have left me in stunned disbelief. It has been over 20 years since I finished my residency in Atlanta. I had the privilege of learning under several attending physicians; including Drs. Shaw, Doug Elleby, and Mark Light who recently shared some of his thoughts on Dr. Shaw's passing a few days ago. It was a demanding year, but an incredible learning experience. What I learned from Dr. Shaw guides me in the OR and the office to this day. 
 
Not only was he a skilled surgeon and knowledgeable teacher, he was a fearless advocate for our profession. As the fight for parity continues to this day, he was one of the first to help to knock down the walls of professional discrimination; fighting these battles in the courts to challenge discriminatory hospital by-laws. His contributions to the education include his time with the American College of Foot and Ankle Surgeons, where he served as ACFAS president in 1992. Beyond medicine and surgery, he was an avid fly fisherman, and he often spoke of his love of fishing and Montana. My condolences to his family, friends, and colleagues. Our profession has lost a great teacher and physician, and I have lost a mentor.
