MPMA Donates 215 Pairs of Shoes to Local School
Starting the school year with a new pair of shoes might seem like a given, but that’s not necessarily the case — especially for teens who live at Wellspring Lutheran Services in Farmington Hills. Yet, thanks to the generosity of the Michigan Podiatric Medical Association (MPMA), a couple of dozen young Wellspring residents will head to their Farmington Public Schools classrooms next month sporting brand-new footwear.

On Friday morning, 215 pairs of shoes were delivered to Wellspring, and Dr. Jodie Sengstock of the MPMA visited the facility to provide proper fitting for the teenagers in its New Beginnings Program. The rest will go to other Wellspring campuses throughout the state of Michigan. This is the third year MPMA has donated shoes to Wellspring, which supports kids in the foster care system as well as those with abusive backgrounds or needing more support transitioning into adulthood.

ID Podiatrist Discusses Heel Pain
Dr. Randal Wraalstad of St. Luke's Clinic says, "Heel pain on the bottom of the foot, without any easily defined trauma, most often is an inflammation of the fascia on the bottom of the arch, known as plantar fasciitis. It begins fairly mild and gets progressively more painful as the inflammation increases and the fascia become tighter. It’s characterized by severe pain in the morning or when standing after sitting or resting for a while. It feels a bit better with stretching or “warming up,“ but can be very achy by evening if you’ve been very active. This condition also can be related to a plantar heel spur, which can be seen on an x-ray."

"If the pain is on the back of the heel, it’s usually related to a posterior heel spur or an inflammation of the insertion of the Achilles tendon on the back of the heel. The inflammation of the second foot can be due to compensating or walking differently on the more painful foot, which can cause overload or strain of the better foot," says Wraalstad.

NC Podiatrist Arrested in Drug Case
An investigation into complaints about physicians’ alleged aggressive prescribing practices has resulted in charges against a Shelby foot doctor, according to Cleveland County authorities. The Sheriff’s Office said Dr. Steven Elliott Skorman was arrested Thursday at his office, Carolina Foot and Ankle. Skorman was charged with 19 counts of prescribing controlled substances without a legal medical purpose; possession of marijuana; possession of drug paraphernalia; and possession of counterfeit CD/DVD. He was jailed under $150,000 bond.

IA Podiatrist Allegedly Obtained Pain Pills from Patients
State officials have suspended the license of a Marshalltown podiatrist who allegedly let patients skip their co-payments for his treatment if they would give him some of the narcotic painkilling pills he prescribed for them. The Iowa Board of Podiatry says Dr. Timothy Harold Smith was treated at Marshalltown Medical and Surgical Center Aug. 11 for the effects of oxycodone, a powerful painkiller best known by the brand name Oxycontin. He had to be resuscitated and treated with Narcan, a medication that helps reverse the effects of narcotics, according to documents the board released this morning. The board said it received a complaint saying that Smith had been receiving pills from patients in exchange for waiving their co-pay obligations.

IA Podiatrist Faces Disciplinary Hearing for Practicing Out of Scope
Dr. James P. Licandro, a podiatrist from Bettendorf, faces a disciplinary hearing with the Iowa Board of Podiatry. Licandro is charged with practicing outside the scope of his profession. As a licensed podiatrist in Iowa, he has been cited for performing a biopsy on a lesion on a patient's upper back. The incident occurred April 30, 2013.

A Gadget Designed to Finally Make Doctors Wash Their Hands Enough
According to the Centers for Disease Control, almost 75,000 patients died last year from healthcare-associated infections (HAIs) in the United States. It’s consistently ranked as one of ten leading causes of death. The culprit is usually unwashed hands. Studies vary, but show that, on average, hospital workers only wash their hands between 10 and 50 percent of the time they enter or exit a patient room. That number, of course, is supposed to be 100 percent.

Biovigil |
This summer, Biovigil rolled out their first product: a sensor-laden electronic badge that uses traffic-light language—red, yellow, and green flashing lights—to hold doctors accountable for hand-hygiene. To use the Biovigil system, hospital workers clip the two-ounce electronic badge onto their pockets. The badge can detect infrared sensors that Biovigil installs in patient rooms, so that every time the doctor or nurse in question walks in or out of a new room, the badge knows. Hospital workers will sanitize their hands, either with Purell or soap and water, and hold one hand near the badge. Chemical sensors in the Biovigil gadget will detect clean hands, and cue a green light. If the doctor delays the process, the badge turns yellow. If they downright ignore it, it glares red.

Query: Onycholysis in a Child
This 6 year old male presented with proximal lysis of b/l thumbs and b/l toes 3-5 for the past 6-9 months according to his parents. His past medical history is unremarkable,with no allergies or meds, or trauma to the nails. He has had eczema at the popliteal areas of both knees and the forearms on an intermittent basis for the past 2 years. The nails are clean, dry and blacked at the lytic areas proximally.

Has anyone seen this condition, especially in a child? What is the diagnosis and treatment? Scrapings and cultures have been taken and I am awaiting results. It looks like onychomycosis. Any suggestions and help would be appreciated.

Query: In-Office Compounding
I have been approached by a company that puts compounding equipment and supplies in your office for a set-up fee, and I would dispense the compounded medications to my patients. They would bill the insurance and I would receive a percentage of the fee collected for the medication. The representative assured me that as long as I gave the patents a choice of where to obtain the compound, I would be safe from Stark Laws. Does anyone have any experience with this type of set-up?

APMA and ACFAS appreciate that CMS is taking steps to resolve the inequity created by its current Physician Payment Sunshine Act (Open Payments) reporting exclusion for indirect payments made to covered recipient physician speakers at certain accredited and certified continuing education events. We strongly disagree with the decision of CMS to omit arbitrarily the accrediting entity for sponsors of continuing education in podiatric medicine, the Council on Podiatric Medical Education (CPME), as an accrediting or certifying entity under the current exclusion.
