PODIATRISTS IN THE NEWS
Fungal Infections Can be Life-Threatening: MI Podiatrist

According to Conway McLean, DPM, "Many fungal infections, once they have developed, never resolve, and stay with the host for the rest of his or her life. If one occurs in the lung, the treatment itself is potentially debilitating, and the success rate low. This likewise is true for toenail infections. If not caught relatively early in this slow but inexorable process, it typically remains there, gradually, slowly worsening, for the remainder of their life."

Dr. Conway McLean

"A healthy immune system prevents the more dire consequences of a tinea infection. But someone taking immune-suppressive drugs (such as an organ transplant recipient) or a person with diabetes is more likely to experience a secondary bacterial infection. And to a diabetic, these can be limb-threatening," says Dr. McLean.

PRACTICE MANAGEMENT TIP OF THE DAY
5 Best Practices for Cleaning a Medical Office

The CDC and OSHA have set industry standards for medical office cleaning. These research-based procedures ensure staff and patient safety, lower risks of healthcare associated infections and reduce cross-contamination. Here are the key areas for medical office staff to focus on when cleaning the clinic.

2. Dust surfaces

When dust accumulates on surfaces, it makes people wonder how clean the rest of the office might be. Dust build-up in medical offices can aggravate respiratory conditions and carry germs that can spread illness. Regular dusting of surfaces throughout the clinic is an easy way to keep the space feeling clean while eliminating dust mites and bacteria from getting into the air.

Stay away from using feather dusters. All these tools do is move the dust around without properly picking it up. Instead, use a dampened cloth, small handheld vacuum, or a microfiber duster. Never wait until dust is visible on surfaces. The longer you wait to clean, the more likely it is to build up bacteria on these surfaces. Patients with allergies are susceptible to dust, aggravating them; so dust at least three times a week to keep your patients safe.

RESPONSES/COMMENTS (DME)
From: Paul Kesselman, DPM

Having done further research on the CMS CMN issue, I would like to make one correction and some follow-up comments:

1) Fee-for-service Medicare does not require CMN on NPWT, nor on wheelchairs.
2) Medicare Advantage Plans and other third-party payers have their own myriad of rules on prior authorization and the need for a "CMN-like" form;
3) The Medicare Fee-for-Service announcement may or may not affect your Medicare Advantage Plans and local third-party payers.
It is best to check with them for follow-up on how the recent Fee-for-Service Medicare announcement on CMN discontinuation will affect your patients’ coverage come Jan 1 2023.
4) Look for a full length article discussing this issue as well as other new DME later this fall in Podiatry Management magazine.

PODIATRISTS AND STUDENT DEBT
From: Michael M. Rosenblatt, DPM

While I agree with Dr. Chaskin that colleges should be disclosing the possibility that podiatry may not be "a perfect substrate" in which to guarantee the ability to repay college loans and become successful, I wonder if this same risk is disclosed to talented musicians who pass the entrance exam to Juilliard? The very recent experience of healthcare workers facing Covid, and even the HIV epidemic suggests that it is impossible to guarantee anything of permanent value.

I was qualified to enter Juilliard by virtue of the fact that I could play all 4 Chopin Ballades, including the 4th, the Grieg Piano Concerto and could reasonably play all 5 Beethoven concertos. But my performances were imperfect. I was "sloppy" in the left hand and used too much pedal to "cover" my technical defects. I suppose I could have worked through those problems at Juilliard. But well enough to become a successful concert artist? No, I don't think so. Juilliard tuition and expenses are...
