PODIATRISTS IN THE NEWS |
Don't Keep Nail Polish on All Summer: OH Podiatrist
If you've just found the perfect nail polish, you're going to want to keep it around. But sometimes, in doing so, it's entirely possible to keep your nail polish on for way longer than is advisable, and to immediately repaint them once it's started to fade or chip. And while you might not think that's a huge problem, it can be murder on your nails. "It's not a good idea to leave nail polish continuously on your toes all summer. They need a break," stresses doctor of podiatric medicine Joy Rowland, DPM (via the Cleveland Clinic). When you do this, the nail polish can start permeating your nails and enter their top layers, and this can dry them out fast, potentially leading to infection.
|
Dr. Joy Rowland |
Instead, you must give your nails a break. After keeping your nail polish on for a few weeks, allow them to be bare and unpolished for a fortnight or so. This will give your nails valuable breathing time to reset themselves and regain strength and moisture.

PODIATRISTS AND FOOTWEAR |
NY Podiatrist Discusses What to Look for in Children's Shoes
Choosing supportive shoes can make a long-term difference in how your child’s foot develops by helping the arch and foot form properly. Suzanne Levine, DPM says, “By restricting motion, you are eliminating the deforming forces that affect the forefoot.” She recommends seeing a podiatrist if your child suffers pain or any of the other complications associated with flat feet like “back pain, knee pain, neuromas, plantar fasciitis, juvenile arthritis, and juvenile bunions.”
|
Dr. Suzanne Levine |
According to Dr. Levine, choosing well-structured shoes that fit properly is critical so that children with flat feet “don’t over-pronate and [cause] problems with their knees, hips, and back.” Things to look for include a supportive yet flexible sole, laces, or adjustable straps for a snug (but not tight) fit. According to Dr. Levine, one thing you definitely want in a shoe is good arch support. She says it’s also important to “pay attention to width, especially in the toe box.”

PRACTICE MANAGEMENT TIP OF THE DAY |
5 Best Practices for Cleaning a Medical Office
The CDC and OSHA have set industry standards for medical office cleaning. These research-based procedures ensure staff and patient safety, lower risks of healthcare associated infections and reduces cross-contamination. Here are the key areas for medical office staff to focus on when cleaning the clinic.
1. Disinfect high-traffic areas.
Furniture, and other surfaces in high traffic areas, like the waiting room, are prone to collecting bacteria, dust mites, and other germs. Staff must keep up with industry standards when it comes to cleaning by wiping these areas down with EPA-registered disinfectants.
In medical offices, the most common high-traffic areas include drawer handles, faucets, cabinets, chairs, and sinks. The same goes for specialty rooms like x-ray areas or suturing rooms. In the reception area, focus the cleaning on phones, computer equipment, clipboards, door handles, and counters.

CODINGLINE CORNER |
Query: Ulceration Treatment in the Global Period
I have a patient with a chronic diabetic ulcer which is now infected and has confirmed osteomyelitis. CPT 28810 was performed at the right hallux and first metatarsal. The ulceration that was present before was not directly incorporated in the surgery and is still there post-operatively. Can we bill for the subsequent treatment of that ulcer during the post-operative period or does it fall under global?
Mary Gail Kwiecinski, DPM, Libertyville, IL
Response: So, the patient had a chronic, diabetic ulcer. Unfortunately, it became infected and osteomyelitis developed. CPT 28810 was billed for the surgery that needed to be performed. CPT 28810 is defined as the following: Amputation, metatarsal, with toe, single. This is a major surgical procedure code with a 90-day post-operative global period. The ulcer is still present. It was not eliminated as a result of the surgery that was performed. The big question is: Can subsequent treatment of the ulcer be billed during the post-operative period created by the amputation? The answer is yes, BUT…It all comes down to whether or not the chronic, diabetic ulcer was directly related to the osteomyelitis that developed, necessitating the amputation that was performed. Based upon the above post, the patient initially had the diabetic ulcer which became infected, and developed osteomyelitis leading to the performance of the amputation. This sure looks directly related to me.
In order to achieve reimbursement for the subsequent treatment of the ulcer, the appropriate CPT/procedure code needs to be billed. In order for the CPT/procedure code to be reimbursed, the correct modifier needs to be appended to the CPT/procedure code. Since the subsequent treatment of the ulcer and CPT 28810 are indeed directly related, the modifier that needs to be applied is the -78 modifier which is defined as the following: UNPLANNED RETURN TO THE OPERATING/PROCEDURE ROOM BY THE SAME PHYSICIAN FOLLOWING INITIAL PROCEDURE FOR A RELATED PROCEDURE DURING THE POSTOPERATIVE PERIOD – It may be necessary to indicate that another procedure was performed during the post-operative period of the initial procedure (unplanned procedure following initial procedure). When this procedure is related to the first and requires the use of an operating or procedure room, it may be reported by adding modifier -78 to the related procedure. For unrelated procedures see modifier -79.

PODIATRISTS AND STUDENT DEBT |
From: Daniel Chaskin, DPM
Podiatric medical colleges, the APMA, and state societies that send representatives to educate potential applicants to our profession have a fiduciary responsibility to give full disclosure of the possibility of uncontrolled student debt, and that the statistics for how much podiatrists earn include some struggling self-employed podiatrists. Furthermore, podiatry may be a better choice for those who have a spouse or family member able and willing to support a podiatry practice until it becomes profitiable. Lastly, they should be told that although currently 100% of graduates were able to secure a residency position, there is no guarantee that upon graduation that they will be able to secure a residency position, which is required for licensure.
Daniel Chaskin, DPM, Flushing, NY
