PRACTICE MANAGEMENT TIP OF THE DAY
Reasons to Rethink Your Patient Communication Strategy - Part 5

Ignoring the communications demands of younger patients is not a winning strategy for any medical practice. Here are some reasons why a unified communications strategy makes sense.

5. Do It Yourself (DIY) includes healthcare, too - Not everyone would be comfortable changing the oil in their car or repairing drywall, but technology has enabled all types of do-it-yourself activities that people are comfortable with. Think self-checkout at the grocery store, online ordering and mobile banking. Healthcare hasn’t escaped this phenomenon, although not all practices have embraced such DIY features for patients as online appointment booking or the ability to text the practices. But practices are ignoring these innovations at their own peril. Two-thirds of millennials and those in Generation X said in a survey that they’d consider changing health providers to ones who offered the ability to book appointments online.

CODINGLINE CORNER
Query: Replacement Orthotics

A patient has been treated with orthotics and has resolved their symptoms (let's say plantar fasciitis). Five or ten years down the road, a new orthotic is recommended because the older device is damaged/top covered deteriorated/lost. Would the original diagnosis be appropriate to use for billing or would this be considered a non-covered item since there is no symptoms to link to a diagnosis? Would this be considered the same as using the original diagnosis for a surgery throughout the post-op interval?

Jeff Leibovitz, DPM, Indianapolis, IN

Response: This would depend on your diagnosis. Plantar fasciitis is more of a symptomatic manifestation of another more over-riding diagnosis. A patient may have an arthritic subtalar or midtarsal joint or tarsal coalition which has been stabilized by a custom foot orthotic and now be asymptomatic. Nevertheless, the symptoms while quiescent with a foot orthotic, the patient’s diagnosis is the same. The narrowed subtalar joint range of motion and pronation could have resulted in an increase in the patient’s pronation and thus their plantar fasciitis.

Thus, my short answer to this interesting question, is if the over-riding issues which caused the plantar fasciitis are still present, then that may be the diagnosis you may need to select from. Remember, however, coverage these days for orthotics is often evasive. Presently, there are much narrower coverage parameters to negotiate with insurance carriers, than when the patient was dispensed their first pair orthotics.

QUERIES (MEDICAL-LEGAL)
Query: Orthotics as Part of Revenue

I recently was approached by an orthopedic group for employment. We have gotten to the contract negotiation stage, but we have gotten hung up on the fact that they will not use orthotic revenue as part of either the base threshold or bonus compensation. Their reasoning is that it is a Stark violation because it is not a personally-provided service. Their legal team informed them that it would fall into the category of DME. They are a large orthopedic group and I would expect many referrals for orthotics and braces.

Does anyone have an opinion on the calculation of orthotics as part of revenue towards a base threshold or even a bonus structure?

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: A Tale of Big Pharma--A Podiatry Perspective
From: Robert Teitelbaum, DPM

To make a very long story short, I have been dispensing celecoxib 200mg from a supply in my drug fridge for a couple of years now. I might give out 4 of them, taken once daily, for four days following a heel pain syndrome injection. I asked four pharmacists a couple of years ago, "What would be your choice for an NSAID?", and they all said Celebrex. They thought that for short-term, the fact that it works in a different cyclo-oxygenase cycle, makes it less irritating to the lining of the GI tract.

A major paper in The New England Journal of Medicine about four years ago looked at the cardiac history five and ten years down the road of taking the drug and concluded...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
RE: The Problem With the American Healthcare System
From: Jill Hagen, DPM

I received a refund request from Humana yesterday. I checked the patient’s account and saw I was overpaid, and in fact, the refund was legit. I then decided to call Humana insurance today to pay the refund.  After 20 minutes on the call, I finally got the representative whom I told I was calling to pay the refund. She asked me these questions: name of the patient, insurance ID number, claim number, and date of service. I answered all these questions. Then she asked, “What is the patient’s date of birth”? I did not have the answer in front of me.

She then said, sorry, I cannot help you if you don’t know the patient’s DOB. I told her that I wanted to speak to her supervisor. She said the supervisor was not available and she was sorry but there was no more she could do for me. I then told her, “if you see your supervisor, tell her a doctor called to pay a refund due, and that you could not process this refund without the patient’s DOB. If this insurance rep worked for me, she would be fired on the spot! This can only happen in the American healthcare system.

RESPONSES/COMMENTS (NEWS STORIES)
From: Steven Kravitz, DPM

Congratulations to Arthur Helfand for his amazing accomplishment of continuing to author medical literature primarily dealing with geriatrics and diabetes in allopathic interdisciplinary publications. His outreach through numerous  (more than 400) publications in  textbooks and medical journals, etc. (particularly in allopathic publications) continues to make great strides for podiatry. He is in the true sense a real "pioneer". In the 1960s, he was unique, as most podiatric authors were writing primarily for podiatric publications. He continues to be a mentor for all podiatric physicians.
