Skin Cancer Can Attack the Feet and Prove Deadly: KY Podiatrist

"Skin cancer is most commonly caused from ultraviolet (UV) ray exposure to the sun or tanning beds. This exposure causes damage to the skin cells and can cause them to grow abnormally causing tumors. The American Academy of Podiatric Sports Medicine (AAPSM) has announced that three recipients of the John Pagliano Golden Foot Award won gold and bronze Olympic medals during the 2016 Olympic Summer Games.

PBCPMA Holds Another Successful Conference
 
The Eleventh Annual Toes in the Sand Conference hosted over 150 physicians and 40 exhibitors at the 5-star Eau Palm Beach Resort & Spa this past weekend in Manalapan, Florida. Organized by the Palm Beach County Podiatric Medical Association (PBCPMA), attendees had the opportunity to earn 10 CME while also taking advantage of the beautiful, oceanfront resort.

Query: Answering Services that Schedule Patients
 
I am semi-retiring in the next few months. My workload will be a satellite office one day per week and two half days at a nursing home. Although I will have part-time help, I will not have full-time staff answering my phone. Are there any good answering service companies that actually schedule patient appointments? We presently schedule appointments on the computer with our practice management software and want to continue that approach.

Query: Lesser MTPJ Implant
 
I performed a total silastic implant on a 3rd metatarsal-phalangeal joint (MTPJ) for an arthritic condition. At the same time, same foot, I performed a partial implant on the first MTPJ. I cannot find a proper code other than an unspecified code for the lesser implant. Does one exist?

Response: There is no code for lesser metatarsal joint implant procedures. Therefore, your options are: 1) CPT 28899 (unlisted foot/toe procedure), assign a fee, and provide operative report for manual review. Request peer review from insurer. You might want to contact the insurer prior to surgery since some consider this type of procedure investigational and won't cover it at all; or 2) CPT 28122-59, ostectomy, lesser metatarsal. Consider the procedure as an arthroplasty of the lesser metatarsal joint. 
 
As far as coding a partial implant of the first metatarsal-phalangeal joint, you would bill CPT 28293 (bunionectomy with implant).

RE: The importance of Biomechanics in Reducing the Need for Narcotic Analgesia

The point of my article in Podiatry Management is that because podiatry started out historically on the "periphery" of medicine, the profession has developed and maintained the ability to treat many conditions without narcotics and dangerous drugs. This has worked out to enormous advantage for our patients. There is no secret that the U.S. is in the midst of a very disturbing epidemic of heroin and multi-pain drug addiction, to a great extent CAUSED by medical doctors who over-prescribe dangerous drugs. Now, with Obamacare, FM physicians are over-booked with both patients and documenting obligations. This has led many of them to simply write pain prescriptions for conditions like migraine, headache, digestive problems, heel pain, "metatarsalgia" and other functional complaints which would otherwise take much longer than an office call to work up, assuming they have the ability to do so.  
 
Just recently, Bret Ribotsky highlighted an excellent interview with Stuart Goldman, DPM, who is on the forefront of treatment of many lower extremity conditions without powerful, dangerous drugs. Dr. Goldman is, in a sense, "honoring our historical traditions" by careful evaluation using functional analysis and biomechanics. I find the timing for Dr. Goldman's book perfect. When patients go to DPMs, they are less likely to have dangerous drugs prescribed for them because DPMs have skills that medical doctors do not have.

I have had near 100% compliance with my patients, which is far better than the compliance with other suggestions I have made in the past.

Kudos to Pam Thompson for advocating a "no fear" attitude of podiatrists who are strategically planning their practice futures. If a podiatrist decides to to nothing about MACRA and MIPS, it saves a tremendous amount of time and money that would have been spent for the personnel and other time-intense factors needed to qualify for the highest percentage of long-term reimbursement gain. In my humble opinion, put that money into effective practice marketing and management, and the revenue gain from the flourishing of your practice will far exceed the lost MACRA-MIPS revenue.
