Western University of Health Sciences’ College of Podiatric Medicine held its white coat ceremony for 31 first-year students Aug. 8, 2025 in Pomona, California. CPM faculty cloaked students in white coats for the first time to welcome them into the profession and to remind them of their responsibilities as healers. California Podiatric Medical Association President Arman Kirakosian, DPM told students when they don their white coat, they are making a promise to their patients, profession, and themselves.

MA Podiatrist Re-appointed to Board of Registration in Podiatry

Raymond S. Murano, DPM of Winchester, MA was recently re-appointed to the Commonwealth of Massachusetts Board of Registration in Podiatry. Dr. Murano has been on the Board since his initial appointment in 1991 by Governor Cellucci. Dr. Murano is currently the Chief of Podiatric Surgery for the Tufts Medicine Melrose Wakefield Hospital.

Dr. Murano has been instrumental in getting rulings to pass to allow podiatrists in Massachusetts to get ankle privileges. He is a board certified foot and ankle surgeon and is in private practice in Medford, MA.

Here are nine ways to get ahead of potential trouble.

HIPAA applies before a patient walks through the door. Avoid leaving detailed voicemails with sensitive information, and confirm that your online scheduling platform meets privacy requirements. Protecting patient data early is both a compliance and trust-building measure.

In my office, I keep a small box filled with used, worn out or broken instrument spare parts, and screws from broken tissue nippers and nail trimmers with broken springs. Often, when a spring breaks on a tissue nipper or a toenail clipper, I can easily replace the broken part quickly and simply with a similar part that I saved, and am able to locate in the box. Otherwise, the broken instrument goes into the box, or into the trash, and I will purchase new ones. 
 
However, if you have broken two out of four new nail nippers, either the quality of the nipper is poor (Pakistani stainless steel vs. German stainless steel) or you are using too small of a clipper on too thick a toenail, or you may be heavy-handed or rushing, or you might just need a larger nail clipper, or you  may need to "adjust" or "modify" your toenail clipping technique.

While it may be more reasonable to change the springs in-house, it can be a daunting and dangerous task. Through my 50 years of providing quality service to podiatrists, I have probably changed and installed well over a million nail nippers springs. Things can happen. Screws can break, screwdrivers can cut your hand, especially a screwdriver that is thin enough to properly loosen and tighten the very small screws.

Nail cutters are usually sharp; they can also cut you. I’ve been wounded by screwdrivers many times. This one being the worse case. We have thousands of springs and nipper screws in stock and we can also make custom size springs for any instrument, if needed.

Our expertise in biomechanics enables us to perform more effective surgeries to relieve pain and improve foot function. It is that same expertise that allows us to create orthotics to further enhance the function of the foot and ankle.
 
While some of you scan feet, just like chiropractors and physical therapists do, I still use plaster because they can't master the technique. And, no, my assistant does not do it!

Considering the contemporary meanings of "tweak," I sincerely hope we don't tweak anymore. But this is an example of how out of touch we can become. They have a new vocabulary and set of social norms.

Sadly, I agree with the previous authors. I used to tell patients that the reason they had better results with foot surgery than their friends who went to an orthopod was not because podiatrists were better surgeons but that podiatrists did not just discharge them after surgery. We made them orthotics, recommended appropriate footgear, and followed up until they were sure their problems were resolved.
 
That is no longer the case with many (most) podiatrists. Some will send them to me or another “old timer” for follow-up care, but many discharge them ASAP just like the orthopods. We might as well become MDs or DOs. We are no longer better at total foot care than they are.

I enjoyed reading Dr. Jon Hultman’s article in the August 2025 Podiatry Management. I couldn’t agree with him more. Podiatry has missed the boat or has been late for the boat historically. Let’s talk about radiology. No podiatrist would think of practicing without x-ray facilities in their office now – but it wasn’t always the case. We’re all familiar with Dale Austin via his bunionectomy (technically the Austin-Leviton bunionectomy - or Chevron, if you’re an orthopedist). Dale Austin was a podiatrist in the Los Angeles area in the 1950s and decided to do something really radical. He was the first podiatrist in the state to have an x-ray machine. 
 
You’d think the other podiatrists (then chiropodists) in the area would applaud him – but just the opposite. They were appalled and angry. They were afraid that the orthopedists who sent palliative care to them would stop referring. Dale Austin became frustrated by this response and decided to go back and become a DO. Then, in 1961, via legislation, the DOs agreed to merge with the MDs in California, so he became an MD, though he still practiced primarily foot care. Too bad for podiatry.
 
Dr. Hultman is right about having a form of certified podiatric hygienist in the office. There’s a huge number of palliative patients who will need surgical services at some point and it would obviously be optimal if these patients could be cared for in a podiatry practice rather than somewhere else. And, as Dr. Hultman pointed out, it pays. Let's not miss the boat again.

RE: The Passing of Alan Shaw, DPM

I have been having a difficult time processing the accidental death of Alan Shaw who was a very special long-time friend and colleague. I had known Alan since he began podiatry school in San Francisco, and then he followed me in our residency at Highland General Hospital in Oakland, California. Then through the years, we served on many APMA committes together. About 30 years ago, Alan introduced me to the art of flyfishing which I will cherish forever. During those 30 years, I would travel to the Bighorn River on the Crow Indian Reservation in Montana every year for 5 days of flyfishing.

Some years, I would make several trips to fish with him. Whenever I fished a California river, I made the guide understand that we would be flyfishing, not bait fishing. He was devoted to all causes related to preservation of our rivers and helping with the lives of the Crow Indians. He will be greatly missed but I know that he was the happiest when fishing for the large rainbows on the Bighorn.
