Compression Socks are an Under-utilized Option: OH Podiatrist
Compression socks (or compression stockings) are socks of various length and tightness that are designed to gently squeeze legs a bit more than typical socks. “The key intention is to promote better blood circulation in the legs,” explains Georgeanne Botek, DPM. “It’s a very practical and common thing. But at the same time, it’s a very under-utilized option.”

Compression socks work most notably to help with venous problems of the legs. “Nearly 90% of leg disorders originate within the veins,” says Dr. Botek. Besides venous insufficiency, Dr. Botek says that another common reason for wearing compression socks is to aid in blood flow when you’re sitting for long periods of time, like on a long flight. With less movement and weaker circulation, there’s more pooling and retention of blood in the legs, which can raise chances of clots.

CA Podiatrist Delivers Grand Rounds at RUHS
Dr. David G. Armstrong, Professor of Surgery at the University of Southern California, co-founder of the Southwestern Academic Limb Salvage Alliance (SALSA) and founding president of ALPS, delivered a grand rounds lecture to the Department of Surgery at Riverside University Health System Medical Center (RUHS). This marks the first time a podiatric surgeon has been invited to give grand rounds to the group, signifying a continued step forward in interdisciplinary collaboration.

Dr. Armstrong’s lecture focused on the critical importance of developing interdisciplinary programs in the USA and worldwide. Drawing from his extensive expertise, he provided insights into the “toe and flow” model, an innovative podiatric and vascular surgery approach that he developed. This model has guided the design of RUHS’s cutting-edge limb preservation program.

5 Things to Consider When Selling Your Practice
3. If you own the practice, prepare for two sales.
Do you own your practice building or have ownership of your office space in an office condo? If so, you now have two sales to make. Here are your options:
• Sell the practice and lease the space. This overcomes resistance to ownership at this time. It also means that you are now the landlord. Following the sale, you could have your property in the hands of a property management company, who would collect rents and maintain the facility.
• Sell the practice and lease the space, with an option to buy at a future date (say five years). If the physician does not exercise the option, you can continue as a landlord or consider finding a buyer interested in investing in rentable space.
• Sell the practice and sell the space. If the practice buyer is not interested in ownership of the facility, consider finding a buyer. This makes sense if you are not planning to remain in the community.

Many years ago, I attended an EMR discussion in Dallas. This was during the time the government was offering incentives for EMR. The speaker was an attorney. At the end of his talk, he posed the question, “How much money do you think EMR (EHR) will cost you over the next few years?” Several of the audience posited anywhere from 10K-25K. My guess was over 100K and climbing. The attorney scoffed at this.
Turned out I was right. Hardware and software maintenance and continuing software updates are very expensive. However, in today’s medicine, EMR (EHR) is necessary to do business and effective record-keeping. There are many reasons for this, ranging from submission of electronic claims, effective communication with other healthcare professionals, compliance with the law, electronic Rx submission, and, of course, accurate medical records available at the push of a button.

I am a 1979 NYCPM alumnus, eight years out from practice, and remain a regular reader of PM News. How disappointing it remains to read the same complaints about unfair/limited/denial of payments to duly licensed well-trained professionals, that existed during my 40 years as a student and in practice. The insurers use the same language and excuses they have for decades in denying legitimate claims and/or reducing procedure payments when E/M codes are billed simultaneously, though for an unrelated diagnosis. I see that essentially nothing has changed since I left practice in 2015, which reinforces my decision to leave as the best one for me. The insurers have won the game, set, and match, and there’s nothing any of us can do about it. When the powers that be decided NOT to change the degree from DPM to MD, with all the attendant rigamarole that that would have entailed, podiatry’s fate was sealed.
For those who remain in practice, I can only wish you well, but the cards are stacked against you. I paraphrase Dr. Kass when I state the obvious, there are only so many hours in your day, so many patients you can see and safely treat, and still retain your health, sanity, and family life.

Every one of the points presented by Dr. Kass is legitimate, concerning, and continues to degrade our profession. The solution would be a class action lawsuit and would portend a leveling of the playing field if we had about 10 million to spend on a retainer and associated billable hours to fight this clearly illegal discrimination. Although a few people have offered a solution in operating as a concierge practice, the fear of the unknown and the questionable economic strata makes that a difficult choice for most.
The salient question—as I see it—is “how do they get away with this?” The answer is “because they can and we let them.” The fix is in legally, as any organized pushback on this by our organized profession (en masse) refusing to take insurance anymore could lead to charges under the provisions of the Sherman-Taft Antitrust statute from 1890. We envy the dental profession, in that they were able to organize and largely avoid the strictures of the insurance companies, but as long as a majority of our providers take falling reimbursement in an age of rising costs, they will continue to stick it to us with impunity. It reminds me of the Pogo cartoon, wherein the character states, “We have met the enemy, and he is us.”
