PA Podiatrist Discusses Benefits of Compression Socks
 
If you’ve ever been on a long flight or worked a job where you’re constantly on your feet, your legs may have felt tired, achy, and swollen afterwards. Fluid begins to build up in your legs after sitting or standing for a while — when you don’t get up and walk around every 60 to 90 minutes, it can create an uncomfortable feeling, said Khurram Khan, DPM, a podiatrist and professor at Temple University's Foot and Ankle Institute. Compression socks, which work to promote better circulation by gently applying pressure to your legs, feet, and ankles, may be able to help.

Compression socks can also help prevent some spider and varicose vein formation, if worn over a longer period of time, experts told us. Many people find them helpful during pregnancy, too. Keep in mind that compression socks should not be worn overnight, Dr. Khan said. They’re designed to be worn during the day because when you lay down at night, swelling in your legs, ankles, and feet naturally decreases.

Interruptions are a chief impediment for doctors and medical office staff when it comes to getting things done. Here's how to deal with them.
 
4. Isolate a period of time when you won't hear notifications.
 
For some part of each day, you need to have a 30-minute period, if not 60 minutes to 90 minutes, when you cannot be reached by external correspondents. In other words, you are where you are. You are in the moment. You are present with those around you. Those around you do not need to compete with those who might otherwise be in touch with you via electronic communications. This simple measure will make a notable difference in your family life, with friends, and even among professional colleagues.

Query: New York Health Care Worker Bonus Program
 
An employee just brought to our attention this program in NY State to reward our loyal employees by paying them to stay at work, the "Health Care Worker Bonus Program." This applies to providers that take Medicaid or Medicaid managed care. The problem is that no one I spoke to so far has heard of it. What is involved? What is the paperwork burden to us?
 
I’m wondering if anyone has some insight or has submitted for the first vesting period, since the due date for it is September 2, 2022.

QUERIES (DME)
Query: Apos Shoe Therapy
 
Does anyone have any information on Apos shoe therapy? I found a post from Dr. Kesselman from 2014 but nothing mentioned in PM News recently. Some primary care providers in my practice have received a number of requests from physical therapists asking for approval for the therapy to treat knee, hip, and back pain. Montefiore Medical Center in the Bronx, NY has a page on their website dedicated to it. My concern is the ability of the physical therapist to properly evaluate the biomechanics involved in the diagnosis and proper use of the devices as well as all of Dr. Kesselman's concerns 8 year ago concerning crossover with orthotic treatment. Apparently, now it is covered by some insurance plans.

Recently there has been a flurry of messages from DME MAC and CMS contractors regarding the discontinuation of Certificates of Medical Necessity (CMN) for dates of service effective January 1, 2023. While I will provide a lengthy description of this issue in an upcoming article in Podiatry Management, I felt it necessary to disseminate this clarification statement.
 
Certificates of Medical Necessity (CMN) are formal specialized documents which are provided by Medicare for specific categories of DMEPOS. These include devices typically prescribed and ordered but not dispensed by podiatrists, including but not limited to...

Until a few years ago, my local hospital always left open bottles of anesthetics in rooms for local injections. Then the hospitals started throwing them away and I received unopened bottles for each case. I thought this was a waste of money and likely some reason to reduce medical liability. I never found out. I often had a hard time getting Carbocaine in the past, even from my suppliers, though not other anesthetics. However, today I can't seem to get any from suppliers.
 
So, my solution is to request the bottles from the hospital when doing my few cases each month. So I get a bottle of Carbocaine and Marcaine. The staff turns their back when I take these open bottles, so they are not insubordinate to their employer. I feel perfectly comfortable using the opened bottles as I always have in my office when I purchased them from suppliers and seemed to be acceptable until a few years ago. I have rarely bought anesthetic over the years due to this surplus. Unfortunately, this won't help those who don't do some cases at the hospital.

I think you need a new billing company. My long answer is: if you are PAR with the insurance companies in question, such as Medicare, you absolutely cannot bill patients for what is considered over U&C. To do so would be to violate your contract with insurances. Just hazarding a guess here, but I bet you pay your billing company based on a percentage of what they collect from insurance/patients (Not the way to do it, you should be on a flat fee rate) .
 
If so, it is in THEIR best interest to collect as much as possible, so that you have to pay them as much as possible. If I am understanding the situation correctly, they are completely wrong in what they are telling you, and taking you for a financial ride, to the detriment of your patients/practice.
