PODIATRISTS IN THE NEWS
Embarrassing Foot Conditions Also Carry Dangers: CA Podiatrist

Dr. Ami Sheth sees a lot of bashful patients. They have warts or nail fungus, calluses or athlete's foot. They're not only embarrassed because the conditions make them want to hide in sneakers during sandal season, but they cower in Sheth's office because many think, "it's petty" to seek help, says Sheth, a podiatrist in Los Gatos, California, and spokeswoman for the American Podiatric Medical Association. "But it's not petty," she says.

Dr. Amy Sheth

While some foot problems are admittedly more cosmetic than medical, others have real consequences. Take calluses, for instance, which can seem like harmless annoyances. But if they're not treated, they can change the quality of the skin, damaging its elasticity. "If the skin can't stretch, it's going to tear, and if it's going to tear, it's going to crack, and if it cracks, it can get infected," Sheth says.

CODINGLINE CORNER
Query: CPT 64450 Denial - Not Medically Necessary

How do we get CPT 64450 paid for when treating neuritis or neuralgia? ICD-9 729.2 was rejected.

Jeffrey Klein, DPM, Waterford, MI

Response: In my experience, and according to my state LCD, CPT 64450 is only payable and medically necessary when billed with ICD-9 355.6 (Mortons neuroma).

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: Bill Beaton, DPM, Ken Hatch, DPM

I am in total agreement with Dennis Shavelson. Considering the education and training that podiatrists have, no one else should be diagnosing and treating the human foot with any type of corrective device, unless provided with a prescription by a physician who has completed a thorough lower extremity examination.

Bill Beaton, DPM, Saint Petersburg, FL

The actual article is LOADED with misinformation. It is Canadian/UK oriented but is STILL damaging to our profession.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B
From: Stephen Peslar, BSc, DCh

In the commentary written by Dr. Shavelson, the article that he links to is disturbing for three reasons.

First, the author mentions that a CPed or CO spends hundreds (?) of hours studying foot anatomy, biomechanics and related pathologies, but the author does not mention that current board certified DPMs do an undergrad bachelor's degree, 4 years of study at a podiatry school, and then a 3-year residency.

The second reason is that podiatry students did manufacture their own orthoses for patients back in 1983. We used Rhoadur back then, and unfortunately it cracked quite often. When I took a course at CCPM back in 1987, a professor there said his preferred choice was Rhoadur because of the level of control he was able to obtain from it. I assume podiatry students today still...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
From: George Jacobson, DPM

Check out Spark Pay, Square, and Pay Pal. They all have devices that plug into your phone or tablet and can be less than 2%, depending on the card that you accept or whether you key in the number instead of swiping. There is no vender equipment to buy. I first learned about Square here on PM News.

After searching Internet and reading customer reviews, I chose to use Spark Pay as they had the best reviews for customer support. I have not been disappointed and after a six month trial, have opted to pay $9.95 per month to lower my percentage fee per charge to under 2%. I use an old ipad and patients sign the screen with a stylus. Printing a receipt is a little harder, but it is well worth the savings. You can e-mail the receipt to the patient or e-mail it to yourself and print it. I made a new e-mail account for this sole purpose.

YOU CAN'T MAKE THESE THINGS UP

RE: Outrageous Feet Memorabilia of the Day

If you haven't heard of the Happy Foot/Sad Foot sign... well, welcome to L.A. The iconic advertisement—portraying a bummed-out, broken foot on one side and a super-psyched, sneaker-bedecked foot on the other—has been spinning circles in front of a podiatrist's office on Sunset Boulevard in Silver Lake for 35 years.

Happy/Sad Feet Pins

Some locals use it as an oracle: See the happy foot first, you're going to have a good day. See the sad foot, it may be better to just stay home. But now we can take destiny into our own hands, to choose a foot based on how we already feel and to wear it proudly.
