NY Podiatrist Discusses Zero Drop Shoes
"Zero Drop" may sound like a lame carnival ride, weight maintenance program, East Village bar, or Korean pop band. But they're actually a kind of shoe. And people are into them. Turns out, it's for good reason; according to trainers and even podiatrists, wearing zero drop shoes is something we should all be doing.

Zero Drop Shoes are the exact opposite of stilettos. "When we talk about a zero-drop shoe, we're talking about the angle between your heel and your toe when it's in a shoe," explains podiatrist Emily Splichal, DPM. Most shoes—heels, sneakers, work shoes, etc.—place your heel higher than your toes, and therefore have some drop. Zero-drop shoes position your toes and heel so that they're level, she explains. This puts your foot in the same position as if you were pawing around with bare feet.

Getting quality footwear that fits does matter, and having a child fit by someone knowledgeable is ideal, said Cary Zinkin, DPM, a Florida-based podiatric physician and spokesperson for the American Podiatric Medical Association. Shoes that fit poorly or lack support can be a recipe for blisters, ingrown toenails, or sore feet.

Zinkin recommends parents take kids to a store rather than buying online, even if it’s not the kind where an employee will measure a child’s feet. It’s easier for kids to figure out what’s most comfortable when they can compare sizes and styles side by side.

Reasons to Rethink Your Patient Communication Strategy - Part 3
Ignoring the communications demands of younger patients is not a winning strategy for any medical practice. Here are some reasons why a unified communications strategy makes sense.
3. Growing acceptance of text-based communications - Cell phones enjoy nearly universal use among Americans, with a 95 percent ownership rate. Three-quarters of those phones are smartphones that support simple texts as well as multimedia messages. Although texting remains a small part of healthcare communications, younger Americans prefer texting to email or phone calls. Texts are opened 99 percent of the time, three times the open rate of emails. Most texts are opened within three minutes of receipt. A multiplatform telehealth communications strategy that supports text messaging and chat/IM can create practice workflow efficiencies.

Query: Current CPT for Dehydrated Alcohol Injections
Can someone give me the current CPT code for injections of dehydrated alcohol for neuromas?

Response: This question requires a discussion about proper billing first and understanding the terminology.
#1) One must use dehydrated alcohol at a concentration minimum of 30% and it can be up to 50% in 0.5% bupivicaine; these are the concentrations to be destructive. Please note that the 4% solution that has been suggested for many years in 0.5% bupivicaine is not considered a destructive concentration but would be considered a therapeutic injection.
#2) CPT Codes
a) CPT 64632-Destruction by neurolytic agent; plantar common digital nerve
b) CPT 64455-Injection(s), anesthetic agent and/or steroid, plantar common digital nerve(s) (eg., Morton’s neuroma)
#3) Anatomically, where are you injecting this dehydrated alcohol destructive injection (Concentration 30-50%)? If it is the plantar common digital nerve, then the proper CPT is CPT 64632. If you are injecting an “Other Peripheral Nerve or Branch”, then it is CPT 64640-Destruction by neurolytic agent; other peripheral nerve or branch.

To pay podiatric residents to do rotations in a department that involves such podiatric residents participating in the care of patients who have Medicare would likely run afoul of Stark Laws.
The referral of Medicare patients to have care given by a podiatric resident "for a fee to a department head" is particularly problematic if that podiatric resident is not a bonafide employee of that department head. Usually that podiatric resident is a bonafide employee of the hospital. The hospital compliance officer should be contacted and asked to obtain a legal advisory opinion before any monies are paid.
