E-HEALTH NEWS
Interstate Licensing Bill for Telemedicine Gathers Support

Twenty-one organizations have signed onto the Telemedicine for Medicare (TELE-MED) Act of 2015, which was recently introduced in both the U.S. House of Representatives and the U.S. Senate. The bill, which was previously proposed in 2013, would enable physicians licensed in one state to care remotely for Medicare patients in other states without getting additional medical licenses in those states. It would not, however, affect the severe restrictions that Medicare now places on telemedicine coverage.

Among the organizations that expressed support for the measure in letters to its bipartisan House and Senate co-sponsors are the American Telemedicine Association (ATA) and an assortment of employer groups, technology companies, patient support groups, and giant health insurer Anthem. In addition, the American Health Information Management Association, representing health Internet technology professionals, and the College of Health Information Management Executives, which consists mainly of chief information officers, support the legislation.

PRACTICE MANAGEMENT TIP OF OF THE DAY

10 Strategies for Protecting Patient Data - Part 5

5. Prioritize Third-Party Vendor Management

Practices will need help with third-party vendor management to strengthen oversight and review processes. Smaller business associates are particularly vulnerable since they may not have as many resources to devote to security and compliance, and may be more likely to experience a data breach.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: Are We Still "Kings" of Orthotics?
From: Dennis Shavelson, DPM

I came across a website that posts this article updated on Dec 2014 entitled: "Orthotics Review". The article is biased and contains many opined and inaccurate statements. It does, however, reflect some truth regarding the current state of podiatric biomechanics and orthotics that our profession has chosen for the future. It also suggests new “kings” atop the orthotic pyramid, the pedorthists and orthotists.

I would prefer if we remained the kings. What about you?

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
RE: Codingline Price Increase
From: Jeffrey Kass, DPM, Harry Goldsmith, DPM

Last week, I received an email from APMA informing me that their APMA Codingline membership on the APMA site was going up in price, I think from $189 to $329. The email was informing me if I signed up before a certain date the price would remain. I am not sure how other APMA members felt, but I was bothered by this. I emailed APMA of my dismay but after a week, there is still no response from anyone.

I get that there might be expenses in creating this site, but what I also get is that in the twenty years that I have been in practice, reimbursements get lowered and are harder to get. I opine that APMA picked the WORST possible time to ask for a price increase for this service. I am not dispelling the virtues of APMA membership. EVERY podiatrist SHOULD be a member, but membership should also have perks. When APMA succeeds in having CPT reimbursement go higher, or stopping these Medicare penalties because one doesn't input a patient's race or ethnicity, then I feel they should have a right to ask their members for more money. Until, then, I think you're failing your membership.

Jeffrey Kass, DPM, Forest Hills, NY

Response: There are several points in Dr. Kass' post that need clarifying. First, there is no “APMA Codingline membership.” Codingline is a separate entity from APMA, and currently offers APMA membership complimentary subscriptions to CodinglineSILVER.

Second, the pricing for the APMA Coding Resource Center (CRC) did not jump from $189 to $329. The $189 price increased in 2013 to $249. Some early subscribers to the CRC were able in the past to renew their subscriptions at $189.

Third, subscriptions to the CRC are benefits to APMA members in their...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 3A
From: Steven Selby Blanken, DPM

With all due respect to Dr. Lang's comments, one must understand that when Dr. Lang retired, patient's did not have huge co-pays and/or deductibles. It is very common now that patients may pay a majority of their healthcare visits and/or surgical or DME. My credit card income has gone way up and my personal check and/or cash income has gone way down. We must now train our staffs to check patient benefits prior to the visits. We thus train our patients to come to the practice expecting them to pay all amounts due at the time of each service. Without that mentality, we will not be able to pay our bills. I commend everyone who tries to reduce their merchant credit charge fees.

I feel that the 4-5 percent in fees we have to pay for this service is too high and can be negotiated to at least half of that. Please also note that the merchant fees increase with credit cards that have bonuses involved with those companies. We start out every year with a goal to do better (increase in income and healthcare delivery). The best businesses that thrive in this economy are the ones that cut their expenses lower and lower, thus potentially increase gross and net income. Now only if we can cut our sequestering penalties or MU decreases... That would be nice!

RESPONSES/COMMENTS (NON-CLINICAL) - PART 3B
From: Gian Steinhauser, DPM

If you are not happy with the high % rate you are paying for the credit card processing, you can switch companies. Two years ago, we switched to MerchantWarehouse (now Cayan) and now pay the following: Merchant Warehouse (Now Cayan): Debit cards: 0.39% Credit cards: 1.69% Monthly fee: $7.95 Monthly minimum: $0 Annual fee: $0 Set-up fee: $0 Per transaction fee: $0.21 Other Misc fees: unknown -No cancellation fee - No application fee - No long-term contract - No annual fee - No reprogramming or changes to account fee - No set-up fee - No undisclosed fees mean no surprises.

We are very happy with their service and we’ve saved some money. The rates will vary, based on your previous historical volume, and of course, there’s some detailed fine print to consider.
