TX Podiatrist Discusses Hyponatremia
Hyponatremia occurs when a person’s blood sodium levels drop below 135 millimoles per liter of blood. If someone drinks to excess, they effectively dilute their blood sodium levels. At the same time, water starts to leak into their body’s tissues, causing them to swell, explains Tamara Hew-Butler, DPM, PhD, a Texas-based podiatrist who specializes in treating and studying runners. In severe cases, the brain can start to swell, leading to some of hyponatremia’s core symptoms: confusion, dizziness, the inability to speak properly, and headaches.

During exercise, the descent into hyponatremia can be exacerbated in particular ways, Hew-Butler said, not least by sweating out salts. But there is a hormonal aspect too: Vasopressin, an antidiuretic hormone, ramps up during exercise, and especially when it’s hot. “Your body’s like, ‘Oh my God, I’m losing water. I’m not going to pee it out,’” Dr. Hew-Butler said. By suppressing the call of nature while you’re losing fluid through sweat, the body is trying to maintain that delicate water-to-salt ratio and keep everything working the way it should, she explained. So if you start chugging sports drinks or water midway through a run, then you can quickly swell your cells. “That’s why sometimes people get in more trouble during exercise,” Hew-Butler said. “They preload and they keep loading.”

KS Podiatrist to Receive KWU Alumni Award
Kansas Wesleyan University (KWU) has announced the winners of its 2025 Alumni Awards. Being honored this year with the Alumni Service Award is David B. Laha, DPM ’79. The award will be given at the Hall of Fame Induction and Alumni Association Award ceremonies at 9 a.m. Oct. 25, during Homecoming. A Biology major, Laha graduated with honors, laying the foundation for his medical career. He was a prominent figure in KWU athletics, excelling in basketball, track, and cross country. His dedication to these sports earned him induction into the Jerry Jones Kansas Wesleyan Athletic Hall of Fame for all three sports.

Dr. Laha established his podiatry practice in Overland Park, KS in 1985, where he has since become a respected specialist in the medical and surgical treatment of the foot. His expertise in reconstructive foot surgery and sports medicine of the lower extremity has made him a leading provider of care in the Kansas City area. He served from 2012 to 2020 on the Kansas State Board of Healing Arts and was the first podiatrist to hold the position of president of the board, from 2017 to 2018.

Physicians formerly working at Delaware County Memorial Hospital (DCMH) announced that they have provided grants to support ChesPenn Health Services in Upper Darby and the Share Food Program’s centralized food bank in Delaware County. Each $10,000 grant is made possible from funds the physicians established with their charitable contributions, now held at The Foundation for Delaware County.

From your online presence to your appointment scheduling process, preventive steps taken before the first visit can help minimize legal exposure and protect your reputation. Here are nine ways to get ahead of potential trouble.
5. Provide clear pre-visit instructions
Unclear directions about fasting, medication adjustments, or arrival times can cause delays, cancellations, or compromised care — all of which can become legal liabilities. Send written instructions via email or your patient portal, and confirm receipt before the appointment. Consistent communication also helps manage expectations.

Many financial documents include most or even all the information needed to steal your identity, like your name and mailing address, email address, bank account numbers, insurance policy numbers, and more. Shredding these financial documents once you no longer need them can help keep your information safe:

It's recommended to keep your bank statements for up to three years in case of any discrepancies, but older statements should be shredded once you no longer need them. After reading the post regarding the post-operative infection of a repaired Achilles tendon, two points come to mind.
1. If the graft and Achilles tendon repair was decimated during debridement of the post-op infection, would not this lead to a dorsiflexed position of the foot rather than equinus?
2. I find it extremely hard to believe that the patient could be walking 7 miles per day at only 4 months post-operatively following any type of Achilles repair, especially given the history of R.A.

I had a very similar patient after removal of a large retrocalcaneal spur and Speed bridge reattachment. There was about a 1.0 cm distal portion of her incision that probed fairly deep and did not respond to conservative wound care. I admitted her and placed her on IV Vanco, took her to the OR for a debridement, antibiotic pulse lavage, and packed the wound with an Orthobiologic graft and closed the wound primarily. She healed without incident.

RE: Have We Lost our Biomechanical Expertise?

I recently attended a seminar where third-year residents gave case presentations that left me with some concerns about the direction of our surgical training. While I was impressed by the third-year residents’ presentations on advanced surgical techniques—showcasing sophisticated hardware applications with screws, plates, and wires, along with comprehensive revisional care protocols —I noticed a troubling gap. Not once during these presentations was patient ambulation discussed. There was no mention of appropriate footgear or post-surgical gait considerations. When I raised questions about biomechanics during the Q&A—specifically about the need for rocker sole modifications following ankle fusion to optimize patient mobility—the residents seemed unfamiliar with these fundamental concepts.
This experience has me wondering: have we become so focused on surgical hardware and technique that we’re overlooking the biomechanical principles that have traditionally set our profession apart? The ability to achieve optimal functional outcomes for our patients has always depended on understanding not just how to perform surgery, but how our interventions affect the patient’s long-term mobility and quality of life.
I’m curious whether other practitioners have observed this trend. Are we adequately preparing the next generation of surgeons to think beyond the operating room to the patient’s functional recovery? I believe our profession’s unique strength lies in combining surgical expertise with biomechanical understanding. I hope we can maintain this holistic approach in our training programs. I’d welcome thoughts from colleagues who may have similar observations or different perspectives on this issue.

My question is, what was Dr. Gottlieb doing on Reddit if it's not a legitimate information resource?

Dr Gottlieb is "leery of a foreign medical school that advertises on Reddit and is also leery of anyone with a diploma from said college." How is advertising on Reddit different from having people attend health fairs, and high schools and colleges soliciting applicants to the many colleges of podiatric medicine? These institutions are all in business to train healthcare providers in various disciplines, not to remain passive and have empty seats abound in every incoming class.
Many students at AUC and other offshore medical schools transfer to stateside medical schools after their second year. Those who remain for the full program get residencies here and elsewhere, and they must pass the required examinations to obtain licensure in the United States. Heads up, Dr Gottlieb, these graduates are practicing all over the country, even here in Maryland. To quote Timothy Leary, who probably wouldn't be as leery as you, "Why not"?

The ABPM certainly understands and we empathize with the difficulties of becoming board certified by a CPME-recognized board if you are outside of the window proximate to residency or have less than 3 years of training, as required in the CPME Specialty Board Standards (CPME Document 220). Many of our colleagues have faced reduced job opportunities due to their lack of CPME-recognized board certification, despite the fact they completed CPME-approved residencies. Many are due to circumstances beyond their control, like starting families, an illness, or the effects of the pandemic. The ABPM has been a leader in advocating for our colleagues and several of the Board of Directors submitted a Policy Proposition to the APMA HOD in 2024 calling for CPME to allow specialty boards to create an alternate pathway for certification. This proposition (4-24) passed the HOD.
In May 2025, CPME published a revised Document 220 to require certifying boards to create an alternate pathway for board certification. These alternate pathways must first receive CPME approval. The ABPM BOD met last weekend and authored our eligibility criteria for this pathway and it will be submitted to CPME in the coming couple of weeks. As always, we believe that podiatric board certification should be a fair and equitable process and creating this alternate pathway is a step in the right direction.

RE: The Passing of Alan Shaw, DPM
I would like to share some comments regarding the passing of Alan Shaw. He and Doug Elleby gave me, along with many others, my first job in the Atlanta metropolitan area. He was involved in the leadership of the American College of Foot and Ankle surgeons, and his relationship with Gary Kaplan, who was involved in my residency program led me to this opportunity. Working in this practice taught me how to practice podiatry and manage the business of running a podiatry practice. This gave me the confidence and skills needed when it came time for me to run my solo practice.
Alan was instrumental in getting privileges in hospitals in Georgia by suing the local hospital that was denying him staff privileges. The change in the law from this suit benefited every podiatrist in the state from his actions. He was an innovator in managed care when HMOs or capitated plans became prevalent, by creating networks of doctors to participate in, and managing these plans. He moved to Montana when he retired to pursue his passion of skiing and fishing. He passed while doing one of the things he most loved doing. I am forever grateful for the opportunity he provided to me and the countless other doctors in Georgia.
